Objective structured clinical examinations/exercises (OSCEs) using standardized patients (SPs) are an efficient means of surveying a diverse range of ability at any point along the continuum of medical education. This book is a guide for the medical student or recently graduate MD preparing to take the third part (Step 3) of the United States Medical Licensing Exam. As anyone wishing to practice medicine in the United States must take and pass this examination, the potential market is a very large one, with 16,000 new medical school graduates eligible for the exam every year. There are a considerable number of books for the Step 1 Exam (basic sciences) and the Step 2 Exam (clinical medicine) but very few for the Step 3 Exam. It had been common to study for Step 3 using Step 2 materials since both cover clinical medicine, but in recent years the Step 3 exam has separated itself from Step 2, focusing on clinical management of specific medical conditions in a case-scenario fashion. As a result, the few available Step 3 books have become very successful, largely by default. Kevin Schwechten, who has recently taken the Step 3 exam himself and has evaluated the other competing books, has come up with an approach focusing heavily on those questions most commonly asked on the exam, and he follows an outline that is provided by the National Board of Medical Examiners, which is the group that writes the exam. As those taking the exam have just recently graduated from medical school and have been thrown into a very hectic schedule on the hospital wards, with long housr, little sleep, and little time for studying, an effective book for Step 3 must be concise and to the point. Endoscopic access to the small bowel has advanced significantly since the introduction of video capsule endoscopy and deep enteroscopy in early 2000. Other major advances have occurred in imaging modalities involving computed tomography and magnetic resonance studies. This book is for all those entering general surgical training. It explains the reasoning involved in recognizing and dealing with general surgical conditions and pays special attention to managing surgical emergencies and trauma patients. It is also intended to serve as a practical guide to safe and effective day-to-day surgical practice. The content is laid out logically so that a patient can be followed from a working diagnosis, through investigation and assessment, preparation for surgery, care in the operating theatre and afterwards, including the prevention and treatment of complications. The vital signs are, by definition, the measurements of the body's most basic functions. So far, the essential number of signs that must be measured is not clear as some references consider that the body temperature, blood pressure, pulse rate and breathing rate are sufficient but other sources consider that the measurement of other variables, such as the respiratory effort and oxygen saturation, are also crucial to determine the health of the person. This book is not intended to define the critical vital signs but presents the correlation of these measurements with other variables as well as the proposition of new methods to determine these variables. It also proposes some innovative applications that relay on these signs in a way to monitor and help the human being. Diagnostic radiology plays a vital role in patient management and all clinicians need to be able to recognize the radiological appearances of many medical conditions. Not only are traditional imaging techniques important, but newer techniques such as interventional radiology, computed tomography (CT), magnetic resonance imaging (MRI), nuclear medicine and ultrasound are increasingly important in clinical practice and also, the interpretation of radiological images is an integral part of professional examinations for MRCP (part II): there are five compulsory questions based on printed photographic material, with additional radiographs or other diagnostic images being shown occasionally throughout. Rapidly Progressive Glomerulonephritis is one of the most exciting areas in renal medicine. However, appropriate immunosuppressive therapy can dramatically improve the prognosis in many cases, such that renal failure is avoided. It is clear now that various immunopathological processes are involved, and that an accurate diagnosis is of value in guiding management. This volume aims to bring together current knowledge of both scientific and clinical aspects of RPN. Recent advances in the immunology, inflammatory mechanisms and pathology of RPGN are discussed. This is followed by consideration of the major causes of RPGN, which include Goodpasture's disease, primary systematic vasculitis and systematic lupus erythematosus. RPGN secondary to other renal and systemic diseases, and RPGN in children are also described. Finally, new approaches to treatment are reviewed. Although certain of these areas are covered in the larger textbooks of renal medicine, our aim is to provide a current overview of RPGN, in a concise volume. This should be of particular interest to nephrologists and general physicians, including those in training, but may also be of value to pathologists, immunologists, and other studying the mechanisms of renal disease. We hope that this volume will help them in the management of patients with RPNG, or in planning their research into this condition.
Dialysis amyloidosis is a syndrome observed in dialysis patients characterized initially by joint pain, followed later by joint destruction. It is at present one of the few compliations in these patients. The factors involved in the genesis of this syndrome are not yet known with certainty and there is as yet no proven efficacious treatment otehr then renal transplantation. As a result, the interest in this topic among nephrologists remains very high. Dialysis amyloid gives general information on the pathophysiology, and the clinical aspects of all types of amyloidosis.
