Health care systems are having to adapt to a growing, aging
population. This book considers services provided in one home care
program and discusses a research project that explores what is
working well with current services and proposes changes to the way
programs are coordinated in order to enhance service delivery. As
the population in Canada ages there is a growing need to explore
innovative approaches to delivering health care services in an
efficient and cost effective way. The findings of this research
project support the need for change and identify specific areas for
