In this book we have taken a comprehensive look at the subject of
familial and hereditary gastric tumors. In particular, the aim of
this novel editorial work is to propose the correct management of
hereditary diffuse gastric cancer patients, focusing in particular
on E-cadherin germline mutations, clinical criteria definition,
genetic screening and molecular mechanisms, pathology and
microscopic features, surgical treatment and clinical approach for
asymptomatic mutation carriers. We also describe other inherited
predispositions involving gastric carcinoma.
In this book the editors and authors provide a comprehensive
overview on basic research and clinical aspects of hereditary
breast and gastric tumors. In particular, this updated editorial
work aims to suggest guidelines for the clinical management of
patients with hereditary diffuse gastric and lobular breast cancer.
Special attention is given to E-cadherin (CDH1 gene) germline
mutations, genetic screening approaches, the underlying molecular
mechanisms and pathological and microscopic features. In addition,
the book aims to define clinical criteria for genetic screening,
and highlights current surgical treatment and clinical approaches
for asymptomatic mutation carriers. Other inherited predispositions
involving gastric and breast carcinoma are discussed as well.
Divided into eight sections, the book starts by providing an
epidemiological overview of gastric and breast cancers, followed by
a section dealing with new descriptions of genetic pathways in
hereditary cancer predispositions. The third section focuses on
pathological features of the diseases, in an effort to bridge the
gap between discovery and cancer therapy development. Subsequent
sections of the book are dedicated to endoscopy and breast imaging,
as well as risk-reducing surgeries to curb the risk of developing
cancer. The sixth section focuses on the generation of ideas for
the identification of targets and novel treatment strategies.
Finally, in the seventh section the authors share the story of two
patients and their experiences with the diagnosis and treatment of
hereditary cancer. This multidisciplinary book brings together
multiple disciplines in science and technology; specifically,
medicine, surgery and biology. The majority of authors are members
of the International Gastric Cancer Linkage Consortium (IGCLC) and
of the European Cancer Prevention Organization (EJCPO), with
relevant experience in this context. Offering in-depth insights
into hereditary cancers, this book represents essential reading for
students, researchers, and specialists who want to extend their
knowledge on hereditary gastric and breast cancers. Â Â

Twenty-five years ago, a monograph on gastric cancer was published
by the Italian Society of Surgery: that book is recognized as a
milestone in the management of these tumors in Italy. Oncological
and surgical knowledge in the field of gastric cancer have changed
dramatically over the last 25 years. The aim of this book is to
offer an essential update on current diagnostic approaches and
optimal treatment strategies. It gives comprehensive information on
gastric cancer not neglecting the basic sciences, which can shed
light on the carcinogenesis mechanisms involved in this pathology.
The principles of tailored and multimodal treatment are examined
according to the latest guidelines with the aim of providing a
valuable synopsis for clinicians. The book not only reviews what
has changed in the field of gastric cancer over the past quarter
century but also offers the reader a glimpse of the future by
describing the ongoing surgical and oncological trials which focus
on new treatment frontiers such as immunotherapy and target
therapy. It also seeks to incorporate the latest discoveries based
on molecular classifications. Further, the book reflects the
findings and experiences of Italian experts belonging to the GIRCG
(Italian Research Group for Gastric Cancer) and will offer an
excellent guide for oncological surgeons in their daily practice of
caring for patients affected by gastric cancer.
