This book is devoted to all the aspects of pediatric minimally invasive surgery and is written under the patronage of the European Society of Pediatric Endoscopic Surgery (ESPES) with the participation of leading international experts on Pediatric MIS. Comprising more than 50 chapters, the book begins with an introductory section describing the general and technical aspects of MIS approaches including laparoscopy, thoracoscopy, retroperitoneoscopy and robotic surgery. The main part of the book is divided into five subsections, each of which focuses on a specific system: thorax, abdomen, urology, gynecology and varia. For each subsection, the book examines several pathologies, accurately describing their clinical and diagnostic aspects and providing detailed information on the operative techniques, tips and tricks used in their treatment. Further, the book addresses potential complications in MIS and better ways to manage and prevent them. The volume will be of interest for pediatric surgeons, pediatric urologists or other professionals that need to access accurate descriptions of the MIS approaches adopted for the different surgical pathologies. At the same time, it addresses the needs of novices, including trainees, looking for general information on the management of the various diseases encountered in the pediatric population.
The first edition laid out the foundation with laparoscopic and robotic surgery utilizing the Da Vinci SI platform. Since then, many new advances in equipment and surgical techniques are becoming more popular. This second edition expands upon laparoscopic and endoscopic techniques and robotic surgery with the use of the new Da Vinci XI platform. This book bridges the gap between the practicing community of surgeons and the surgical innovators and provides a foundation for all classic and new techniques in minimally invasive colorectal surgery. By enhancing the surgical toolbox, the surgeon is able to progress from the novice to the master. Rather than describing the entire operative procedure by an individual author, this book compares operative steps of various technical difficulties throughout different chapters, thereby allowing the surgeon to tailor surgery to patient and surgeon`s own comfort level and experience. Chapters are written by a myriad of renowned experts in the field and discuss the major advances in advanced laparoscopic and endoscopic, robotic, and transanal minimally invasive surgical techniques. Great emphasis is placed on transanal total mesorectal excision (TaTME), which is dramatically changing the surgical approach to rectal resections. The second edition of Advanced Techniques in Minimally Invasive and Robotic Colorectal Surgery serves as a valuable resource to general surgeons, colon and rectal surgeons, minimally invasive surgeons, as well as residents and fellows.
Learn the fundamental principles and clinical applications of the diagnostic and risk assessment imaging test of choice for prostate cancer: prostate MRI Although prostate cancer is the second leading cause of cancer death in men in the USA, it can be treated successfully if detected early. Disease management has gradually changed to a paradigm that relies on close monitoring through active surveillance in select patients, as well as ongoing refinements in treatment interventions, including minimally invasive procedures. This has resulted in a critical need for a more exacting methodology for performing targeted biopsies, assessing risk levels, and devising treatment strategies. Prostate MRI has emerged as the most precise, state-of-the-art imaging modality for prostate cancer diagnosis and management, thereby creating an immediate demand for radiologists to become proficient in its use. Conceived and edited by a leading authority, with contributions from renowned experts in the field, MRI of the Prostate: A Practical Approach is the first book to tackle this important topic. It provides an overview of the fundamentals of prostate MRI acquisition, interpretation, and reporting. Readers will benefit from a wide range of insightful perspectives gleaned from years of hands-on experience. Key Highlights Prostate Imaging Reporting and Data System (PI-RADS) for prostate MRI interpretation and cancer risk scoring Clinical pearls on the optimization and application of prostate MRI for risk assessment, disease staging, MRI-targeted biopsy, recurrent disease, and active surveillance The emerging utilization of PET and PET/MRI for primary prostate cancer evaluation More than 700 illustrations with one entirely image-based chapter featuring educational case studies Radiologists will learn how to optimally perform and interpret prostate MRI, and referring physicians will learn to integrate it into day-to-day practice. This book is an essential resource for radiologists and radiology residents, as well as urologists, oncologists, MRI technicians, and other medical practitioners who treat patients with genitourinary disorders.
Functional urology is that part of urological practice which deals with functional disorders of the lower urinary tract. Among the diseases that cause functional disorders are spina bifida, spinal cord injury, pelvic organ prolapse, and urethral strictures. This book on practical functional urology will help urologists, urogynecologists, and others to diagnose and treat these functional disorders. The theoretical background and scientific basis are summarized, but the emphasis is very much on clinical practice. Diagnosis and treatment are presented for a variety of conditions, including overactive bladder, detrusor underactivity, painful bladder syndrome/interstitial cystitis, prostatitis, stress urinary incontinence, pelvic organ prolapse, urinary tract infections, and urethral disorders. The content is based on the EAU guidelines, and for each disorder, helpful tips and tricks are highlighted.
Uroscopy - the diagnosis of disease by visual examination of the urine - played a very prominent role in early modern medical practice and in the lives of ordinary people. Widely considered as the most reliable way to diagnose diseases and pregnancies it was taught at the best universities. Leading physicians prided themselves on their mastery in this field. Countless medical writings were dedicated to uroscopy and artists represented it in hundreds of illustrations and paintings. Based on a wide range of textual and visual sources, such as autobiographies, court records, medical treatises and genre painting, this book offers the first comprehensive study of the place of uroscopy in early modern medicine, culture and society and of the - gradually changing - ways in which medical practitioners, lay persons and, last but not least, artists perceived and used it.

Although impotence may be the most widely recognized manifestation of male sexual dysfunction, many other forms of sexual disorders do not involve the erectile mechanism, from deficiencies of desire to disturbances in ejaculatory function to the failure of detumescence. With such a myriad-and often co-existing-number of disorders, the successful treatment of male sexual dysfunction requires not only a thorough understanding of the underlying physiology and pathophysiology, but also the coordinated efforts of multiple specialties, including endocrinology, andrology, urology, radiology, sex therapy, and even sometimes psychiatry, cardiology, or oncology. Male Sexual Dysfunction: Pathophysiology and Treatment presents the collective expertise of more than 60 international authorities in a single landmark text. From foundations in the anatomy of the male genital tract to the latest neuroimaging data, readers will appreciate the comprehensive information detailing the tremendous advances made in the delineation of sexual function and its disorders as well as the expert descriptions of practical and cost-effective medical, surgical, and psychological strategies for the treatment of all forms of male sexual dysfunction.
The problems a transplant patient faces do not stop once the surgery is over; in addition to problems with the transplant itself, the host body reacts in a number of different ways to the arrival of a new organ and the immuno suppression required to sustain it. This pioneering text from an internationally known expert on the kidney details the problems that a patient faces with a kidney transplant and looks at how the physician can treat these; it will be an essential resource for all those involved in the initial and continuing treatment of kidney transplant patients.
As prostate cancer is the most common type of cancer found in men, the clinical and surgical commitments to treating it are enormous, as well as to be expected given the aging male population. Urologic cryotherapy is one such method for those with organ-confined or minimally-spread prostate cancer. The benefits it offers both patient and surgeon are hugely significant: it is minimally invasive, has a favourable success rate and complication rates, is followed by a short period of recuperation, is repeatable if necessary, and costs considerably less than a radical prostatectomy. Presenting their adroit surgical and extensive teaching experience with urologic cryoablation in the treatment of renal and prostate cancers, Drs. Daniel Ruskstalis and Aaron Katz, along with a superb team of U.S. contributors, share their mastery of this revolutionary technique with the reader, whether qualified or in-training. An important, accessible guide to a ground-breaking advance in the treatment of prostate cancer, Handbook of Urologic Cryoablation is requisite reading for those urologic surgeons wishing to perform cryoablation effectively and efficiently for the patient's benefit.

In this remarkable book, Dee Howe, wife of the nation's leading lay expert on prostate cancer, offers a frank, witty and information-rich examination of her husband's and her experience with this frightening disease, from diagnosis all the way through successful treatment for post-surgical side effects. In clear and readable prose, she delineates the many new and effective treatment options available and offers details of the numerous resources existing to provide a couple with support in their fight. When caught early, prostate cancer is highly curable. Out of fear for the potential side effects of treatment, however, many men forego the simple PSA blood test that could indicate the disease in its most treatable stages. Howe argues persuasively against this head-in-the-sand approach by supplying facts that work to counter the fear. A prostate cancer diagnosis does not foretell a pleasant experience; but neither does it mean the end of a vigorous life for a couple. New techniques have made treatment, especially treatment of early stage cancer, less traumatic. New devices and drugs have markedly decreased the permanence of dreaded side effects such as incontinence and impotence. The process of diagnosis and therapy, however, is complex and confusing. Howe leads the reader through the maze with the latest information and helpful tips on coping gathered from her husband's and her experience, as well as that of others. With an irrepressible sense of humor coupled with a profound religious faith, Howe writes at a high level of technical competence, interweaving up to date information with her compelling human-interest story. Her extraordinary frankness about the difficulties sheand her husband transcended is inspirational. Every wife, partner, or loved one of a man diagnosed with prostate cancer will derive tremendous insight, guidance, and hope from the Howes' story.
There have been many exciting advances in our understanding of
mammalian sex determination and differentiation in the last decade.
Using these advances to elucidate clinical conditions of abnormal
sexual development, the authors bring together great expertise in
molecular endocrinology, molecular genetics, and dysmorphology. The
text begins with a discussion of normal gonadal and sexual
development that presents enough embryology, biochemistry, and
endocrinology to make the remaining chapters easy to assimilate.
Then the authors discuss overarching clinical issues that are
common to genetic abnormalities of gonadal and sexual development,
providing a detailed account of genetic causes of gonadal
maldevelopment, followed by a compendium of the singular,
syndromal, endocrinologic, and systemic-metabolic genetic causes of
sexual maldevelopment. The final section describes genetic forms of
gamete failure.
This book consists of 45 chapters written by leading international authorities. Each chapter will analyse the available literature to produce an authoritative statement about the management of urological disorders. There will be a concise summary of the best available scientific evidence providing up-to-date information. Individual doctors cannot read everything that is published and cannot possibly synthesise what they do read, so that they are not always aware of what is proven to be effective and what probably is not. The Evidence for ... series seeks to fill that need. In The Evidence for Urology we have commissioned systematic reviews of the common operations and some innovative treatments, to present the facts to busy doctors. Where is there and where is there not evidence?
The topics covered in this book have been specifically chosen to give guidance to surgeons established in practice and those embarking on their careers. Examples include the changing management in the treatment of the catastrophic abdominal wall, perianal Crohn's disease in the biological era, decision making in rectal cancer that responds to radiotherapy, and the assessment of outcomes in colorectal cancer surgery. Recent years have seen the introduction of technical advances with respect to robotics in colorectal surgery and trans-anal total meso-rectal excision, and which are rapidly becoming established in clinical practice. Chapters also cover those aspects of colorectal practice, which are common to all, and provide guidance on management of topics including complications of radiotherapy, management of pouch dysfunction and iatrogenic complications in pelvic cancer surgery.
The editors of this comprehensive third edition of the Textbook of the Neurogenic Bladder have assembled an impressive team of world specialists to develop an essential resource for physicians, continence specialists, and other health care professionals involved in the diagnosis and management of patients who have lost normal bladder function. The book provides a succinct update on epidemiology and briefly covers lower urinary tract anatomy and physiology. This is followed by a discussion on the pathophysiology of various types of dysfunctions and a clinical section that focuses on evaluation and treatment. Different types of treatments from behavioral to surgical are reviewed, including the most recent and sophisticated pharmacotherapy and tissue engineering approaches. A special section is devoted to myelomeningocele, a congenital condition with lifelong repercussions on voiding. Other topics such as complications, sexual function, fertility, maternity aspects, and prognostic factors round off the book. Each topic is covered in detail by several authors for a better overall understanding. A selection of reports and guidelines in relation to neurogenic bladder dysfunction is also included. This revised edition builds on the strong reputation of its predecessors. It is an essential reference tool for urologists, nurses, and technicians involved in the care of patients with neurogenic bladder dysfunction.
How far would you go to have a baby? Making Babies the Hard Way is a frank account of one couple's discovery that they cannot have children of their own, and their ensuing struggle through four years of fertility treatment. One in six couples worldwide seek assistance to conceive and 80 per cent of couples undergoing fertility treatment are currently unsuccessful. Writing with humour and honesty, Caroline Gallup describes the social, emotional, spiritual and physical impact of infertility on her and her husband, Bruce, including feelings of bereavement for the absent child, the unavoidable sense of inadequacy and the day-to-day difficulties of financial pressure. As well as telling her own moving story, she also offers information and guidance for others who are infertile, or who are considering or undergoing treatment. This courageous and poignant book will be of interest to couples who cannot conceive and those who are undergoing treatment, as well as their families and friends.

This complete, comprehensive, and current compendium presents diagnosis and treatment of the entire spectrum of anorectal diseases. The focus is almost exclusively on anorectal conditions, leaving colonic diseases to other texts. This emphasis is necessary as anorectal pathology is often more complex and challenging than colonic pathology, for the surgeon. The text reviews anorectal anatomy, physiology, and embryology as a foundation to a detailed description of preoperative, intraoperative and postoperative patient management. All surgical procedures are shown in step-by-step detail by world-renowned surgeons and gastroenterologists.
