The aim of this book is to give an up to date account of recent developments in the field of cardiomyopathy. Many authors, all experienced workers in their fields, - most from Germany - have contributed to this volume. It would appear that the diagnosis of cardiomyopathy is now made more often than it was 10 years ago. The reason for this is probably not that the incidence of the disease has increased, but that it is better recognized. Where conventional clinical investigation may have failed to define the underlying disease process in a given patient, improved or more widely used diagnostic tools, such as selective coronary arteriography and endomyocardial biopsies, have contributed greatly to proper diagnosis. Furthermore, uniformity of terminology has also taken place during the last few years, permitting more accurate categorization of patients suffering from this entity. The difficult subject of terminology is discussed in a special section.

The first cardiovascular operation in Duesseldorf, a ligation of an open ductus arteriosus (Botallo), was performed in 1938 by E. K. Frey. During the Second World War and the subsequent years many difficulties hindered cardiovascular surgery in Duesseldorf. However, it was resumed on 3 May 1949 by Prof. Ernst Derra, again by ligating an open ductus arteriosus. This was the starting pOint of the successful cooperation between the cardiological and surgical working groups - the "birthday" of the Duesseldorf Heart Centre. Further highlights in cardiac surgery were the introduction of open heart surgery by means of surface-cool- ing hypothermia (9 February 1955) and extracorporeal circulation (21 February 1959) . In addition to cardiac catheterization and surgery in patients with acquired and congenital heart disease, the diagnosis and treatment of arrhythmias was one of the focal points of the centre. On 3 October 1961 the first pacemaker was implanted in a patient with a third-degree AV-block after he had been treated using an external device for about 1 year. In recent years interest has increasingly turned to the tachy- arrhythmias. Close cooperation between the medical and surgical teams made possible the application of new pacing techniques for the treat- ment of tachycardia. In patients with intractable arrhythmias, surgical interventions were made to eliminate the "focus" of the arrhythmia. In the past 2 years intraoperative electrophysiologic studies (endo-cardi- al and epicardial mapping) were applied to enhance the success of the surgical procedures.

Hiermit liegt nun auch del' erste Teil (a) des Handbuchbandes X/2 VOl', nachdem del' zweite Teil (b) bereits VOl' 3 J ahren erschienen war. Nach pathologisch-anatomischen Vorbemerkungen von SCHOENMACKERS bzw. SCHOEN- MACKERS, DAHM, BIGALKE behandelt diesel' Teilband als zentrales Thema in einem um- fangreichen Beitrag von LOOGEN/SEIPEL/GLEICHMANN und VIETEN die erworbenen Herz- fehler. Es folgen, von KUHN/LoOGEN/BREITHARDT/SEIPEL und KRELHAUS bearbeitet, die Kardiomyopathien. SchlieBlich werden noch die Myokarditis und Perikarditis von GILL- MANN und JUNGBLUT besprochen. Auf den genannten Gebieten sind in den letzten 10 Jahren so viele neue, auch fiir die Rontgendiagnostik wichtige Erkenntnisse gewonnen worden, daB das verhiUtnismaBig spate Erscheinen dieses Teilbandes des Handbuches del' Medizinischen Radiologie lmines- wegs zu bedauern ist. Zu einem wesentlich friiheren Zeitpunkt hatten z.B. Spatergebnisse operativeI' Herzkorrekturen kaum beriicksichtigt werden konnen. Allen Autoren sei herzlich gedankt. Preface Following the appearance three years ago of the second part (b) of Volume X/2 of this handbook, we are now able to publish the first part (a). After some preliminary remarks on pathology and anatomy by SCHOENMACKERS, re- spectively SCHOENMACKERS, DAHM, BIGALKE the cetral theme of this part of the volume is discussed at length by LOOGEN/SEIPEL/GLEICHMANN and VIETEN: congenital cardiac defects. This is followed first by a discussion of the cardiomyopathies by KUHN/Loo- GEN/BREITHARDT/SEIPEL and KRELHAUS, and finally by a discussion of myocarditis and pericarditis by GILLMANN and JUNGBLUT.

Die beabsichtigte Dreiteilung des Bandes uber die Rontgendiagnostik des Herzens und der GefaBe (vgl. Vorwort zu Band X/3) hat sich als undurchfuhrbar erwiesen. Herausgeber und Verlag haben sich deshalb entschlossen, die angeborenen Fehler des Herzens und der groBen herznahen GefaBe in einem besonderen Teilband (X/4) zu be- handeln. Dies schien die sinnvollste Moglichkeit einer weiteren Unterteilung der gesamten Thematik. Wenn gerade die angeborenen Herz- und GefaBfehler in einem besonderen Teilband abgehandelt werden, so hat das mehrere Grunde; l. Es besteht keine zwingende Notwendigkeit, angeborene Fehlbildungen als "Er- krankungen" im ublichen Sinne aufzufassen. 2. Die Mannigfaltigkeit der Fehlbildungen ist so groB, daB ihre Besprechung selbst in knappster Form leicht den Umfang eines Bandes erreicht. 3. Die chirurgische Therapie der angeborenen Herz- und GefaBfehler wird immer einigen Spezialkliniken vorbehalten bleiben, und nur dort, wo gegebenenfalls auch operiert werden kann, sollte auch die notwendigerweise auBerst subtile und fur Patienten mit gewissen Herzfehlern keineswegs ungefahrliche Rontgendiagnostik erfolgen. Nur engste Zusammenarbeit zwischen diagnostischer und chirurgischer Arbeitsgruppe schutzt den Patienten vor unzweckmaBigen rontgendiagnostischen MaBnahmen und er- moglicht gute Behandlungsergebnisse. Band X/4liegt nunmehr vor. Seine Autoren widmen ihn Herrn Professor Dr. Dr. h.c. E. Derra zur Vollendung seines 65. Lebensjahres.
