Dottie's life adventure begins before Dottie is even born. Based on
the actual life of my mother, her adoption and eventual return to
visit her real family during the 1920's and 30's, it is a fictional
story that includes information, places and some speculation as to
what happened at the time of her birth and why her lifes adventure
began so young. 1918 was not a time when a young girl had a child
without being married, and when this situation happened in Dottie's
grandmother's household, it brought about events that would shape
Dottie's life forever. Touching the lives of many people along the
way, Dottie would be sent halfway across the country as a Train
Child from New York City to the farmlands of Iowa. Follow along on
this twisting train ride as Dottie travels and grows into the young
women who completes a circle that answers many of the questions her
adoption left with her.

"Another Way Home" is the fictional story of Rick and his tour of
duty in Nam. Weaving from his current home in Casper, Wyoming, he
is taken back in his memories when he encounters a young soldier
returning from Iraq and offers him a ride. Taken from stories told
over a few drinks and many hours of brotherly conversation, this
book tells of a strength and courage that integrates fiction with
reality.
This is the third in a series of stories surrounding my family. The
first was my mother's story entitles "Ones Way Home." This book
picks up from her story and mixes with my father's. Told from
various points of view, it shows the various life situations both
of my parents, as well as many others of that time, lived through.
I hope you enjoy reading the story as much as I have telling it.
For over a decade, there has been a high prevalence of potentially
inappropriate medication (PIM) use as defined by the "Beers
criteria" in the outpatient elderly population. Using CPOE
medication lists, the measured prevalence of PIMs as of April 1,
2006 was 23.3% (8,693/37,247) of patients at Intermountain
Healthcare and 23.0% (5,528/24,004) at the Cleveland Clinic.
Patient characteristics associated with PIMs were polypharmacy,
female, and multiple primary care visits. A decision support system
of geriatric precaution screening alerts for 86 PIMs was integrated
into Intermountain Healthcare's CPOE system. A 9-month prospective,
randomized controlled trial of the system was conducted in 52
outpatient primary care and specialty clinics. Intervention clinics
received the alerts, while control clinics received usual care. No
significant difference in canceled PIM prescriptions was found
between intervention clinics at 4.7% (187/3977) and control clinics
at 4.8% (113/2364). None of the individual 86 PIM alerts showed
significant differences. Positive physician feedback suggest the
alerts have some clinical value.
