Die rechtzeitige Erkennung von Patienten, die durch den plotzlichen Herztod gefahrdet sind, stellt unveriindert ein zentrales Problem der Kardiologie dar. Zahlreiche Untersuchungen der letzten 10-15 Jahre haben die Kenntnisse der zum plotzlichen Herztod fuhrenden pathophysio- logischen Mechanismen erweitert und zu einem besseren Verstiindnis der verantwortlichen Faktoren gefiihrt. Dies gilt in besonderem MaBe fUr die koronare Herzkrankheit, trifft jedoch nur bedingt auf andere Herzerkrankungen zu, bei denen ebenfalls eine Gefahrdung durch den plotzlichen Herztod besteht. Die vorliegende Darstellung basiert auf einer wissenschaftlichen Sitzung der Arbeitsgruppe "Herzrhythmusstorungen" der Deutschen Gesellschaft fur Herz-und Kreislaufforschung. Ziel der von Experten abgegebenen Stellungnahmen war es, die fur die einzelnen Krankheitsbilder als .gesichert geltenden Befunde, die eine Erkennung von Risikopatienten ermoglichen, darzu- stellen. Gleichzeitig sollte auf die noch vorhandenen Wissenslucken eingegangen werden. Die gewiihlte Form der Darstellung solI es dem Kliniker und dem niedergelassenen Internisten und Kardiologen ermoglichen, sich rasch uber das derzeit als gesichert anzusehende Wissen als Basis fUr eine rationelle Therapieentscheidung zu informieren.
