Until some years ago serum or crude tissue extracts were used pre dominantly or exclusively as media supplements for the cultivation of cells. However, during this time evidence accumulated that these sup plements could not provide in an optimal way most of the cultivated cells with all factors necessary for their survival, their prolifer ation and/or differentiation. Moreover, a variety of cells could not be cultivated at all under these conditions and often the composition of the cultures changed within rather short periods of time by overgrowth of initially present subpopulations of those cells which grow well in these supplements, as for example fibroblasts. Nevertheless, using these supplements (or fractions thereof), insight could be gained into some of the influences of serum or tissue extract constituents with re gard to survival, proliferation and differentiation of cells in cul ture. It became obvious from these experiments that serum or tissue extracts did not only supply cells with nutrients or vitamins (which are now constituents of all basic media), but also with hormones as well as growth-, differentiation-, and attachment-factors. In course of time experiments were performed in which serum enriched with hormones and other growth factors was used to successfully culti vate those cells which could not survive in serum-supplemented media alone. Under normal conditions in an organism, however, only a small population of cells has direct contact with serum."

Die ICPC ist als Leistung einer multinationalen Arbeitsgruppe der WONCA in englischer Sprache entstanden. Dieses System der Klassifizierung hilft, Gesundheitsprobleme aus Sicht der allgemein rztlichen Betreuer und ihrer Patienten zu kodieren.Die ICPC wurde 1987 erstmals von der WONCA ver ffentlicht. Sie erm glicht die Kodierung von Konsultationsanl ssen, Diagnosen oder Gesundheitsproblemen sowie medizinischen Ma nahmen und erlaubt daher eine Kategorisierung aller Elemente von der ersten Konsultation bis zum Abschluss eines Behandlungsfalles. Im Gegensatz zur ICD beruht die Kapiteleinteilung der ICPC auf Organsystemen. Die Lokalisation hat also Vorrang vor der tiologie, wodurch die eindeutige Kodierung von Gesundheitsproblemen erleichtert wird. Die ICPC-2 ist aber sorgf ltig auf die ICD-10 abgestimmt und enth lt Umwandlungstabellen, die eine reibungslose Zusammenarbeit der beiden Systeme erm glichen. Die ICPC-2 ist bereits in mehr als 20 Sprachen bersetzt und bietet nun auch f r den deutschen Sprachraum die M glichkeit einer ad quaten Kodierung der allgemeinmedizinischen T tigkeit.

Eigene Unzufriedenheit wie oeffentlich geubte Kritik beziehen sich darauf, dass zum einen vielfach unreflektiert diagnostiziert und gehandelt wird, und dass Handlungsstrategien unkritisch ubernommen werden. Zu viel wird der einzelne Patient geroentgt, im Computertomogramm untersucht, biopsiert, mit Pharmaka behandelt und operiert, statt dass er eingehender mit dem Arzt sprechen kann. Zum anderen wird der einzelne Patient mit UEberweisungszetteln vom Allgemein- arzt zum Spezialisten, zum "Feindflug" geschickt und hier weitergereicht. Einer "Verzettelung der Verantwortung" durch problembezogenes interkollegiales Handeln ist oft schwer entgegenzuwirken. Daruber hinaus wird die Wissen- schaftlichkeit der angewandten Medizin, und hier v. a. in der ambulanten Kran- kenversorgung, in Frage gestellt. Die Folge ist, dass sich weite Bereiche der Me- dizin in einem standigen Abwehrkampf befinden. Fragwurdige UEberzeugungen werden vertreten: Wissenschaft sei nur in Klinik und Labor realisie'rbar; Wissen- schaft in der Praxis habe dagegen keine methodische Eigenstandigkeit. Als Folge dieser einseitigen Betrachtungsweisen werden wichtige politische Ent- scheidungen zur Wissenschaftspolitik getroffen, die bestehenden Verhaltnisse perpetuieren.

Die therapeutische Schulung des praktischen Arztes soIl das Leitmotiv dieses Werkes sein Klinische Behandlung und arztliche "Alltagstherapie" zeigen tiefgreifende Unterschiede.
