Airway management is a crucial skill to master and the first priority in the assessment of a critically ill or injured patient. Failure to secure an adequate airway can quickly lead to death or disability. Though primarily the remit of the anaesthesiologist, airway management also presents challenges to the surgeon, for example in obese patients or during surgery for thyroid cancer, particularly where reconstruction of the airway is involved. Management of the Difficult Airway: A Handbook for Surgeons is the first book designed to help surgeons with the issues they face maintaining a clear airway and preventing obstruction. It provides practical guidance not just on emergency situations, but also on complex, planned interventions where it is the surgeon who oversees the management of the airway, for example during surgery for congenital airway anomalies. Early chapters cover anatomy, imaging, indications and specific techniques; later ones cover airway situations in specific groups, for example patients requiring surgical reconstruction of the airway resulting from cancer. Management of the Difficult Airway provides a comprehensive, timely review of the core knowledge and clinical skills necessary for successful airway management in the surgical environment. Key Points Provides a 'go-to' skills manual for surgeons focussing on difficult airway management in surgical patients Covers surgical airway management issues in adult and paediatric patients Multidisciplinary author team combines the expertise of ENT and other surgeons, radiologists and anaesthesiologists
Within the past twenty years, the field of robotics has been finding many areas of applications ranging from space to underwater explo rations. One of these areas which is slowly gaining popularity among the users group is the notion of service robotics. This book is an in vestigation and exploration of engineering principles in the design and development of mechanisms and robotic devices that can be used in the field of surgery. Specifically the results of this book can be used for designing tools for class of Minimally Invasive Surgery (MIS). Generally, Minimal Invasive Surgery (MIS), e. g. laparoscopic surgery, is performed by using long surgical tools, that are inserted through small incisions at the ports of entry to the body (e. g. abdominal wall) for reaching the surgical site. The main drawback of current designs of en doscopic tools is that they are not able to extend all the movements and sensory capabilities of the surgeon's hand to the surgical site. By im proving surgical procedures, training, and more practice, it is possible for surgeons to reduce completion time for each task and increase their level of skill. However, even in the best cases the level of performance of a surgeon in Minimally Invasive Surgery is still a fraction of the con ventional surgery. Any dramatically improvement is usually driven by introduction of new tools or systems that in turn bring totally new pro cedures and set of skills."

Cancer surgery of the mouth and throat can involve major incision wounds, tracheotomy, or necessitate the breaking of bones. Recognizing the benefits and success of robotic surgery in other areas of medicine, the authors present its use in head and neck surgery, resulting in less surgery, less deformity, and faster rehabilitation. In this new book, the authors describe the preoperative aspects of Transoral Robotic Surgery (TORS), including patient evaluation, instrumentation and set-up, moving on to diagnostic endoscopic and surgical techniques for tonsillectomy, laryngectomy, tongue base and glottic resection, and speech and swallowing considerations. They conclude with an examination of the potential for TORS in skull-base surgery, its future application and, importantly, telesurgery via the Internet. A pioneering book for a pioneering surgical technique in head and neck surgical technique and management, this should be on the shelf of every otolaryngology-head and neck surgeon. http://www.youtube.com/watch?feature=player_embedded&v=DTp7Mv9hkbI

The teaching of radiological interpretation is a crucial element of
general surgical training and forms an important part of
postgraduate examinations. In this book there is a strong emphasis
toward the radiological assessment of the acute abdomen and also
the investigation of the gastrointestinal tract. There is
particular reference to the abdominal radiography which provides an
important initial diagnostic tool in the acutely ill surgical
patient. The book also contains sub-sections within each chapter
devoted to the other facets of surgical radiology including trauma
and orthopaedics, unoradiology, head and neck imaging,
interventional and breast radiology. Alongside key surgical points the text discusses further relevant imaging in each case. Numerous examples of barium contrast studies, ultrasound, CT and MRI scans are included to further illustrate the surgical cases chosen.
In this compilation, the authors begin with a focus on the indications for surgery in hepatocarcinoma and how to perform a liver resection. Long-term outcomes and different theories on the causes of recurrence after resection are also discussed. Most patients with hepatocarcinoma have underlying chronic liver disease and are thus at high risk for development of postoperative complications, which can sometimes be fatal. The authors propose that we may be able to prolong the overall survival of patients by using laparoscopic liver resection as a powerful local therapy, which can be applied repeatedly with minimal deterioration of liver function.Bronchoscopy has become an important procedure for diagnostic and therapeutic purposes, particularly in the context of lung cancer. As such, the medical and surgical advances in bronchoscopy are described herein.Post-operative pancreatic fistula is one of the most dreaded complications in any pancreatic operation given its associated morbidity and mortality. This collection examines how the amylase-rich fluid can cause inflammation, become infected, or cause serious bleeding from neighboring vasculature. The authors go on to discuss cystic fibrosis, the most common life-threatening autosomal recessive disorder affecting Caucasians. The indications of endoscopic sinus surgery in cystic fibrosis patients are discussed, drawing from personal experience in the diagnosis and treatment of cystic fibrosis-related sinus disorders.Following this, the origins of functional sinus surgery as it relates to the evolution of functional endoscopic sinus surgery and minimally invasive sinus technique are discussed. The rationale for our departure from Messerklingers functional concepts is also analyzed.Additionally, endoscopic sinus surgery complications should be distinguished in minor and major based on the treatment they need and the permanent sequelae they can cause. Over the years, the incidence of complications in endoscopic surgery has gone from 8% for the major and 21% for the minor to a percentage of 0 -1.5% and 1.1 - 20.8%, respectively.Sphenoid sinus is very close to important and vulnerable structures of the skull base and therefore a delayed diagnosis and treatment can be lethal. Thus, the authors provide a review of classification of transsphenoidal skull base approaches and show the main transnasal corridors to the sphenoid sinus based on the anatomical sphenoidal target and extension of pathology.Lastly, a synopsis of the pathophysiology of post-craniotomy temporomandibular dysfunction and a practical approach to its evaluation and treatment is provided. Several surgical procedures can be used to perform craniotomy, such as the classical pterional, pre-temporal and other fronto-temporal approaches. However, they are all based on the dissection of the temporal muscle, which can lead to a variety of negative side effects.

Meniscal repair and regeneration is a rapidly evolving science - early attempts at meniscal restoration or repair resulted in short-term gains which often sacrificed long-term joint integrity. Now, the practitioner is afforded numerous means of retaining or restoring meniscal tissue. Breakthroughs in scaffold and allograft replacement, as featured herein, offer the promise of articular cartilage preservation like never before. In addition, tissue regeneration and gene therapy techniques, featured throughout, offer a glimpse into emerging technologies aimed at preserving or replacing meniscal tissue in previously considered "hopeless" cases. As such, Meniscal Injuries will be an indispensible resource to orthopedic surgeons and sports medicine practitioners interested in providing the absolute most contemporary and evidence-based care to their patients. |
