Challenging histories of plastic surgery that posit a complete disappearance of Gaspare Tagliacozzi's rhinoplasty operation after his death in 1599, Rhinoplasty and the nose in early modern British medicine and culture traces knowledge of the procedure within the early modern British medical community, through to its impact on the nineteenth-century revival of skin-flap facial surgeries. There are books about penoplasty (penile enlargement) but none from men who have actually undergone the procedure. I have appeared on television and been interviewed in magazines about my operation.
Fran Stiff is a silicone gel survivor. Implanted with silicone gel-filled breast implants in November 1981 she was told it was a 'one off' and that the implants would last a lifetime. It was the beginning of a 22-year nightmare. Intended to enhance her beauty, the procedure had the opposite result. By 1983 her right breast had become hard and deformed. The implant had ruptured and loose silicone gel had been broadcast into her breast and other bodily tissues. This was surgically removed and the implant replaced. By May 1989 her right breast had re-hardened, but this time she was unable to find a plastic surgeon willing to deal with it. They were only prepared to explant the implants, because they said the hardening would only reoccur. This would have left her with a grossly deformed chest and no breasts. She refused, deciding it was preferable to live with the deformity. She found herself progressively afflicted with major allergies and rheumatism. It was only in 1997 – 16 years after the first implantations – that she found a plastic surgeon who was prepared to tackle her problems. After a string of operations to remove the remnants of silicone gel that remained in her body and the replacement of both her implants with saline-filled ones, Fran is at last at the stage she had thought she had reached in 1981. Even her silicone gel-inspired allergies and rheumatism has begun to diminish. While silicone gel implants have been banned in the USA since 1992, their virtually unregulated use has continued in the European Union, South Africa and elsewhere. Effectively, women there have been used as lab rats by manufacturers of silicone gel breast implants to gather data for the FDA so that their products might be allowed back on the lucrative US market.

Its products claim to optimise visual appearance, to bestow inner and outer health and to delay aging, under a veneer of medical credibility and reliable results. Cosmetics deals in alleged 'deep-acting' substances, which are supposed to detoxify and to purify the body from within. However, the whole arsenal of cosmetic ingredients is founded more on persuasion than conviction. Cosmetics is always a matter of mimicking an ideal. To this extent, cosmetic discourse deals in what might be described as an iconography of hunger -- it requires and is predicated upon a feeling of lack. At the same time, it promises to remedy that lack. The referential frame for cosmetics is constituted by cultural history and iconology, by semiotics and sociology, by psychology and rhetoric. Like fashion, it has called into being a linguistic system of considerable depth and complexity; one that draws its subtexts from futurology and history, from medicine and alchemy, from nostalgia and from heritage preservation. Cosmetics are supposed to make one more attractive and more seductive -- to make one positively irresistible, in fact. Cosmetics hold out the prospect of sexiness to women and men alike. All that one has to do is to acquire the right creams and lotions, the right palette of powders and rouges, of lipsticks and mascara, and one has a form of beauty that can be bought! Regenerating damaged organs and tissues, an act that once was considered magic, is currently entrusted to the surgeons who have allowed us to move from replacement and reconstructive plastic surgery to regenerative plastic surgery, through autologous and allogeneic cell-based therapies and growth factors. The enthusiasm for regenerative plastic surgery and for the treatment of some pathologies addressed by it, such as breast reconstruction, hemifacial atrophy, burns, scars, and aesthetic improvements such as breast and buttock augmentation, face rejuvenation and hair regrowth, has led the author, Professor Pietro Gentile, to rigorously investigate the possible new minimally invasive strategies based on adipose-derived stem cells, human follicle stem cells and growth factors contained in platelet-rich plasma. One major by-product of the aging baby-boom generation has been a surging interest in cosmetic surgery. Out-patient cosmetic surgery clinics have sprouted up in droves all over the U.S., and the number of cosmetic procedures performed in 2005 increased by over 95% from the previous year. Although procedures like facelifts and abdominoplasties (the 'tummy-tuck') are considered minimally invasive, the anesthetic protocols and regimens here are often overly complex and unnecessarily toxic. Major complications involving anesthesia in this (and any other) surgical milieu can range from severe post-operative nausea and vomiting (PONV) to neuromuscular spasticity to mortality. The mortality spectrum of things may be rare, but there have been many cases in which perfectly healthy cosmetic surgery patients require emergency intervention due to a severe complication involving anesthesia. In recent years, many new anesthetic protocols have been developed to reduce the incidence of PONV and other complications, while ensuring that effective pain management and level of 'un-awareness' during surgery is always maintained.
