In einem neuen Kapitel "Die Bedeutung der Manuellen Medizin in der Praxis" berichten Kollegen verschiedener Fachgebiete aus allen drei Arzteseminaren der DGMM uber ihre Erfahrungen mit der Manuellen Medizin/Therapie. Die Beeinflussung der Fliesseigenschaften des stroemenden Blutes erhalt zunehmende klinische Bedeutung: arterielle Durchblutungs- stoerungen, Herzinfarkt und postoperative Thromboseverhutung sind moegliche Indikationsgebiete. Eine wesentliche verbesserung der Fliesseigenschaften des Blutes ist in der Klinik nur durch Senkung des Hamatokritwertes bei gleichbleibendem oder leicht erhoehtem Gesamtblutvolumen moeglich, also durch normo- oder hyper- volamische Hamodilution. Die Grenze einer sinnvollen Hamodilution wird durch Veranderungen der Sauerstofftransportkapazitat des ver- dunnten Blutes bestimmt. Tierexperimentell konnte eine verlassliche Korrelation zwischen Hamodilution, rheologischen Eigenschaften des Blutes und Sauerstoffversorgung der Gewebe aufgestellt werden; die UEbertragung und Anwendung der Ergebnisse auf die Klinik bei be- kannten oder unbekannten hamodynamischen sowie cardio-vascularen Vorschaden ist nur mit Einschrankung moeglich. Besonders bedeutsam werder die Fragen nach ausreichender Sauerstoffversorgung aller Gewebe, wenn das Hauptziel der Hamodilution in der Gewinnung groesstmoeglicher Mengen von Patienteneigenblut liegt. Gerade dieses Ziel verfolgt die akute normovolamische Hamodilution vor opera- tiven Eingriffen, wobei operativer Blutverlust allein durch Re- transfusion von Eigenblut anstelle der Gabe von Fremdblut ausge- glichen wird. Die grosse Gefahr der Hepatitisubertragung durch Fremdbluttransfusionen - von uns Chirurgen wahrend operativer Eingriffe teils nicht voll gewurdigt, teils notgedrungen in Kauf genommen - ist Anlass genug zur Anwendung der Hamodilution in der Chirurgie. Dies ist auch unter Berucksichtigung des Aufwandes an Arbeit und Zeit bei dieser Methodik gultig. Alleinige Vorausset- zung ist, dass die Risiken der Hamodilution deutlich unter denen der Hepatitisubertragung liegen.
