This volume is the first concentrated effort to offer a philosophical critique of relational and intersubjective perspectives in contemporary psychoanalytic thought. The distinguished group of scholars and clinicians assembled here are largely preoccupied with tracing the theoretical underpinnings of relational psychoanalysis, its divergence from traditional psychoanalytic paradigms, implications for clinical reform and therapeutic practice, and its intersection with alternative psychoanalytic approaches that are co-extensive with the relational turn. Because relational and intersubjective perspectives have not been properly critiqued from within their own schools of discourse, many of the contributors assembled here subject advocates of the American Middle School to a thorough critique of their theoretical assumptions, limitations, and practices. This book offers a practitioner's guide to evidence-based practice in working with psychotic patients in an outpatient setting by clinicians and scholars who are internationally recognized for their work in treating severe psychopathology. Topics cover conceptual, technical, and practical considerations in the parameters of working with adult and adolescent populations that exhibit thought disorder, delusions, hallucinations, borderline organizations, trauma, and schizoid phenomena. Different theoretical models are presented from psychoanalytic traditions that introduce the student and practitioner to eclectic ways of conceptualizing and treating these challenging clinical groups. Concrete approaches to establishing a proper treatment environment, working alliance, symptom management, managing countertransference, and facilitating a therapeutic framework are provided. Various psychodynamic techniques are demonstrated by master clinicians through the extensive use of clinical case material culled from outpatient settings that illustrate how psychoanalytic perspectives enrich our understanding of the psychotic spectrum and lead to therapeutic efficacy.

An attempt is made to present Lacan's unorganized theories to apply to conceptual paradigms in psychoanalysis and the humanities as well as applied clinical practice. This effort is in the spirit of fostering dialogue and educating different theoretical orientations within psychoanalysis on what Lacan and his followers have contributed to emerging contemporary perspectives on psychotic phenomena in both normative and pathological populations. Within Lacanian circles there is debate over what constitutes psychosis, including defining the ordinary from pathological variants that have historically defined the phenomena as a mental illness. Here psychosis is not defined by hegemonic authoritarian psychiatry, but rather as a conceptual framework or philosophical perspective supported by descriptive narrative and symptomatic phenomenology that challenges preconceived notions of what we typically consider psychosis to entail. In this book a variety of perspectives are presented by internationally respected scholars and clinicians who examine what Lacan had to say about psychosis, from his nuanced theories represented in select texts, including omissions, extrapolations, and new applications, as well as how clinical methodology and technique have been adapted and advanced by practitioners treating psychotic individuals.
