During the recent transition between acute diseases caused by swarms of single planktonic bacteria, and chronic infections caused by bacteria growing in slime-enclosed biofilms, a general clinical consensus has emerged that pathologies with bacterial etiologies are frequently culture negative. Because biofilm infections now affect 17 million Americans per year (killing approximately 450,000), the suggestion that these common and lethal infections regularly go unnoticed by the only FDA-approved method for their detection and characterization is a matter of urgent concern. Biologically, we would expect that planktonic bacterial cells would colonize any new surface, including the surface of an agar plate, while the specialized sessile cells of a biofilm community would have no such proclivity. In the study of biofilm diseases ranging from otitis media to prostatitis, it was found that direct microscopy and DNA- and RNA-based molecular methods regularly document the presence of living bacteria in tissues and samples that are culture negative. The editors selected orthopedic biofilm infections as the subject of this book because these infections occur against a background of microbiological sterility in which modern molecular methods would be expected to find bacterial DNA, RNA-based microscopic methods would be expected to locate bacterial cells, and cultures would be negative. Moreover, in Orthopedics we find an already biofilm-adapted surgical group in which current strategies are based on the meticulous removal of compromised tissues, antibiotic options as based on high biofilm-killing local doses, and there are practical bedside strategies for dealing with biofilm infections. So here is where the new paradigm of biofilm infection meets the equally new paradigm of the culture negativity of biofilms, and this volume presents a conceptual synthesis that may soon combine the most effective molecular methods for the detection and identification of bacteria with a surgical discipline that is ready to help patients.

This book reviews the main topics in thromboprophylaxis around orthopedic surgery, from a general scope of the problems with the disease highlighting them in orthopedics to the new specific protocols involving, for example, new oral anticoagulants. The prevalence of the venous thromboembolism in each procedure (from "easy" to "hard" surgeries, with different rates of related thrombosis) and the risk factors to bear in mind in each one (related and non-related with the orthopedic procedure) are also revised. A chapter focus on the diagnosis and treatment of venous thromboembolism, which is commonly "forgotten" in many books addressed to orthopedic surgeons and anaesthesiologists. The methods for thromboprophylaxis have three specific chapters: the most common drugs used and recommended when pharmacological prophylaxis is needed, new drugs which are arising day by day and which management will be of main importance in a close near future, and mechanical methods, recommended both as additional when possible and for sole indications when the risk of bleeding could move us to minimize the real risk of thrombosis. Anaesthetic implications for thromboprophylaxis and, also, main implications of the application of antithrombotic protocols in the anaesthetic practice are covered by another chapter. Worldwide experience with the Lcs* mobile bearing total knee prosthesis has been unpar alleled both in terms of enduring popularity and outstanding long-term clinical results. Buechel and Pappas's design was based on the principles of; restoring anatomical joint function to as near normal as possible, minimising contact stresses to avoid wear and darn age to the bearing surfaces. and finally the idea that constraint should refiect the need for mobility, to avoid shear stresses and loosening of the implant. In 1977, the LCS (R) knee was implanted by Dr. Frederick Buechel. This was the first mobile bearing, tri-compartmental knee implant. This was also the first to successfully address the key issues ofloosening, wear and patello-femoral problems associated with earlier designs. The unique design solution was the creation of a common articulating geometry for the tibia and patella on the distal femoral surface. This resulted in a tibial and patellar articulation that was mobile in nature, but with an identical radius of curvature and conformity. The mobile bearing concept was considered sufficiently novel and unproven that the US FDA (Food & Drug Administration) required that it be validated in an Investigational Device Evaluation (IDE). An FDA IDE study involving 25 US surgeons was initiated in 1981. Validation of the clinical success of the device in this study resulted in FDA approval of the LCS, Knee (for cemented, tri-compartmental use) in 1985.
