keep the quality of these images, so they represent accurately the
original artefact. Although occasionally there may be certain
imperfections with these old texts, we feel they deserve to be made

Secrecy and Community in 21st-Century Fiction examines the relation
between secrecy and community in a diverse and international range
of contemporary fictional works in English. In its concern with
what is called 'communities of secrecy', it is fundamentally
indebted to the thought of Jacques Derrida, Jean-Luc Nancy and
Maurice Blanchot, who have pointed to the fallacies and dangers of
identitarian and exclusionary communities, arguing for forms of
being-in-common characterized by non-belonging, singularity and
otherness. Also drawing on the work of J. Hillis Miller, Derek
Attridge, Nicholas Royle, Matei Calinescu, Frank Kermode and George
Simmel, among others, this volume analyses the centrality of
secrets in the construction of literary form, narrative sequence
and meaning, together with their foundational role in our private
and interpersonal lives and the public and political realms. In
doing so, it engages with the Derridean ethico-political value of
secrecy and Derridaās conception of literature as the exemplary
site for the operation of the unconditional secret.
This book describes the application of novel signal processing
algorithms to improve the diagnostic capability of the blood oxygen
saturation signal (SpO2) from nocturnal oximetry in the
simplification of pediatric obstructive sleep apnea (OSA)
diagnosis. For this purpose, 3196 SpO2Ā recordings from three
different databases were analyzed using feature-engineering and
deep-learning methodologies. Particularly, three novel feature
extraction algorithms (bispectrum, wavelet, and detrended
fluctuation analysis), as well as a novel deep-learning
architecture based on convolutional neural networks are proposed.
The proposed feature-engineering and deep-learning models
outperformed conventional features from the oximetry signal, as
well as state-of-the-art approaches. On the one hand, this book
shows that bispectrum, wavelet, and detrended fluctuation analysis
can be used to characterize changes in the SpO2Ā signal caused
by apneic events in pediatric subjects. On the other hand, it
demonstrates that deep-learning algorithms can learn complex
features from oximetry dynamics that allow to enhance the
diagnostic capability of nocturnal oximetry in the context of
childhood OSA. All in all, this book offers a comprehensive and
timely guide to the use of signal processing and AI methods in the
diagnosis of pediatric OSA, including novel methodological insights
concerning the automated analysis of the oximetry signal. It also
discusses some open questions for future research.

This book critically analyses the influence of international
policies and guidelines on the performance of interventions aimed
at reducing health inequities in Latin America, with special
emphasis on health promotion and health in all policies strategies.
While the implementation of these interventions plays a key role in
strengthening these countries' capacity to respond to current and
future challenges, the urgency and pressures of cooperation and
funding agencies to show results consistent with their own agendas
not only hampers this goal, but also makes the territory invisible,
hiding the real problems faced by most Latin American countries,
diminishing the richness of local knowledge production, and
hindering the development of relevant proposals that consider the
territory's conditions and cultural identity. Departing from this
general analysis, the authors search for answers to the following
questions: Why, despite the importance of the theoretical advances
r egarding actions to address social and health inequities, haven't
Latin American countries been able to produce the expected results?
Why do successful initiatives only take place within the framework
of pilot projects? Why does the ideology of health promotion and
health in all policies mainly permeate structures of the health
sector, but not other sectors? Why are intersectoral actions
conjunctural initiatives, which often fail to evolve into permanent
practices? Based on an extensive literature review, case studies,
personal experiences, and interviews with key informants in the
region, Globalization and Health Inequities in Latin America
presents a strategy that uses monitoring and evaluation practices
for enhancing the capacity of Latin American and other low and
middle-income countries to implement sustainable processes to
