La dificultad que puede representar estudiar a pacientes con hipoglucemia organica constituye un reto en la practica diaria del especialista. Mas aun, cuando el resultado es totalmente opuesto al esperado. La elevacion en el nivel de insulina es la primera sospecha ante cualquier caso clinico que se estudia por hipoglucemia. Sin embargo, que hacer cuando la insulina es normal o baja? Este fenomeno es la regla al evaluar pacientes con proinsulinomas. Los niveles de glucosa en rangos de hipoglucemia grave con niveles de insulina totalmente normales o suprimidos. A pesar de sospecharse un origen no tumoral, el diagnostico en varios casos es, en realidad, un adenoma pancreatico secretor de proinsulina y el cual, puede pasar totalmente desapercibido. En este trabajo se describen cinco pacientes con esta dificultad diagnostica que sin duda resultaran de gran interes para el especialista. Ademas, se explica un posible mecanismo fisiopatologico. Al leer este libro se entendera claramente el abordaje diagnostico y tratamiento correcto de los proinsulinomas, evitando realizar estudios diagnosticos o tratamientos innecesarios a los pacientes con este tipo de tumor.
