In this innovative new book, Steve Selvin provides readers with a
clear understanding of intermediate biostatistical methods without
advanced mathematics or statistical theory (for example, no
Bayesian statistics, no causal inference, no linear algebra and
only a slight hint of calculus). This text answers the important
question: After a typical first-year course in statistical methods,
what next?
This book comprehensively addresses the use of pulmonary function measurement for the evaluation, screening and timing of noninvasive mechanical ventilation (NIMV) from hospital to home care. To do so, it describes three clinical stages of NIMV support: before NIV, to detect early markers and determine whether NIV is appropriate; during NIV, to evaluate NIV response; and in long-term NIV support. Additionally, it assesses a range of complementary health care organizations (pulmonary function labs, pneumology wards, semi-intensive care units and home mechanical ventilation programs), techniques (chest physiotherapy/airway secretions, etc.) and applications. In closing, the book offers practical recommendations on how noninvasive ventilation and lung function measurement can improve outcomes and quality of life, making it a valuable resource for all specialists, e.g. intensivists and pneumologists, as well as anesthesiologists and therapists.
Launched on Oxford Medicine Online in 2012, with the full-text of
eight Mayo Clinic Scientific Press (MCSP) print titles and a bank
of multiple-choice questions, Mayo Clinic Toolkit provides a single
location for resident, fellow, and practicing clinicians to
undertake the self-testing necessary to prepare for, and pass, the
Boards.
A knowledge of anatomy has always played a vital role in medicine, and is especially important in the everyday practice of anaesthesia. This 2002 text presents the core anatomical knowledge required for the Primary and Final FRCA examinations in a simple and straightforward manner. Adopting the philosophy of the examination itself, where candidates are often required to reproduce simple line drawings to illustrate the depth of their knowledge, the many illustrations in the book are clear and easy to understand at a glance. The illustrations are accompanied by explanatory text throughout and for each anatomical region, sample examination questions are included which follow the format of those asked in the examinations. This book will interest anaesthesia Primary and Final candidates, as well as providing a valuable quick reference for busy anaesthetists at all levels of experience.
Orofacial pain - defined aspain lasting over three months in the
mouth or face - is increasing in prevelance, with approximately 7%
of the UK population having experienced it. It is currently managed
by either dentists or general medical practitioners, both of whom
have radically different approaches to treatment. While management
of orofacial pain has improved quite dramatically over recent
years, it is essential that dentists and general practitioners are
aware of these advances and of the differences in their approaches.
Back Pain: The Facts is aimed at people who want to help
themselves. Affecting nearly everyong at some time in their lives,
40% of people will have experienced some form of back pain within
the last year, with around 5% of sufferers taking time off from
work within the last month. Long term back pain causes a great deal
of distress and unhappiness, affecting work life, income, home
life, relationships, fitness, and mood.
Neuromodulation implants are currently one of the most successful techniques in the treatment of chronic pain, a condition that affects more and more patients each year, leading to reduced quality of life as well as economic losses.Neuromodulation implants for pain are a relatively new technique and are being increasingly used around the globe. However, technical training is not available for every implanter as there are relatively few centers in the world. Further, refresher courses for low-rate implanters are not available. This book fills that gap by offering an update on the best techniques currently used, providing a step-by-step guide on how to perform these procedures correctly. Based on the experiences of leading physicians in the field, it also discusses how to manage both major and minor complications. Lastly, it covers the neuromodulatory intervention techniques used to treat chronic pain in various clinical areas. As such it is a valuable resource for pain physicians as well as neurosurgeons, orthopedists and anesthetists.
Visceral pain, or pain in the internal organs, is often quite
difficult for the clinician to recognize promptly as its
characteristics are often atypical and vary with time. Patients
with visceral pain also often present with concurrent painful
conditions in more than one internal organ; this can give rise to
an intricate complex of symptoms which can be misleading if not
accurately evaluated. Also, despite its frequency, the mechanisms
behind pain in the internal organs are not fully understood, and
this impacts negatively on therapy.

Both pain and addiction are tremendous public health problems.
Practitioners of every stripe say that they learned precious little
about pain or addiction in their training and readily admit that
instruction on the interface of pain and addiction is nonexistent.
The recent problem of prescription drug abuse has only served to
highlight the fact that these two worlds need unificationthose who
treat pain must be informed about the risks of controlled
substances and those who treat addiction need to better and more
fully understand their benefits. Nowhere is the pooled knowledge of
pain management and addiction medicine brought together to allow
for a greater appreciation of the risks of addiction when treating
people with pain and the pain problems of those with chemical
dependency. This major new volume brings this vast knowledge base
together, presenting an array of perspectives by the foremost
thought leaders at the interface of pain and chemical dependency,
and is the most comprehensive resource on the subject to
date.
This book highlights remarkable new endoscopic, laparoscopic, and thoracoscopic approaches to the removal of surgical lesions for different pathologic conditions under general endotracheal anesthesia in infants and children. It details how newer techniques in airway management, monitoring, regional nerve blocks for these innovative minimally invasive approaches have resulted in a decrease in intraoperative and postoperative morbidity and early recovery process after pediatric anesthesia. This unique book contains features that provide the audience with several clinical scenarios where exceptional surgical outcomes are achieved with optimum pre-op preparation via collaborative team efforts. To date there is no other textbook emphasizing the anesthetic and surgical management during the most innovative advances in endoscopic surgery in children. Per oral endoscopic myotomy (POEM) for the definitive treatment of achalasia in children is probably the most advanced endoscopic surgery done successfully in children. POEM is the best example of endoscopic surgery performed via natural orifices known as the Natural orifice transluminal endoscopic surgery (NOTES). Another new laparoscopic surgical intervention -Median Arcuate Ligament surgical release for Median Arcuate Ligament Syndrome (MALS) for patients with Postural Orthostatic Tachycardia Syndrome (POTS) is described with established perioperative protocols emphasizing the need for early admission, intravenous hydration, and premedication. Novel approaches in the anesthetic management in children with short bowel syndrome for bowel lengthening techniques like the serial transverse enteroplasty (STEP) and in teenagers for laparoscopic bariatric surgery with adjustable gastric banding (AGB) and vertical sleeve gastrectomy (VSG) for morbid obesity are new areas that would enlighten the readers. Anesthetic Management in Pediatric General Surgery is an invaluable resource for pediatric anesthesiologists, surgeons, and their trainees specializing in the care of pediatric patients.
Chronic pain affects an estimated 14 million people in the UK and is the most common reason for seeking medical help. There's increasing evidence that mindful practices can have a beneficial effect on pain (sometimes reducing intensity by as much as 40%) and its associated anxiety and depression. This book combines the principles of mindfulness with research from the clinical field of psychology to help you improve your emotional, psychological and physical state. With guided meditations and other practices. Topics include: Defining pain How we feel and perceive pain How a mindful approach can help Why stress makes it worse Mindful body movement The importance of breathing and posture Taking responsibility for your pain
This book presents a timely and multidisciplinary update on the modalities currently available for treating the most feared symptom of patients diagnosed with cancer. The various cancer pain syndromes are explored in detail, covering those related directly to malignancy and those due to the after-effects of cancer therapy. Treatment modalities, including pharmacologic approaches, interventional procedures, and palliative surgical options, are discussed clearly and concisely, with provision of recommendations for the practitioner. Further topics include new and emerging treatments for cancer pain, survivorship considerations, pain management in special populations, and implementation of systems-based pain programs. The book has been written by a multidisciplinary group of experts, reflecting the evolution in pain and symptom management that has occurred in parallel with progress toward more targeted oncologic treatments. Oncologists, palliative care physicians, allied health professionals, and other practitioners involved in caring for cancer patients will find Fundamentals of Cancer Pain Management to be a rich source of evidence-based insights into effective pain management.
This book is a guide for any clinician who has a pain patient, including biomedical/allopathic and complementary practitioners. The reader can use each chapter independently when considering a referral to the specific complementary approach, or as an educational tool to learn about the various massage, movement and mindfulness (MMM) approaches and provide comprehensive, evidence-informed support for patients seeking pain relief.
The last decade has seen major advances in the neurobiology of
pain, primarily resulting from a deeper understanding of the way in
which pain signals are coded and processed in the nervous system.
This volume in the Molecular and Cellular Neurobiology is the first
book for many years to present an integrated overview of the
current state of research into the neurobiology of chronic and
acute pain.
This book discusses all thoracic diseases of surgical interest, from thorax malformations to airways disorders. It is divided into six main sections; the first two analyze general aspects, while the following three focus on malformations of the thorax, airways, esophagus and diaphragm. The final section describes the main pediatric tumors involving the chest and internal thoracic organs. Each chapter starts with a classification of the main pathologies related to the anatomical region considered. For each pathology, the various aspects of the diagnostic-therapeutic procedure are described in details - from the clinical presentation and diagnosis to the pre-operative preparation, the surgical aspects, and the post-operative course. Possible future developments are also evaluated. The volume will be a useful tool for specialists in pediatric and thoracic surgery, but will also represent an educational resource for medical and nursing students.
Ultrasound has revolutionized the practice of regional anesthesia, yet there remains a paucity of good resources on ultrasound-guided regional anesthesia in children. This book offers a much-needed practical guide to all the major ultrasound-guided blocks in pediatric patients, including neuraxial, truncal, upper and lower limb blocks. The core principles of good clinical practice in regional anesthesia are described and discussed, including the pharmacology of local anesthetics in children, the performance of regional anesthesia, the management of complications, and the clinical anatomy of each block. Every block chapter provides both a 'how to' section and also a comprehensive literature review, with an up-to-date and relevant bibliography for reference and further reading. Chapters are illustrated with unique anatomical images and detailed descriptions. Both trainee and experienced anesthesiologists will find this an essential resource for the safe and effective performance of modern regional anesthesia in children.
In most patients, pain medication, along with physical therapy and supportive counselling, adequately controls the pain of terminal disease, but in some cases pain medication fails or produces unacceptable side effects, and other more invasive interventions may be used. This practical book provides comprehensive and easy-to-follow guidelines on nerve blocking and neuromodulation techniques to help patients and professionals make choices in pain management. Patients selection and appropriate referral are discussed as well as ethical issues and consent. This book will be an invaluable source of information for a variety of professionals working with patients with advanced disease, including palliative care doctors and specialist nurses, as there is a scarcity of consultants in pain management in the field of palliative care. Some healthcare professionals may not have experience of the full range of techniques that may benefit their patients, therefore limiting the choices available to patients with uncontrolled pain in the context of palliative care. This book will ensure that the full range of techniques are considered to provide excellent care for patients with pain that is difficult to manage.
It may come as something of a surprise that pain, the most prevalent symptom in clinical practice, is not always addressed specifically in health professions training. Approximately one in six Americans lives with chronic pain in addition to the millions that experience acute pain each day. Half of older adults live with chronic pain-associated conditions, and about half of all healthcare visits are initiated because of pain. Despite this, reports indicate that the vast majority of health professions schools in the United States do not teach required courses on pain, and the total amount of content pertaining to pain is a fraction of a percent of the total. Almost certainly, the lack of education in coordinated, comprehensive, compassionate care for pain-associated conditions contributed to pervasive opioid over-prescribing and the ensuing wave of addiction and deaths that swept the country in the first part of this century. This book is our response to the pain care crisis - it is designed to prepare young clinicians to assess and treat a wide variety of pain conditions in a manner that balances competence and compassion, incorporating coordinated elements of pharmacological and non-pharmacological therapies. Designed to be read during or after pre-licensure training, e.g. medical, nursing, pharmacy school, and to inspire students to learn more about painful conditions, this book is unique in its clinical focus and the level of detail that is included. This book aims to improve pain care, most especially if used alongside a formal pain care course as part of pre-licensure training, whether spread over four years or condensed into a shorter period. Through engagement in the interprofessional curriculum planning process, the content of the book has been shaped to align with the International Association for the Study of Pain (IASP) interprofessional pain curriculum vision and to focus on the primary questions of: What is pain? How is pain assessed? How is pain managed? How does clinical context influence pain?
This book uses a case-based approach to provide current information on a range of medical issues with the goal of enhancing preoperative evaluation and optimization. It meets the market need for a resource that concisely encapsulates current knowledge on the medical management of specific topics in a setting relevant to the preoperative clinic. In so doing the book aims to improve patient care and safety, enhance resource use, facilitate appropriate and timely management of preexisting conditions and diminish patient concerns. Organized into sections according to body system, each section consists of chapters delineating a specific disorder. Each chapter starts with a clinical vignette followed by a question-and-answer style investigation of the relevant issues. These questions attempt to address commonly encountered clinical dilemmas where opinion often differs between, and occasionally within, medical sub-specialties. Expertly written chapters are also supplemented by a number of chapters which address special considerations such as the frail patient and chronic opioid use. Preoperative Assessment: A Case-Based Approach is an invaluable reference for all physicians involved in preoperative assessment including anesthesiologists, surgeons, internists, family doctors and residents in these fields. Nurse practitioners and other allied heath professionals involved in preoperative evaluation may also find this a book a valuable and timely resource.
The product of six years of collaborative research, this fine biography offers new interpretations of a pioneering figure in anaesthesiology, epidemiology, medical cartography, and public health. It modifies the conventional rags to riches portrait of John Snow by synthesising fresh information about his early life from archival research and recent studies. It explores the intellectual roots of his commitments to vegetarianism, temperance, and pure drinking water, first developed when he was a medical apprentice and assistant in the north of England. The authors argue that all of Snow's later contributions are traceable to the medical paradigm he imbibed as a medical student in London and put into practice early in his career as a clinician: that medicine as a science required the incorporation of recent developments in its collateral sciences, chiefly anatomy, chemistry, and physiology, in order to understand the causes of disease. Snow's theoretical breakthroughs in anaesthesia were extensions of his experimental research in respiratory physiology and the properties of inhaled gases. Shortly thereafter, his understanding of gas laws led him to reject miasmatic explanations for the spread of cholera, and to develop an alternative theory in consonance with what was then known about chemistry and the physiology of digestion. Using all of Snow's writings, the authors follow him when working in his home laboratory, visiting patients throughout London, attending medical society meetings, and conducting studies during the cholera epidemics of 1849 and 1854. The result is a book that demythologises some overly heroic views of Snow by providing a fairer measure of his actual contributions. It will have an impact not only on the understanding of the man but also on the history of epidemiology and medical science. |
