Tumour therapy depends essentially on being able to destroy the clonogenic activity of tumour cells while keeping the damage to the normal tissue low. Clinical experience shows that tumour response varies greatly even if tumours with the same localisation, clinical, and histopathological staging are compared. Some tumours appear to be resistant to conventional radiotherapy (X-rays, y-rays or fast electrons) or chemotherapy. In these cases new therapy modalities are necessary. Combined therapy modalities seem to have advan- tages for some resistant tumours; one possibility of such a treatment is to combine radiotherapy or chemotherapy with hyperthermia. This means that the local tumour, the tumour region or even the whole body of the patient has to be heated to temperatures between 40 to 45 C (in case of whole body hyperthermia to 42 C maximal) for a certain time (usually 30-60 min are adequate). Hyperthermia has a long tradition in medicine as a treatment modality for various diseases. Inscriptions of the old Egyptians and texts of the Greeks have pointed out its importance. Usually whole body hyperthermia has been used by the induction of fever. Local hyperthermia began around 1900 when Westermark treated unre- sectable cervix carcinomas with hot water in a metallic coil. By the beginning of this century an increase of radiation effects was hy- pothesised with hypothermia and later observed. However, only in the 1960s and 1970s were systematic investigations started which showed radiosensitisation and chemosensitisation by hyperthermia in cells and tissues including tumours.

Die Haufigkeit nosokomialer, d. h. krankenhauserworbener Infektionen hat in den letzten Jahren standig zugenommen. Das bedeutet nicht, daB das Klinikpersonal hygienisch nachiassiger geworden ist, sondern daB unsere Patienten vor allem auf Intensivstationen, immer infektionsanfal liger werden und die Eingriffe, die wir bei ihnen vornehmen miissen, urn deren Oberleben zu ermoglichen, eine immer groBere Infektionsgefiihr dung mit sich bringen. Gleichzeitig muBten wir in den letzten J ahren akzeptieren, daB verschiedene, z. T. sehr aufwendige und teure MaB nahmen (z. B. Umbauten, Schleusen, Raumdesinfektionen in regelma Bigen Abstanden) nicht den Erfolg brachten, den wir gewiinscht hatten, namlich eine Senkung der Infektionsrate. Andere MaBnahmen, wie z. B. Handewaschen und Handedesinfektionen, sorgfiiltige pflegerische Techniken, Desinfektion von Instrumenten und Beatmungszubehor, Sterilisation, spielen in der Infektionskontrolle eine weit wichtigere Rolle. 1m vorliegenden Buch haben wir neben der zuletztgenannten Methode der Isolierung von infizierten Patienten einen relativ groBen Raum zuge messen, da diese MaBnahme die Infektionsgefahrdung nachweislich sen ken kann, auch wenn viele der erwahnten Erkrankungen auf Intensiv stationen nur selten vorkommen. Nicht geauBert haben wir uns zu Anti biotika, obwohl ohne Einschrankung des Antibiotikagebrauchs, insbe sondere fiir ungezielte Prophylaxe, Infektionskontrolle liickenhaft ist. Dieses Thema haben wir in einem anderen Buch ausfiihrlich behandelt. Wir mochten die Leser dieses Buches bitten, uns Hinweise zu geben, an welcher Stelle wir Erganzungen oder Verbesserungen anbringen konn ten, damit wir moglichst praxisnahe bleiben und die Neuauflage entspre chend aktualisieren konnen."

Wichtig erschien es, die fur den prakti- schen Strahlentherapeuten grundlegenden physikalischen und strahlenbiologi- schen Fakten in etwas breiterer Form zu bringen. Eingehendere Ausfiihrungen tiber die gegewartig getibte Chemotherapie und auch die Grundtatsachen der Immunbehandlung waren eigentlich notwendig gewesen, hatten aber gleichfalls den Rahmen dieses Buches gesprengt. Es wendet sich vor allem an den angehen- den Strahlentherapeuten, fur den erfreulicherweise soeben eine entsprechende Facharztbezeichnung geschaffen worden ist, die hoffentlich dazu beitragen wird, die Etablierung eigenstandiger radiotherapeutischer Kliniken und Kranken- hausabteilungen zu beschleunigen.
