Our pocket-book has come about as a direct attempt to answer the needs of our clinical students. We have tried to use radiology as a magic window for looking at their patients' medical and surgical problems. The book is very simple and highly selective. A number of excellent introductory texts already exist for students seeking comprehensive and balanced accounts of radiology as a specialty. We have tried to keep close to our title: a good guide shows you a bare outline of where you might go, and makes sure you see the highlights. He will point to interesting places that deserve study, without going into them himself. Occasionally he may enlarge on a topic when the information is not readily available anywhere else (as in our chapter on the skull). And he will be ready to listen to students asking, perhaps rather shyly, some basic questions (as in our first and last chapters). We have taken Voltaire as our own guide: "Le secret d'ennuyer est . . . de tout dire." The book has been written by only a small group of all those teaching radiology at Cambridge. This pocket introduction for first year clinical students starting clinical training on the ward provides basic information about how to talk to patients, take a medical history and how to start a clinical examination. The result of many years of clinical teaching experience at the University of Cambridge, this second edition is clearly thought out. With those first clinical encounters in mind, the text has been simplified to allow easy distinction of basic essential information from more detailed discussion. New clinical terms and issues have been included and the text further enlivened by clear line illustrations.

The construction of a US-Mexico border fence has proved an increasingly divisive method for securing the southern US border since its initial construction as the San Diego Fence in 1990. Since the terrorist attacks of September 11, 2001 the construction of a US-Mexico border fence has morphed from a method to stem the tide of illegal immigration from Mexico into a legislated method to prevent the entrance of terrorists. All terrorists except two have gained entrance into the US through manipulation of the US immigration system. Those two exceptions crossed the US-Canada border. This thesis analyzes what threat Mexico poses as a transit for terrorists attempting to enter the US by surreptitious means, and what deterrent or defeat effect a US-Mexico fence provides. The absence of a terrorist attempt to gain entrance into the US via Mexico indicates that Mexico does not provide the essential support required for an Al Qaeda terrorist, and that other countries and other means of entrance provide a more viable option and likelihood for success. The one support element that could provide a terrorist entrance from Mexico is that of the "coyote." A strategy of vigorous enforcement against illegal immigration without combating the motives for illegal immigration has helped to enable the "coyote" to gain the capability to transport a terrorist into the US from Mexico. The US-Mexico border fence has a limited deterrent effect against terrorists as demonstrated by the migration of illegal immigrants to more remote crossing sites. Finally, this thesis provides recommendations to better secure US land borders.
