his volume of the series Lecture Notes in Medical Informatics contains the T proceedings of the Workshop on System Engineering in Medicine, which was held in Maastricht, The Netherlands, 16-18 March 1989. This workshop was sponsored by the EC under the framework of the Medical and Health Research Programme. The aim of the workshop was to assess whether there was sufficient support in the Medical Informatics community in the EC to establish a concerted action. This proceedings contain papers of the presentations given at the workshop. These presentations were centred around three themes: * Methods and Tools * Applications in the domains of chronic care and critical care * Evaluation of decision support systems The papers were prepared after the workshop and therefore we were able to include the relevant parts of the discussions which were related to the presentations. As a result of the discussions during the workshop, a proposal was prepared for the establishment of a concerted action, specifically addressing the development of guidelines for the evaluation of medical decision aids. This proposal was granted early 1990 under the same Medical and Health Research programme of the EC. Over 40 institutes are participating in this concerted action. It have been the outstanding presentations and the open discussions at the workshop that have been the starting point of this concerted action. The papers in this proceedings formed a starting point for the discussions in the meetings of the concerted action.

Jane Whorwood(1612-84) was one of Charles I's closest confidantes. The daughter of Scots courtiers at Whitehall and the wife of an Oxfordshire squire, when the court moved to Oxford in 1642, at the start of the Civil War, she helped the Royalist cause by spying for the king and smuggling at least three-quarters of a ton of gold to help pay for his army. When Charles was held captive by the Parliamentarians, from 1646 to 1649, she organised money, correspondence, several escape attempts, astrological advice and a ship to carry him to Holland. The king and she also had a wartime 'brief encounter'. After Charles's execution in 1649, Jane's marriage collapsed in one of the most public and acrimonious separation cases of the seventeenth century. The current scarcity of expert systems where the reasoning is based on Bayesian probability theory may be due to misconceptions about probabilities found in the literature. As argued by Cheeseman (1985), these misconceptions have led to the attitude: "The Bayesian approach doesn't work - so here is a new scheme." Several of these expert systems based on ad hoc "probability" concepts have been successful in a number of ways, demonstrating the necessity of being able to handle uncertainty in medical expert systems. They also demonstrate the need for a theoretically sound handling of uncertainty. In Andersen et al. (1986) it was postulated that knowledge organized in a causal network can be used for a unified approach to the main tasks of a medical expert system: diagnosis, planning of tests and explanations. The present paper explores this postulate in a causal probabilistic network. It also provides a practical demonstration that the problems supposedly associated with probabilistic networks are either non-existent or that practical solutions can be found. This paper reports on the methods implemented in MUNIN* -an expert system for electromyography (EMG) (Andreassen et al. 1987).
