The European Federation for Medical Informatics is a regional coordinat- ing body. The Congress in Dublin. MIE 82. from 21st to 25th March 1982. is the fourth in the series following MIE 78 in Cambridge. MIE 79 in Berlin. There was a break in 1980 for the World Congress - MEDINFO 8- in Tokyo. This was followed by MIE 81 in Toulouse. The rationale behind these congresses is the scientific need to share results and ideas. and the educational need to train a wide variety of professional staff in the potential of Medical Informatics in health care delivery. All the caring professions are involved. doctors. scientists. nurses. pharma- cists. paramedical staff. administrators. health care planners. commu- nity physicj-US'I,!Dedical educationalists. epidemiologists. statisti- cians. o. pe-~atio'ns 'analysts. together with specialists from the comput- ing profession dealin~ with systems analysis. hardware. software. lan- guages. data ba,s~s and marketing of systems. The pre-publi~~tion of'conference proceedings from a multi-stream con- ference is partic~l~t~y valuable in a rapidly expanding multidisciplin- ary field su~h as'M~dical Informatics. It enables participants to fol- low work presented at sessions that they are unable to attend. More im- portantly. is also provides a permanent record with relevant bibliogra- phy for other workers to assess which groups are active and in which areas. All the papers have been refereed and the referees' suggestions incorporated in the final texts. Rapid publication. using camera-ready copy. reduces the time available for editing and indexing.

SALUTIS UNITAS, Unity for Health, an organization 'Uniting physicians of the world to foster better health for all people through international exchange of technological information', was officially founded in 1972 after preceding years of preparation. One of its main objectives is the scientific communication, be it by meeting or by publications. So several scientific meetings could be held, mostly together with other scientific organizations, and resulted in the publication of the respective proceedinqs 1. Studies were commissioned 2 to investigate trends in health care delivery and their socio-economic consequences, which also provided the basis for further investigations 3. So also in 1978 an international congress was organized by SALUTIS UNITAS and sponsored by the international Menarini Foundation under its directorship of Dr. Gorini. This volume is based on the contributions of SALUTIS UNITAS mewbers of this meeting 'Man and his World, Technology and Health', supplemented by further studies and materials. The main objective was to look at various recipients of health care delivery: the individual, the community, the region and the nation. Emphasis was placed on systems analysis, simulation techniques and evaluation. According to the composition of the SALUTIS UNITAS membership, conceptual and interdisciplinary aspects prevail over descriptions of in-depth methodology,thus providing a broad access to the problems investigated. The modern development of medicine has been characterized by the grow- ing use of new technologies in health care delivery and research. As an empirical science, medicine is based on many types and quantities of information to recognize alterations, explore causes and apply cor- rective action. Dealing with biological objects, signals have to be collected, processed and interpreted to recognize the state of this object. It is therefore understandable that data processing technology and informatics have been employed to a growing extent in medicine. The increasing economic repercussions of modern medicine lead also to the demand of ways and means to assess the system as such and to develop means for evaluation and regulation. However, the application of data processing to the medical field has very often grown in parallel to and remote from the development of in- formatics and data processing in general. Furthermore, difficulties have occurred resulting from the differing concepts of reasoning, deci- sion making and methodology. We therefore decided to start a series of seminars with the attempt to bring scientists from both medicine and informatics together to discuss basic principles of informatics and medicine and to attempt a synthesis between the problems in medicine and health care delivery and methods in informatics to approach a solu- tion of these problems. This volume contains the lecture notes of the first seminar of this type.

Drei Jahre sind vergangen, seit im Januar 1979 auf einer Arbeitstagung der Deutschen Gesellschaft fur Medizinische Dokumentation, Informatik und Statistik e.V. (GMDS) und der Gesellschaft fur Rechts- und Verwal- tungsinformatik e.V. (GRVI) juristische Probleme der Datenverarbeitung in der Medizin diskutiert wurden. Die Ergebnisse dieser Veranstaltung, welche in der gleichen Reihe veroeffentlicht worden sind (Band 12; Kilian, W., Porth, A.J.: Juristische Probleme der Datenverarbeitung in der Medizin, Springer-Verlag, Berlin/Heidelberg, 1979), fanden grosses Interesse und dienten als Anregung fur weitere Diskussionen. In der Zwischenzeit liegen Erfahrungen mit dem Datenschutz in der Medi- zin vor. Die Jahresberichte der Datenschutzbeauftragten beleuchten zu- nehmend diesen Aspekt. Daruber hinaus haben neue Gesetze (z.B. Sozial- gesetzbuch), Gesetzentwurfe (z.B. Krebsregistergesetz) und AEnderungen im arztlichen Berufsrecht (Schweigepflicht und medizinische Forschung) die Diskussionen uber den Datenfluss im medizinischen Bereich und dessen Kontrolle belebt. So erschien die erneute Veranstaltung einer Arbeits- tagung sinnvoll. Sie wurde von den Herausgebern des Sammelbandes orga- nisiert und von GMDS und GRVI sowie der gemeinsamen Kommission unter- stutzt. Der Aufbau der einzelnen Beitrage ist etwas unterschiedlich, da der Stand der weiteren Ausarbeitung der Referate variiert. Teilweise sind die Referate als Thesenpapier zusammengestellt, teilweise stellen sie tiefergehende Ausarbeitungen der Problematik dar. Im Gegensatz zu dem fruheren Tagungsband oblag die Herstellung der Manuskripte den Autoren selbst. Daraus mag ein hin und wieder etwas unterschiedliches Druckbild resultieren; den Herausgebern schien es wichtig, dass der Tagungsband zu den aktuellen Problemen schnell erscheinen konnte.

Am 3. und 4. Marz 1982 fand an der Freien Universitat Berlin eine Ar- beitstagung mit dem Thema "Erfahrungen mit dem Anwendungsfach Medizin im Rahmen eines Informatikstudiums" statt. Eingeladen hatte hierzu der FachausschuB 14 "Anwendungen in der Medi- zin" der Gesellschaft fUr Informatik (GI); und unterstUtzt wurde die Veranstaltung von dem Fachbereich Medizinische Informatik der Gesell- schaft fUr Medizinische Dokumentation, Informatik und Statistik (GMDS). Teilnehmer waren Dozenten, Absolventen und Interessenten von Studien- gangen der Fachrichtung Informatik mit dem Anwendungsfach Medizin. Ziel war die Diskussion der bisherigen Erfahrungen sowie die Entwicklung von Vorschlagen und Anregungen fUr die weitere Gestaltung dieser Studien- richtung. Dabei standen zwei Modelle zur Diskussion: Einmal der Diplomstudiengang Informatik mit dem Anwendungsfach Medizin und einem Nebenfachanteil von 20 - 50% an den lehrveranstaltungen, zum anderen das Studienmodell Hei- delberg/Heilbronn mit einer starkeren Betonung der medizinischen Grund- lagen und der Prinzipien der Medizinischen Informatik, welches zur Ver- leihung des Diploms eines Informatikers in der Medizin fUhrte. In beiden Studienmodellen wird angestrebt, die Absolventen zu befahi- gen, spater auch in einem anderen Anwendungsfach als Informatiker zu arbeiten. 1m Hintergrund der Tagung stand das sogenannte Reisensburger Protokoll aus dem Jahre 1973, das eine Diskussion zwischen Vertretern der GI und GMDS zusammenfaBt und in dem Grundsatze fUr den Aufbau eines Anwen- dungsfachs Medizin im Rahmen eines Informatikstudiums eben so niederge- legt wurden wie die Prinzipien fUr die Weiterbildung zum Medizinischen Informatiker.
