infolge von Verteilungs- und Diffusionsstorungen in der Lunge sind
unmit- te1bare Folgen der Embolie. Mechanische, humorale und nervos
re- flektorische Pathomechanismen schlieBen sich im
verhangnisvollen "Circulus vitiosus". Ibn zu durchbrechen und seine
Beseitigung ist Anliegen in der Behandlung. Die schnelle
Revaskularisation der pulmonalen Strombahn mit dem Ziel der
Entlastung des rechten Herzens ist vordringliches Ziel der Therapie
und rutt in Abhiingigkeit von der Schwere des Bildes Inter- nisten
und Chirurgen auf den Plan. Die interdisziplinare Fragest- lung
gipfelt bei den schweren und schwersten Formen der Embolie
letztendlich in der Entscheidung, ob sofort chirurgisch
interveniert werden muB, oder ob mit der fibrinolytischen
Behandlung zuminde- stens zunachst die Wiedereroffnung der
GefaBbahn angestrebt wer- den solI. Nach wie vor aber gilt gerade
ftir die Lungenembolie der alte Satz: "daB die beste Therapie die
Prophylaxe ist". Der Internist Fritz Heinrich, ehemals Oberarzt
