Pulmonaler Hochdruck mit der oft fatalen, akuten Rechtsinsuffizienz des Herzens und eine arterielIe Hypoxie infolge von Verteilungs- und Diffusionsstorungen in der Lunge sind unmit- te1bare Folgen der Embolie. Mechanische, humorale und nervos re- flektorische Pathomechanismen schlieBen sich im verhangnisvollen "Circulus vitiosus". Ibn zu durchbrechen und seine Beseitigung ist Anliegen in der Behandlung. Die schnelle Revaskularisation der pulmonalen Strombahn mit dem Ziel der Entlastung des rechten Herzens ist vordringliches Ziel der Therapie und rutt in Abhiingigkeit von der Schwere des Bildes Inter- nisten und Chirurgen auf den Plan.
