Wiederholt man die Injektion, so entwickelt sich eine sturmische nicht eitrige Arthritis, die sich in Form und Verlauf der akuten Polyarthritis bei Rheumatischem Fieber weitgehend angleicht. Der Modellver- such stammt von ROESSLE, die experimentelle Erweiterung und Auswertung sind das Lebenswerk von KLINGE. Nach KLINGE lasst sich die rheumatische Entzundung in 3 Phasen aufloesen: 1. das rheumatische Fruhinfiltrat, 2. das rheumatische Granulom, 3. die rheumatische Narbe. Das rheumatische Fruhin- filtrat entspricht einer fibrinoiden Bindegewebsverquellung. RICH et al. haben die Klingeschen Versuchsergebnisse bestatigt und daruber hinaus nachgewiesen, dass beispielsweise auch Sulfonamide antigene Eigenschaften besitzen und bei wiederholtem Einsatz fibrinoide Verquellungen verursachen. Die fibrinoide Ver- quellung ist die entscheidende Primarlasion des Rheumatischen Fiebers. Im Handbuch der speziellen pathologischen Anatomie und Histologie Band IX/2 (1934) fasst KLINGE seine Untersuchungsergebnisse wie folgt zusammen: "rheu- matisch in weitem Sinne heisst die sich infolge besonderer immunbiologischer Verhaltnisse ergebende allergisch-hyperergische Reaktion mit dem Ergebnis einer (fibrinoiden) Quellung des Bindegewebes und all ihren Folgen. " KLINGE ersetzt damit die Erregerspezijitat durch die Reaktionsspezijitat. Eine Bestatigung der Klingeschen Auffassungen brachte die Aufnahme des Rheumatischen Fiebers und der rheumatoiden Arthritis in den 1942 von KLEMPE- RER geschaffenen Begriff der Kollagenosen. In diesem werden die entzundlichen Alterationen der Bindegewebszwischensubstanzen des Gesamtkoerpers als Gemeinschaftsreaktion zusammengefasst. Die Bedeutung dieser Zuordnung liegt in der Anerkennung des extraartikularen Gefassbindegewebes als gleichwertigen Partner des Gefassbindegewebes der Gelenke, Sehnenscheiden und Schleimbeu- tel.
