1. Das hervorstechende Symptom. Krankheitsfalle zeigen uns, wie wir zu sagen pflegen, gewisse fuhrende Symptome. Wie ein Soldat, der sein Gewehr prasentiert, schieben sie Klagen wie Schmerz, Husten oder Nervositat in den Vordergrund, so dass sie aus dem klinischen Bilde hervortreten. Solche "hervorstechende Symptome," ver gleichbar den "vorliegenden Teilen" in der Geburtshilfe, konnen sich als weniger wichtig erweisen, wenn wir den ganzen Fall studiert haben; aber im Beginn konnen sie uns zu richtigen oder falschen Schlussen in der Diagnose, Prognose und Behandlung fuhren, je nachdem wir die Kunst, sie richtig zu ver folgen, gelernt haben oder nicht. Dieses Buch ist ein Versuch, die Medizin von dem Standpunkt des "fuhren den Symptoms" aus zu betreiben. Ich hoffe zu zeigen, wie die Klagen des Patienten - bestimmte Ausdrucke uber die zugrunde liegende Erkrankun- als Fuhrer benutzt werden sollen, und wie man so bis zu dem wirklichen Sitze der Krankheit vordringen kann. Der so umrissene Plan setzt sich aus drei Teilen zusammen: 1. eine Zusammenstellung der gewohnlichen Ursachen der Symptome zu geben, uber die am haufigsten von Patienten geklagt wird, wie z. B. die Ursachen der Ruckenschmerzen, des Erbrechens oder der Hamaturie; 2. diese Ursachen, soweit es moglich ist, nach ihrer Haufigkeit zu ordnen; 3. sie durch Krankengeschichten zu veranschaulichen, in denen das fuhrende Symptom bis zu dem diagnostischen Problem verfolgt und dessen Losung dargeboten wird. 2. Die Anordnung vernunftiger Moglichkeiten. Diagnosen werden verfehlt, gewohnlich, weil physikalische Zeichen der Erkrankung nicht erkannt werden, gelegentlich, weil wir nicht scharf denken."

Bei der Vorbereitung eines Lehrbuches der Differentialdiagnose innerer Krankheiten kam mir das Original der vorliegenden Bearbeitung zu Handen, das wie die meisten englischen Monographien bei uns ganz unbekannt geblieben war. Ich hatte die Absicht gehabt, in ahnlicher Weise das Gebiet zu behandeln und fand hier meinen Plan von einem amerikanischen Kliniker und Forscher in nahezu vollkommener Weise ausgefuhrt. Das Studium des Buches fesselte mich ungemein; nach einigem Zoegern gab ich meinen Plan auf. Hier lag schon alles vor, wie ich es mir gedacht hatte. Die Urteile uber die Berechtigung, Differentialdiagnostik in besonderen Lehrbuchern zu behandeln, sind sehr schwankend. Manche klinische Lehrer halten solche Bucher fur uberflussig und sehen sie als unerwunschte Erleichterung des Studiums an, die dem medizinischen Denken und Muhen des Anfangers Gefahr bringen; in den Lehrbuchern der inneren Medizin findet sich ja auch alles auf die Differentialdiagnose Bezugliche besprochen. Das gilt aber nur insofern, als alles, was die physikalischen Ergebnisse der Untersuchung be- trifft, behandelt ist. Das wird in unserem Buche vorausgesetzt. Aber der Arzt, der dies alles beherrscht, kann doch ein schlechter Diagnostiker bleiben, wenn er es nicht versteht, die durch die Untersuchung gewonnenen Befunde zu deuten und miteinander zu einem befriedigenden Ganzen zu vereinen. Dazu gehoert eine ausgereifte klinische Erfahrung und die Fahigkeit, konstruktiv zu denken.
