Until some years ago serum or crude tissue extracts were used pre
dominantly or exclusively as media supplements for the cultivation
of cells. However, during this time evidence accumulated that these
sup plements could not provide in an optimal way most of the
cultivated cells with all factors necessary for their survival,
their prolifer ation and/or differentiation. Moreover, a variety of
cells could not be cultivated at all under these conditions and
often the composition of the cultures changed within rather short
periods of time by overgrowth of initially present subpopulations
of those cells which grow well in these supplements, as for example
fibroblasts. Nevertheless, using these supplements (or fractions
thereof), insight could be gained into some of the influences of
serum or tissue extract constituents with re gard to survival,
proliferation and differentiation of cells in cul ture. It became
obvious from these experiments that serum or tissue extracts did
not only supply cells with nutrients or vitamins (which are now
constituents of all basic media), but also with hormones as well as
growth-, differentiation-, and attachment-factors. In course of
time experiments were performed in which serum enriched with
hormones and other growth factors was used to successfully culti
vate those cells which could not survive in serum-supplemented
media alone. Under normal conditions in an organism, however, only
a small population of cells has direct contact with serum."

Entwickelungsperlode in der Medi- dn, dass die historische
Kenntniss der Dinge mit jeder Generation VOD Studirenden abnimmt.
Sogal' von den selbstthatigen jiingeren Arbeitern Imnn man in der
Regel annehmen. dass ihr Wissen im hochsten Falle nur bis auf 3-5
Jahre riickwiirts leicht. Was vor [) Jahren publicirt ist, existirt
nicht mehr. Virchow 1&7U. Je weiter von Jahr zu Jahr die
Grenzsteine der medicinischen Naturwissenschaften aus einander
geriickt werden, urn so berechtigter wird die Frage: ist es dem
praktischen Arzte moglich den Fort- schritten der Wissenschaft zu
folgen? Hat der junge Doctor von der Universitat Abschied genommen
und ist mit dem felsenfesten Vorsatze der Wissenschaft ewig treu zu
bleiben in die beginnenden Hungerjahre der Praxis eingetreten, so
wahrt es nicht lange und er steht fast unbemerkt vor einem
Scheidewege, wo unwiderruflich ein Entschluss gefasst werden muss.
Hier giebt der Charakter, nicht das Talent den Ausschlag, welcher
Weg eingeschlagen wirdj die Wahl ist furchtbar ernst, denn sie
entscheidet meistens fiir das ganze Leben. Entweder sagt er der
Wissenschaft Lebewohl und steuert direct ins Lager der
medicinischen Handwerker, wobei unterwegs nicht selten in dunkler
Stunde ein krummer Schleicbweg benutzt und eine siisse Frucht vom
Baume der Reclame gepfiuckt wird, oder er arbeitet fort, urn sich
den Namen eines wissenschaftlichen Praktikers zu verdienen. Allein
vor der Hand konnte man nach dem anstrengenden Examens- jahre sich
doch wenigstens eine Zeit lang Ruhe gonnenj die Uni-
versitatskenntnisse .werden ja fiir die ersten Jahre wohl
ausreichen.
Derzeit erfahren Initiativen fur Qualitatsmanagement und
Laborakkreditierung, bei denen die Dokumentation der
Personalstruktur einen sehr hohen Stellenwert hat, uneingeschrankte
Unterstutzung von den auf dem Gebiet der Laboratoriumsdiagnostik
tatigen medizinischen Fachgesellschaften. Die
Personalbedarfsermittlung, die in diesem Buch dargestellt wird, ist
sehr nutzlich fur eine ausreichende personelle Ausstattung eines
Laboratoriums und sollte als Leitlinie dienen.
Der Schutz der Gesundheit verlangt medizinischen Fortschritt und
die tatsachliche Verfugbarkeit von Spitzenmedizin. Allerdings
werden damit zusammenhangende Ausgabensteigerungen angesichts der
Kostendampfungsmaxime" oft als Fehlentwicklung wahrgenommen. Der
Aufsatzband stellt diese Maxime interdisziplinar auf den Prufstand.
Autorinnen und Autoren aus Rechts- und Wirtschaftswissenschaft, aus
Medizinethik und medizinischer und juristischer Praxis erortern, ob
Sozialrecht und medizinische Ethik die Vermeidung von
Innovationshemmnissen fordern. Volkswirtschaftlichen
Wachstumseffekten eines innovationsfreundlichen Gesundheitssystems
wird nachgegangen, ebenso der Zahlungsbereitschaft der
Versichertengemeinschaft fur Innovationen und dem Stellenwert des
Gutes Gesundheit" aus Sicht der Bevolkerung. Mit Blick auf die
politische Durchsetzbarkeit wird diskutiert, ob eine Akzeptanz
sichernde Implementation von Gesundheitsreformmassnahmen gelingen
kann."
Die ICPC ist als Leistung einer multinationalen Arbeitsgruppe der
WONCA in englischer Sprache entstanden. Dieses System der
Klassifizierung hilft, Gesundheitsprobleme aus Sicht der allgemein
rztlichen Betreuer und ihrer Patienten zu kodieren.Die ICPC wurde
1987 erstmals von der WONCA ver ffentlicht. Sie erm glicht die
Kodierung von Konsultationsanl ssen, Diagnosen oder
Gesundheitsproblemen sowie medizinischen Ma nahmen und erlaubt
daher eine Kategorisierung aller Elemente von der ersten
Konsultation bis zum Abschluss eines Behandlungsfalles. Im
Gegensatz zur ICD beruht die Kapiteleinteilung der ICPC auf
Organsystemen. Die Lokalisation hat also Vorrang vor der tiologie,
wodurch die eindeutige Kodierung von Gesundheitsproblemen
erleichtert wird. Die ICPC-2 ist aber sorgf ltig auf die ICD-10
abgestimmt und enth lt Umwandlungstabellen, die eine reibungslose
Zusammenarbeit der beiden Systeme erm glichen. Die ICPC-2 ist
bereits in mehr als 20 Sprachen bersetzt und bietet nun auch f r
den deutschen Sprachraum die M glichkeit einer ad quaten Kodierung
der allgemeinmedizinischen T tigkeit.
Schritte arztlicher Erkenntnis und Entscheidungen werden
dargestellt. Die Dar- stellungen entstammen dem
allgemeinarztlichen, klinisch-ambulanten bzw. -sta- tionaren
Bereich sowie dem Bereich zwischen Klinik und Praxis. Sie gehoeren
dem Alltag an, wie er zwischen Hannover und Bern ablauft. Die
Szenen sind nicht gestellt. Die Berechtigung, ein derartiges Buch
herauszugeben, sehen die Herausgeber in ihrer eigenen
Unzufriedenheit gegenuber der Entwicklung in der Medizin so- wie in
der Kritik, welche in der OEffentlichkeit an dieser Medizin geubt
wird. Eigene Unzufriedenheit wie oeffentlich geubte Kritik beziehen
sich darauf, dass zum einen vielfach unreflektiert diagnostiziert
und gehandelt wird, und dass Handlungsstrategien unkritisch
ubernommen werden. Zu viel wird der einzelne Patient geroentgt, im
Computertomogramm untersucht, biopsiert, mit Pharmaka behandelt und
operiert, statt dass er eingehender mit dem Arzt sprechen kann. Zum
anderen wird der einzelne Patient mit UEberweisungszetteln vom
Allgemein- arzt zum Spezialisten, zum "Feindflug" geschickt und
hier weitergereicht. Einer "Verzettelung der Verantwortung" durch
problembezogenes interkollegiales Handeln ist oft schwer
entgegenzuwirken. Daruber hinaus wird die Wissen- schaftlichkeit
der angewandten Medizin, und hier v. a. in der ambulanten Kran-
kenversorgung, in Frage gestellt. Die Folge ist, dass sich weite
Bereiche der Me- dizin in einem standigen Abwehrkampf befinden.
Fragwurdige UEberzeugungen werden vertreten: Wissenschaft sei nur
in Klinik und Labor realisie'rbar; Wissen- schaft in der Praxis
habe dagegen keine methodische Eigenstandigkeit. Als Folge dieser
einseitigen Betrachtungsweisen werden wichtige politische Ent-
scheidungen zur Wissenschaftspolitik getroffen, die bestehenden
Verhaltnisse perpetuieren.
Die therapeutische Schulung des praktischen Arztes soIl das
Leitmotiv dieses Werkes sein Klinische Behandlung und arztliche
"Alltagstherapie" zeigen tiefgreifende Unterschiede. Der Praktiker
arbeitet auch thera peutisch unter anderen, meist schlechteren
Bedingungen als der Krankenhausarzt. Zahlreiche therapeutische
Forderungen, die der Kliniker stellt, kann er gar nicht erfUllen;
es fehIt scho- von vielen anderen, wohlbekannten Schwierigkeiten
abgesehe- der groBe und teure Hilfsapparat zur Krankenbeobachtung
und Krankenbehandlung, den sich die Klinik nutzbar macht. Junge
Kollegen, die sich niederlassen, empfinden oft bitter die gewaltige
Spannung zwischen "Theorie" und "Praxis." Diesen Unterschieden will
unser Werk Rechnung tragen. Es soIl die besonderen Schwierigkeiten
der Alltags prax: beleuchten, die Richtlinien fiir ihre Linderung
und Uberwindung angeben und die fiir den Haus- und Kassenarzt
giiltigen, be s ten Behandlungsmethoden schild ern. Spezialisten,
die nur an Krankenhausern tatig waren und in Privat-Sp chstunden
"Praxis aurea" treiben, verlieren in ihren an die Arzteschaft
gerichteten Publikationen oft den MaBstab fUr die dem Haus- und
Kassenarzt gegebenen thera peutischen Moglichkeiten. Die
Diatzettel, die sich in anerkannten Monographien iiber
Stoffwechselkrankheiten finden, gelten beispiels weise eher fUr

aurea" werden nur wenige teilhaftig; die weit iiberwiegende
Mehrzahl der Arzte ist auf miihevolle Kassen praxis, wo nur
Massenwirkung das notige Einkommen bringt, fast ganzlich
