Inotropic stimulation of the myocardium, as well as vasodilation and diuresis as essential principles in the treatment of congestive heart failure have recently met with considerable criticism and reevaluation. It is generally agreed that unloading of the heart, either through vasodilation and/or diuresis, improves the working conditions of the dilated, failing heart. It reduces myocar dial oxygen consumption through reduction of chamber radius and, thereby, wall tension as the major determinants of myocardial oxygen consumption. Inotropic stimulation, quite in contrast, does not conserve oxygen. It rather consumes energy and that may be disadvantageous in situations of compromised oxygen supply and energy metabolism of the working myocardium. However, under conditions of suf ficient oxygen supply and metabolic support inotropic stimulation may bring about in creased pumping and subsequent improvement of myocardial failure. In recent years it could convincingly be demonstrated that vasodilation leads to symp tomatic improvement of congestive heart failure, improvement of exercise tolerance, and it prolongs life - especially in the case of ACE-inhibitors and the combination of hydra lazine with long-acting nitrates. Quite in contrast, equally beneficial effects could not be demonstrated for inotropic agents in congestive heart failure. Only for the cardiac glyco sides has it been shown that beneficial effects can be achieved, especially if atrial fibril lation with absolute arrhythmia is present. The influence of the cardiac glycosides on the latter represents an effect which is independent of the inotropic action."
Wolfgang H Scholz (b. 1958) is a visual artist and film director. His work spans more than three decades and encompasses apart from painting many forms of expression, ranging from theatrical and documentary films, sculpture, photography, and installations to multimedia stage pieces. His central theme is the vision of an imaginary arrival, and his work method is a form of decoding. Other essential concepts that recur in Scholz's work include the labyrinth, time, memories or localization and the questioning of reality. The title The Void is taken from a Buddhist term for the Fifth Element: The Void. Since 2010 Scholz has worked with Japanese Butoh masters, creating several multimedia stage pieces and series of photographic works on this theme. This volume includes a conversation with Prof. Dr. Boris Groys of New York University, one of the most important scholars of the arts and humanities of the twentieth century. This dialogue is an essential text for understanding the creative processes, references, and influences of Wolfgang H Scholz concerning the philosophical and programmatic themes of The Void. This book will be published to coincide with exhibitions by the MACO - Museum of Contemporary Art Oaxaca (2019), Mexico, the Museum Ex Teresa Arte Actual (2019), the gallery Casa Galván - UAM - Universidad Autónoma metropolitana (2020) and in collaboration with the presentations of the performance THE VOID at the Butoh Festival Kyoto, Japan (July 2019) and at the Theatre CC Los Talleres, Mexico City in 2019. Das vorliegende Lehrbuch der Magen-Darmkrankheiten ist ur sprunglich als Abschnitt eines skandinavischen Handbuchs der inneren Medizin geschrieben. Einer Aufforderung von Herrn Dr. Springer, das Werk in seinem Verlage als selbstandiges Buch erscheinen zu lassen, bin ich gern gefolgt und begruBe die Gelegenheit, einem deutschen Leser kreis seine Bekanntschaft zu vermitteln.
