This book is written for the users and designers of joint replacements. In its second extended edition it conveys to the reader the knowledge accumulated by the authors during their forty year effort on the development of replacement devices for the lower limb for the purpose of aiding the reader in their design and evaluation of joint replacement devices. The early chapters describe the engineering, scientific and medical principles needed for replacement joint evaluation. One must understand the nature and performance of the materials involved and their characteristics in vivo, i.e. the response of the body to implant materials. Total joint arthroplasty is an effective surgical procedure for end-stage osteoarthritis of major joints with satisfactory long term clinical outcome. This book addresses the background and significance of the factors potentially influencing the clinical and biological outcomes of metal-on-metal hip implants. Metal-on-metal bearings were introduced and evaluated as an alternative to other bearing couples, particularly metal-on-polyethylene, due to their enhanced wear resistance as determined in laboratory testing. Initially, reports of short-term clinical outcomes were favorable and an increasing number of metal-on-metal prostheses were implanted. Neglected finger injuries may lead to chronic lesions that often have detrimental consequences for the practice of a ball sport, whether at a recreational level or at the top athlete level. The initial injury management is crucial. Depending on the type and the severity of the lesions, different people may get involved: the players themselves or their entourage, physiotherapists, club doctors, emergency physicians, family doctors, sports doctors or hand surgeons. Treatment is usually conservative: the long fingers require rapid mobilization to prevent stiffness and contrary to that, the thumb requires stability. Surgery may be necessary to reach these goals especially for athletes, because of the demands of their sport. Chronic lesions are also in part related to repeated trauma, requiring specialized long-term multidisciplinary treatment. They can often lead to the end of a sporting career, but also may limit the functionality of the fingers at the time of conversion.
