Several contributions in our first book about schizo affective disorders (Marneros and Tsuang, Schizoaffective Psychoses, Springer-Verlag, 1986) supported the assumption that schizoaffective disorders differ in relevant ways from schizophrenic disorders. The classification of schizo affective disorders as a subgroup of schizophrenia has also been criticized, and empirical research in clinical, genetic, therapeutic, and prognostic areas supports the idea that there are some strong similari ties between schizo affective and affective disorders. Of course, there are not only similarities between these two groups, but also differences just as there are between schizo affective and schizophrenic disorders. It is precisely the existence of similarities and differences between schizo affective disorders and the other two so-called typical mental disorders, i.e., schizophrenia and affective disorders, which makes them a challenge in psychiatric research, a challenge to the traditional dichotomy in the classification of disorders which originated with Kraepelin. This challenge is certainly proving fruitful in psychiatric research. These "cases in between" may well demonstrate that sep arating, dividing, and limiting is not always meaningful. Sometimes it can be more meaningful to unify; to unify in the sense of building bridges between typical groups. It is here that the assumption of a "psychotic continuum" can become relevant, and the investigation of schizoaffective disorders is of prime importance in research on a possible continuum of psychosis.

The purpose of the World Psychiatric Association is to coordinate the activities of its Member Societies on a world-wide scale and to advance enquiry into the etiology, pathology, and treatment of mental illness. To further this purpose, the Association organizes mono- or multi thematic Regional Symposia in different parts of the world twice a year, and World Congresses dealing with all individual fields of psychiatry once every five or six years. Between these meetings the continuation of the Association's scientific work is assured through the activities of its specialty sections, each covering an important field of psychiatry. The programs of the World Congresses reflect on the one hand the intention to present the coordinating functions of the Association and on the other to open a broad platform for a free exchange of views. Thus, the VII World Congress of Psychiatry, held in Vienna from July 11 to 16, 1983, was composed of two types of scientific events - those structured by the Association and those left to the initiative of the participants. The first type comprised Plenary Sessions, planned by the Scientific Program Committee, and Section Symposia, organized by the WPA sections; the second embraced Free Symposia, free papers, video sessions, and poster presentations prepared by the participants. Altogether, 10 Plenary Sessions, 52 Section Symposia, and 105 Free Symposia took place, and 78 free papers and poster sessions and 10 video sessions were held.

Mit Ausnahme des Kapitels uber allgemeine Psychopharmakologie ist dieser Band ausschliesslich den Schizophrenien gewidmet. Der Leser beachte den Unterschied zur zweiten Auflage: dort wurde den Schizo- phrenien ein Kapitel gewidmet, heute legen wir einen ganzen Band vor. Heisst dies, dass die Forschung ganz neue Resultate hervorgebracht hat? Unsere Antwort ist: nein. Die Herausgeber meinen jedoch, dass hervor- gehoben werden kann eine Vertiefung des psychodynamischen Ver- standnisses und eine Verbreiterung des biologischen Ansatzes anderer- seits. Durch umfassende Katamnesen wurde der Verlauf der Schizo- phrenien klarer erkenntlich, die Behandlungsstrategien haben sich verfeinert, insbesondere was die Ruckfallprophylaxe durch Dauermedi- kation betrifft. In den Abschnitten zur Psychopathologie musste Ruck- sicht genommen werden auf die heute scharfer denn je ins Blickfeld getretene Schwierigkeit der Abtrennung der Schizophrenien sui generis zu Grenzsituationen. UEberhaupt wird der Leser in den Kapiteln zur Psychopathologie wohl den auffallendsten Unterschied zu den vorher- gehenden Auflagen konstatieren. Wohl haben die Autoren mit Recht die von Kraepelin und Bleuler vorgezeichneten Pfade nicht ganz verlassen, aber das Hauptgewicht liegt nicht mehr auf der subtilen Beschreibung sogenannter typischer Bilder, sondern auf der Einbettung der Sympto- matologie in einen allgemeinen Zusammenhang. Die unaufhaltsame Spezialisierung hat mit sich gebracht, dass es keinen Manfred Bleuler mehr gibt, der wie damals in der zweiten Auflage das ganze weite Gebiet der neueren Schizophrenieforschung uberblickt hatte. So mussten wir in Kauf nehmen, dass die einzelnen Beitrage sich gelegentlich uberlappen, aber auch dass der Puzzle-Charakter der heutigen Schizophrenielehre scharfer zum Ausdruck kommt als in der ersten und zweiten Auflage.

Seine Einleitung zur ersten Auflage dieses klinischen Bandes beschloss MAYER GROSS 1959 mit dem Hinweis auf die Polarisierung der Psychiatrie zwischen dem "synthetischen Denken der Klinik" und den "Einseitigkeiten der UEberspeziali sierung". Dieser Spannung fuhlten sich die Herausgeber der vorliegenden zweiten Auflage womoeglich noch starker ausgesetzt, als sie ein Jahrzehnt spater die Gliede rung der jetzt erscheinenden heiden klinischen Bande planten. Sie suchten eine Loesung, indem sie das schnell anwachsende klinische Erfahrungsfeld starker differenzierten. Der Schwerpunkt dieser klinisch-psychiatrischen Bande konnte im Vergleich zur ersten Auflage in die Richtung der Neurosen und endogenen Psychosen ver schoben werden. Der mit der Erstauflage vertraute Leser wird in ihnen neuartig abgehandelten Themen und neuen Autoren begegnen. Wo eine Psychiatrie der Gegenwart vermittelt werden soll, kann es nicht damit sein Bewenden haben, bewahrte fruhere Beitrage auf den neuesten Stand zu bringen. Neue nosographi sche und therapeutische Ansatze waren einzuordnen, Disproportionen der thema tischen Gliederung der Erstauflage mussten geglattet werden. Solche redaktionelle AEnderungen bedingten einen grossen Anspruch an die Disziplin aller Mitwirkenden. Die Herausgeber danken ihnen dafur. In ihren grossen Zugen konnte die Kapitelaufteilung beibehalten werden. Epilepsien, Oligophrenien und Alterspsychiatrie - letztere wurde von C. MUELLER redaktionell betreut - treten jetzt allerdings als eigenstandige Bereiche hervor. Erfahrungen der Grundlagenforschung mit Bedeutung fur die klinische Theorie und Praxis mussten erfasst werden, wobei UEberschneidungen mit Themengruppen anderer Bande dieses Werkes zu vermeiden waren.
