Cancer chemotherapy frequently attacks the gastrointestinal tract. Many of the possible side-effects reduce the patient's quality of life, may limit acceptance of further drug administration, and even life-threatening complications must be considered. This book is the proceedings of the Falk Workshop 'Side-effects of Chemotherapy on the Gastrointestinal Tract: Pathogenesis and Therapy', held in Freiburg, Germany, on October 11, 2002 (Part IV of the Freiburg Gastroenterology Week). Chapters by pathologists, pharmacologists, oncologists and gastroenterologists will improve our understanding of the effects of anticancer agents on the organs of the gastrointestinal tract and help to compensate for chemotherapy-induced adverse effects.

Gastric Carcinoma/Classification, Diagnosis, and Therapy presents the most current perspective on gastric carcinoma, with particular emphasis on the surgical and chemotherapeutic modalities that offer hope for future treatment. The book discusses epidemiology, pathogenesis, and precancerous and clinical stage classification of the disease and provides valid practical guidelines for stage-specific diagnosis, therapy, and patient guidance. Topics explored include endoscopic criteria for premalignant lesions and early gastric carcinoma; clinical staging of gastric cancer by ultrasound, computerized tomography, and magnetic resonance tomography; surgical treatment of carcinomas of the gastroesophageal junction; possibilities for palliative treatment in surgical practice for advanced tumors; adjuvant treatment of gastric cancer; and chemotherapy of advanced gastric carcinomas in elderly and high-risk patients.

Die Beschaftigung mit dem Magenkarzinom ist in den letzten Jah- ren von der intemistischen Gastroenterologie vemachlassigt worden. Dies laBt sich an den Schwerpunktthemen der nationalen und intemationalen gastroenterologischen Veranstaltungen ablesen, die die b6sartigen Erkrankungen des Magen-Darm-Traktes - abgese- hen yom Kolonkarzinom - weitgehend ausklammem. Dies mag zum Teil daran liegen, daB die endoskopisch-bioptische Diagnostik des Magenkarzinoms einschliel3lich der Suche nach dem Friihkar- zinom ausgefeilt ist und wenig neue wissenschaftliche Substanz herzugeben scheint. Zum anderen sind die konservativ-therapeuti- schen Bemiihungen beim inoperablen oder nicht kurativ operierten Patienten in vielen Fallen frustran gewesen und gaben AnlaB zu einer eher fatalistischen Haltung des intemistischen Gastroentero- logen gegeniiber dieser Erkrankung. 1m Gegensatz dazu bestimmt das Thema "Magenkarzinom" nach wie vor in hohem MaBe das Interesse der allgemeinchirurgi- schen Disziplin auf der Suche nach einem adaquaten operativen Vorgehen. In jiingster Zeit hat sich - oft unbemerkt von der Gastroenterologie - die intemistische Onkologie auf diesem Gebiet vorwartsbewegt und erste echte chemotherapeutische Erfolge errungen. So besteht heute kein Zweifel mehr daran, daB das Magenkarzinom eine relativ hohe Chemosensitivitat gegeniiber Zytostatika besitzt im Vergleich zu den anderen Tumoren des Gastrointestinaltraktes wie dem Kolon-, Pankreas-, Gallengangs- und Leberkarzinom. Wirksame Palliation in fortgeschrittenen Sta- dien sowie kombinierte operativ-chemotherapeutische Therapie- modalitaten sind hoffnungsvolle Zukunftsperspektiven.
