Cerebrovascular disease is today the cause of death in a fifth of the population in countries of modern technology. It seems legitimate that society expects medi cine to analyse the causes of this menace and, as far as it is possible, to cure its sequelae and prevent its origin. Even when the patients survive, sequelae such as herni-or monoplegia, aphasia and visual deficits severely lirnit the mode of life of those afflicted. This book tries to show the methods by which medicine is striving to investi gate and analyse the causes of this important condition-cerebrovascular disease. The report which follows constitutes the proceedings of a sym posium on the classification of brain tumours held in Cologne in 1961. It contains the contributions and discussions reported as far' as possible in a word-for-word manner, in the hope not only of providing out of the thoughts of the participants stimulation for further research, but also to indicate the atmosphere which prevailed during the symposium in which many completely opposed views were able to be expressed without any feeling of embarrassment. This freedom of expression is already obvious in the discussion of a proposed classification of brain tumours for the Unio Internationalis Contra Cancrum (UICC). The symposium endeavoured here to pro duce a positive criticism in order to arrive at a significant and practical conclusion viz a general, internationally acceptable classi fication. It was necessary to accept a degree of compromise even if the classification suggested did not in every point correspond to all the personal views of the various participants. The second part of the symposium was concerned with a critical discussion of numerous brain tumour problems, for example the scientific delineation of tumour groups; the discussion was started by general reviews on the biology of brain tumours so that the later discussion could cover as wide a ground as possibly. Here also we all felt that a great stimulus for future work arose from the discussion."

Die erste Auflage dieses Buches hat sich einen Platz bei den AErzten erworben, die das neuroradiologisch gewonnene Bild bei Hirnkrankheiten nicht nur mit seinen Abweichungen erfassen und beschreiben, sondern auch seine Genese verstehen wollten. Diese Form der Darstellung hatte im In-und Ausland viele Freunde gefunden und zahlreiche Schemata sind inzwischen in Lehr-und Hand bucher ubernommen worden. Jedoch ist die erste Auflage im Laufe der Zeit durch die rasche Weiterentwicklung der Neuroradiologie uberholt. Die Verfas ser, die noch selbst als Kliniker neuroradiologisch gearbeitet hatten, mussten dieser Entwicklung Rechnung tragen. Es wurden zwei hauptberuflich arbeitende Neuroradiologen zur Erweiterung und UEberarbeitung der Themen hinzuge beten. Einige Kapitel, wie die Hirnpunktion und der Farbversuch, haben in zwischen ihre Bedeutung verloren und wurden fortgelassen. Die Szintigraphie, die Hirndurchblutungsmessung und die in der Entwicklung begriffene Computer Tomographie wurden nicht abgehandelt, da sie ausserhalb des hier gesteckten Rahmens der neuroradiologischen Diagnostik liegen. Trotz mancher Anregun gen wurde auch weiter bewusst auf die Besprechung des Nativbildes von Schadel und Wirbelsaule verzichtet, sie hatten den Rahmen dieser Bearbeitung gesprengt. Andererseits musste die Myelographie und die neu entstandene radiologische Pathologie der cerebrovaskularen Stoerungen mitberucksichtigt werden. Die ubrigen Kapitel wurden dem heutigen Stand angepasst. Die Bilder wurden grossenteils erneuert. Doch hat sich an der Grundlinie der Darstellung nichts geandert. Das Buch soll auch in seiner vorliegenden Form eine Einfuhrung in die Besonderheiten der neuroradiologischen Diagnostik intrakranialer und spinaler Erkrankungen sein, die die Ergebnisse auch dem Kliniker verstandlich machen koennen.

zum Beitrag "Biologie und Pathologie der Hirngeschwulste". Die Tumoren des Schadelinneren stellen wohl, verglichen mit denen aller anderen Koerperhoehlen, die groesste Zahl verschiedener Typen. Unabhangig von den biologischen Eigenschaften des Tumorgewebes hat raumforderndes Wachstum am Hirn oder seinen Bedeckungen innerhalb der starren Schadelkapsel besondere Konsequenzen, wie sie sich so unmittelbar und lebensbedrohlich nirgends sonst ergeben koennen. Diese Tatsachen kennzeichnen die Sonderstellung der intrakraniellen Geschwulste unter den Blastomen. Pathologisch-anatomische und klinische Besonderheiten haben fruhzeitig spezielle Untersuchungstechniken erfordert, deren sich meist der Kliniker annahm; oft lag die Bearbeitung also nicht mehr in den Handen der Allgemeinen Pathologie. Diese Ent wicklung erfolgte zum Nutzen, aber auch zum Schaden der Sache, ahnlich ubrigens, wie in der Gynakologie. Denn hier wie dort stellte die Klinik besonders geartete Fragen, die vom Pathologen kaum zu beantworten waren, und hier wie dort musste die Loesung vom allgemeinpathologischen Denken besondere Gefahren mit sich bringen. Diese Ge fahren hat unter den Neuropathologen wohl SPIELMEYER besonders deutlich gesehen und in seinen Arbeiten zu vermeiden gesucht. gehoert einerseits der all Die Untersuchung der Hirngeschwulste in den Rahmen gemeinen und speziellen Cancerologie, andererseits ist sie eine Hilfswissenschaft der Klinik. Die Klinik verlangt von ihr Auskunfte, die unmittelbar praktische Bedeutung bei Prozessen haben. Neben dem Problem der Massenverschiebungen raumfordernden Stelle.
