This book aims to bring into focus the major advances in cardiac pacing over the last 5-6 years, constituting a valuable resource to update all those involved in the care of patients with implanted devices. The team of internationally respected contributors place particular emphasis on new techniques for the treatment of congestive heart failure. Advances in pacemaker technology are covered extensively, with a focus on the technical and clinical aspects of expanding pacemaker memory, the diagnosis of arrhythmias and pacemaker malfunction, and follow-up of device function. Contributors also discuss more controversial indications for pacing, particularly emphasising atrial fibrillation. Taking account of the new field of implantable cardioverter-defibrillation, and the new skills associated with ICD therapy, the text highlights the interface between pacemaker and defibrillator.

Atrial fibrillation is a common arrhythmia, difficult to treat pharmacologically and associated with significant morbidity and mortality. Although traditionally prescribed when pharmacologic therapy is not effective, not tolerated, or contraindicated, electrical therapy should no longer be considered as a treatment of last resort. Implantable devices such as cardiac pacemakers and internal defibrillators capable of monitoring atrial tachyarrhythmias have enhanced understanding of the mechanisms and natural history of atrial tachyarrhythmias. This knowledge allows electrical therapy to alter the natural course of atrial tachyarrhythmias early after onset or create an unfavorable electrophysiological environment in circumstances predisposing to their initiation.

Thus, electrical therapies are playing an increasingly important role in the overall management of atrial fibrillation and tachyarrhythmias, often in combination with antiarrhythmic drugs and sometimes with other strategies. New dual-chamber ICDs can safely and effectively differentiate atrial from ventricular tachyarrhythmias and treat atrial tachyarrhythmias without compromising detection and treatment of ventricular tachyarrhythmias. Technological advances in standard electrical cardioversion including the use of prior pharmacotherapy have facilitated the success of the procedure and maintenance of sinus rhythm. This is the first book devoted solely to the rapidly expanding options to treat atrial tachyarrhythmias electrically.

Electrical devices offer the promise of prevention and control of atrial tachyarrhythmias, with significant improvements in all health care professionals dealing with atrial fibrillation and otheratrial tachyarrhythmias.

Here is an essential text for cardiologists, heart surgeons, intensive care specialists and anyone interested in pacing. It is a comprehensive guide to contemporary devices used in the resynchronization of patients' heartbeats. The treatment of congestive heart failure by implanted biventricular pacemakers, or cardiac resynchronization, has revolutionized the practice of implanting pacemakers and defibrillators. More cardiac resynchronization therapy devices than conventional pacemakers are now being implanted and the numbers are growing worldwide.

"Rate Adaptive Cardiac Pacing" provides a comprehensive overview of this most advanced form of stimulating the heart by means of cardiac pacemakers that vary the pacing rate according to the needs of the patients. The heart rate is controlled by one or several sensors that detect various parameters such as respiration, blood temperature, oxygen saturation, intracardiac pressure, QT interval, stroke volume andbody activity. Besides describing the clinical and functional characteristics of these various sensors, the book also gives a clear understanding of hemodynamic aspects and in particular, all clinical issues of importance such as indications for rate adaptive pacing and selection of the appropriate patients for rate adaptive pacemakers, with many ECG samples. The book also covers new concepts in rate adaptive pacing such as single lead atrial synchronous pacing and the combination of various sensors, which are among the most recent developments in the field. With contributions from the most wellknown experts in the field from allover the world, this book is the first publication to cover all the hemodynamic, clinical and technical aspects of rate adaptive cardiac pacing.
