Roger Dean Smith, Mireille Truong
R281 Discovery Miles 2 810 Ships in 10 - 15 working days

For every complex and expensive system, there emerges a need for training devices and scenarios that will assist new learners in mastering the use of the device and understanding how to apply it with value. This has proven to be true in aviation, nuclear power control, and medicine among other fields. Laparoscopic surgery simulators have played a valuable role in improving the practice of surgery over the last 20 years and the same trends and values will likely apply in robotic surgery. The complexity, criticality, and cost associated with the effective application of the da Vinci surgical robot have stimulated the commercial creation of simulators which replicate the operations of this robot. Each of these simulators provides a slightly different perspective and solution to the problem. This book explores the characteristics and differences between all of the currently available devices. The details provided here are structured to equip readers with sufficient knowledge about the simulators to make their own decisions about which best meets their needs. Each of them possesses unique traits which make them valuable solutions for different types of users. It is not our intent to make a universal recommendation of one device over the others. Readers should draw their own conclusions based on their unique needs for a device. The three current simulation devices for the da Vinci robot are the: da Vinci Skills Simulator (Intuitive Surgical Inc.), dV-Trainer (Mimic Technologies Inc.) and Robotic Surgery Simulator (Simulated Surgical Systems LLC). The three simulators which are described in this book offer a different value proposition to potential purchasers and to novice learners. The da Vinci Skills Simulator, dV-Trainer, an RoSS are complex systems which are significantly less costly than the actual da Vinci robotic surgical system and can be operated at a fraction of the cost of the instruments required for this robot. The intent of this book is to present the characteristics of each system to enable intelligent and informed purchasing and usage decisions.

A Surgeon in Khaki - Through France and Flanders in World War I (Paperback, Revised): Arthur Anderson Martin A Surgeon in Khaki - Through France and Flanders in World War I (Paperback, Revised)
Arthur Anderson Martin; Introduction by Glyn Harper
R512 R483 Discovery Miles 4 830 Save R29 (6%) Ships in 10 - 15 working days

Considered by critics to be an accurate portrayal of frontline medical conditions, A Surgeon in Khaki is New Zealand surgeon Arthur Anderson Martin's account of his experiences in 1914, early in World War I. Already a well-respected and widely traveled surgeon when war broke out, Martin joined the Royal Army Medical Corps. Under Field Marshal Sir John French, he served at Le Havre, Harfleur, and at the battle of the Marne. He marched to Aisne, to the new lines behind La Bassee, and finally to Flanders. During his entire service, he advocated immediate specialist surgery for the direst wounds, even under fire. In this engaging narrative, the reader experiences the daily life of war through the eyes of the medical officers who tried valiantly to help the wounded and ill on the front lines of World War I. Martin provides colorful descriptions of the soldiers and officers, harrowing details of the battles, and riveting accounts of the difficulty of treating men in a war zone. A better firsthand account of medicine during World War I is not to be found.

Inflammation in itself is not to be considered as a disease . . . and in disease, where it can alter the diseased mode of action, it likewise leads to a cure; but where it cannot accomplish that solitary purpose . . . it does mischief - John Hunter, A Treatise on the Blood, ITfIlammation, and Gunshot Woundr (London, 1794)1 As we reached the millennium, we recognized the gap between our scientific knowledge of biologic processes and our more limited clinical capabilities in the care of patients. Our science is strong. Molecular biology is powerful, but our therapy to help patients is weaker and more limited. For this reason, this book focuses on the problems of multiple organ failure (MOF), multiple organ dysfunction syndrome (MODS), and systemic inflammatory response syndrome is, patients who have severe injuries; require major, (SIRS) in high-risk patients, that overwhelming operations; or have serious illnesses requiring intensive care; patients who have diseases elsewhere, in other organs or systems, that limit their capabilities to survive a new insult; and patients who are elderly or at high risk for sepsis or other complications. These are the patients who need our help. They need the advances in science, in molecular biology, immunology, pathophysiology, biochemistry, genetics, high technology, and other areas of maximum support at the bedside. These advances could potentially have the greatest impact on improving patient care.

Peter M Schlag
R2,856 Discovery Miles 28 560 Ships in 10 - 15 working days

Rectal cancer is an exceptional tumour entity within the variety of gastrointestinal tumours. While clinical techniques such as rectal digital examination and endoscopy would enable diagnosis as early as the stage of polypoid preneoplasia, only 10% of rectal cancer patients undergo surgery at an early stage of the disease. The prognosis of rectal cancer without infiltration of the muscu- laris is relatively good. However, with locally more advanced tu- mours, possibly associated with lymphnode metastases, the prob- ability of successful treatment decreases considerably. Further- more, there is a clear prognostic discrepancy between proximal and distal rectal cancers. Rectal carcinomas, especially in the dis- tal location, except those diagnosed at an early stage, usually re- quire surgical treatment with postoperatively disturbed or lost anal sphincter function. A permanent colostomy may not be avoidable. The current trends and developments in diagnosis and therapy of rectal cancers may be described as follows: * The search for new diagnostic procedures and the attempt to define risk groups for which intensive preventive measures would be clearly successful. New insights may come from mod- ern molecular biology or molecular genetics, especially with re- gard to hereditary colorectal carcinoma. * A further increase in the rate of sphincter-preserving surgery without impairment of local tumour control or overall prog- nosis. New surgical techniques such as trans sphincteric resec- tion, but also various multimodal local treatment regimens, especially for low-risk tumours, are promising approaches.

Female Circumcision and Clitoridectomy in the United States - A History of a Medical Treatment (Hardcover): Sarah B. Rodriguez Female Circumcision and Clitoridectomy in the United States - A History of a Medical Treatment (Hardcover)
Sarah B. Rodriguez
R2,776 Discovery Miles 27 760 Ships in 12 - 19 working days

An engaging and surprising history of surgeries on the clitoris, revealing what the therapeutic use of female circumcision and clitoridectomy tells us about American medical ideas concerning the female body and female sexuality. From the late nineteenth century through the mid-twentieth century, American physicians treated women and girls for masturbation by removing the clitoris (clitoridectomy) or clitoral hood (female circumcision). During this same time, and continuing to today, physicians also performed female circumcision to enable women to reach orgasm. Though used as treatment, paradoxically, for both a perceived excessive sexuality and a perceived lack of sexual responsiveness, these surgeries reflect a consistent medical conception of the clitoris as a sexual organ. In recent years the popular media and academics have commented on the rising popularity in the United States of female genital cosmetic surgeries, including female circumcision, yet these discussions often assume such procedures are new. In Female Circumcision and Clitoridectomy in the United States: A History of a Medical Treatment, Sarah Rodriguez presents an engaging and surprising history of surgeries on the clitoris, revealing how medical views of the female body and female sexuality have changed -- and in some cases not changed -- throughout the last century and a half. Sarah B. Rodriguez is lecturer in medical humanities and bioethics and in global health studies at Northwestern University.

R1,800 Discovery Miles 18 000 Ships in 12 - 19 working days

Essential Surgery is well-established as one of the leading textbooks of surgery for medical students, core surgical trainees and those in professions allied to medicine. Covering general surgery, trauma, orthopaedics, vascular surgery, urology, paediatric surgery, cardiothoracic surgery, neurosurgery, maxillofacial surgery and ENT, it also incorporates appropriate levels of basic science throughout. The book is ideal to accompany clinical courses, as well as being a practical manual for readers at more advanced levels requiring a revision aid for exams. Its main aim is to stimulate the reader to a greater enjoyment and understanding of the practice of surgery. The uniformity of the writing style and the clarity of elucidation will encourage continued reading, while the emphasis on the principles of surgery will enable a real understanding of the subject matter. The book incorporates a problem-solving approach wherever possible, emphasising how diagnoses are made and why particular treatments are used. The pathophysiological basis of surgical diseases is discussed in relation to their management, acting as a bridge between basic medical sciences and clinical problems. The book's extensive use of original illustrations, colour photographs, radiology images, boxes and tables emphasises important concepts and will promote easy revision of topics. The principles of operative surgery and pre-, peri- and postoperative care are explained together with outlines of common operations, enabling students and trainee surgeons to properly understand procedures and to participate intelligently in the operating department. A major and comprehensive revision of the text has taken full account of the progressive evolution of surgery and includes new scientific and clinical concepts that have advanced medical understanding. Ensuring the book's contents are aligned to the curriculum of the UK Intercollegiate MRCS examination has resulted in the addition of new topics including an emphasis on peri-operative care optimisation, enhanced recovery programmes, antibiotic use and resistance, updated cancer staging, minimally invasive surgery, and the evolving role of interventional radiology. Additional essential governance and management topics are covered, including patient safety, surgical ethics, communication, consent, clinical audit and principles of research. The broad experience of surgical teaching and training of two new editors and many new contributors, all with sub-specialist expertise, have brought a contemporary perspective on the book's contents and its presentation. New national and international guidelines for managing common disorders have been incorporated where appropriate, including major haemorrhage management pathways and trauma. There are new dedicated chapters on major trauma management and elective orthopaedics.
