George Pitt-Rivers began his career as one of Britain's most promising young anthropologists, conducting research in the South Pacific and publishing articles in the country's leading academic journals. In der Wissenschaft vollzieht sich der Fortschritt niemals kontinuierlich im gleichen Tempo. Auf Perioden stiirmischer Entwicklung folgt stets eine Zeit der Ruhe, der kritischen Betrachtung und Auswertung des Erreichten, bis dann wieder frische Impulse das schon mit Erstarrung Bedrohte mit neuer Dynamik erfiillen. Ein klassisches Beispiel hierfiir ist die Magenchirurgie. Die erste, von THEODOR BILLROTH im Jahre 1881 erfolgreich durchgefiihrte Magenresektion eroGBPfnete ein chirurgisches Neuland von zunachst uniiberschaubarer Weite. Sie diente der mor- phologischen Beseitigung eines stenosierenden Pyloruscarcinoms. Auch die im September desselben Jahres von RYDYGIER durchgefiihrte erste Resektion eines peptis chen Ulcus, die iibrigens von seinen Zeitgenossen aUGBPs scharfste abgelehnt wurde, war zunachst nur zur technischen Beseitigung des Geschwiirs gedacht. Erst spater wurde die Ulcusresektion aufgrund unzahliger klinischer und experi- menteller Arbeiten zur kausalen Therapie ausgebaut. Fast 50 Jahre lang schienen Indikationen und Technik der chirurgischen MaBnahmen im groBen geklart und festgelegt. Und damit verlor dieses chirurgische Teilgebiet auch an allgemeinem Interesse. Seit etwa 20 Jahren steht es nun wieder zunehmend im Mittelpunkte lebhafter Diskussionen. Die Impulse kamen einmal von den erweiterten Moglich- keiten chirurgischer, insbesondere thoraxchirurgischer Technik, zum anderen von den neuen Erkenntnissen der Pathophysiologie und Biochemie. Wie allgemein in der Medizin tritt die Morphologie gegeniiber der funktionellen Betrachtung und Behandlung in den Hintergrund. So werden wir auch in der in Bewegung geratenen Magenchirurgie manches neu durchdenken und umlernen miissen.
