In the last decade, ACE inhibitors have become a cornerstone of cardiovascular drug therapy. Beneficial effects in patients with hypertension, heart failure, and post myocardial infarction have been established in large clinical trials. In parallel, ex perimental research has shifted the interest from the immediate homeostatic function of the circulating renin-angiotensin-system to the multiple paracrine functions of local tissue renin-angiotensin-systems, including hypertrophy, hyperplasia, athe- oenesis, and vasomotion. . It was the aim or a symposium held in Hamburg, October 22-24, 1992, to bring together an international panel of experimental and clinical scientists and to discuss the most recent findings on the role of angiotensin and associated mediators in car diovascular adaptation and disease, with a particular emphasis on the treatment of such disease by ACE inhibitors. In den letzten Jahren haben Hemmstoffe des Konversionsenzyms bei der Indikation hoher Blutdruck, Herzinsuffizienz und bei Zustanden nach Myokardinfarkt eine Dif- ferentialtherapie erlaubt, die in ihren Ergebnissen bemerkenswert ist und uber eine Organ protektion zur Senkung der Mortalitat dieser letalen kardiovaskularen Erkrankungen beigetragen haben. Experimentelle Untersuchungen haben die Bedeutung der organstandigen Renin-Angiotensin-Systeme hervorgehoben und neue Aussichten zur Behandlung der Hypertrophie des Herzens und der Arteriosklerose der Gefasse eroeffnet. Im hier vorliegenden Band, in englisch erschienen als Supplement der Zeitschrift Basic Research in Cardiology, werden die Vortrage eines Internationalen Sym- posiums in Hamburg vom 22. bis 24. Oktober 1992 in deutscher UEbersetzung zusammengefasst. Der interdisziplinare Charakter des Symposiums kommt in ex- perimentellen und klinischen Untersuchungen zum Ausdruck. Die Herausgeber hof- fen, dass das Symposium "Angiotensin and the Heart" sich stimulierend auf die Grundlagenforschung und die Behandlung von kardiovaskularen Erkrankungen mit Hemmstoffen des Konversionsenzyms auswirkt. ATj-Rezeptorantagonisten sind eine Substanzklasse, die ahnlich wie die ACE-Hemmer wirken, wenngleich sie unterschiedliche Wirkmechanismen auszeichnen, und die ahnlich erfolgreich sind, obwohl sie erst seit gut drei Jahren auf dem deutschen Arzneimittelmarkt verfugbar sind. Die zweite Aussage ist von der ersten abhangig: die erfolgreiche Behandlung kardio- vaskularer Erkrankungen mit Hemmstoffen, die am Renin-Angiotensin-System (RAS) angreifen, namlich der ACE-Inhibitoren, hat den Weg fur die Therapie mit ATj-Antago- nisten gebahnt. Der voraussehbare Erfolg des Prinzips AT j-Rezeptorenblockade bei AErzten und Patienten hat dazu gefuhrt, dass sich viele Pharmazeutische Unternehmen mit der neuen Substanzklasse beschaftigten und nun ihre Substanzen auf dem Markt einfuhren. Innerhalb von nur drei Jahren wurde bereits die sechste Substanz in Deutschland zuge- lassen, und die ATI-Antagonisten brechen damit den Rekord der ACE-Hemmer. Die antihypertensiven, kardioprotektiven, vasoprotektiven und nephroprotektiven Eigenschaften der ATI-Antagonisten wurden in praklinischen Untersuchungen am Tier- experiment mit ACE-Inhibitoren aber auch anderen Antihypertensiva verglichen und ein- drucksvoll belegt, die klinische Wirksamkeit wird zur Zeit in grossangelegten Mortali- tatsstudien fur alle protektiven Wirkungen untersucht. Ein Fazit kann bereits jetzt schon gezogen werden: AT I-Antagonisten sind bis heute die Antihypertensiva mit den wenigsten Nebenwirkungen! Das vorliegende Buch fasst die Vortrage eines internationalen Candesartan Kongresses auf Deutsch zusammen, der im April 1998 in Hamburg im ICC stattfand.

Seit den fruhen 70er Jahren standen Betablocker in Deutschland zunachst fur die Therapie der arteriellen Hypertonie und der koronaren Herzkrankheit zur Verfugung. Das Indikationsspektrum hat sich seitdem gewaltig geandert: Kontraindikationen, wie zuerst der Diabetes mellitus und die Herzinsuffizienz, sind von der Kontraindikationsliste verschwunden oder haben sich wie die Herzinsuffizienz sogar als Indikation fur die Betablocker herauskristallisiert. Das Millenium bot daher einen willkommenen Anlass, Betablocker und besonders ihren Einsatz bei der Herzinsuffizienz erneut in einem grossen internationalen Rahmen zu diskutieren. Das vorliegende Buchg fasst in deutscher Sprache die Beitrage zusammen, die am 1. April 2000 im Kongresszentrum in Hamburg einem grossen Forum vorgestellt wurden. Speziell wurde dabei die MERIT-Studie vorgestellt, die eine Mortalitatsreduktion durch den Betablocker Metoprolol bei Patienten mit Herzinsuffizienz demonstrieren konnte.
