This book is derived from a series of articles written for dental practitioners and first published in Dental Update. The articles were written as a consequence of running a weekly clinic, concerned largely with the correction of referred endodontic problems. In most cases the 'problems' did not exist and were treated simply by redoing the root filling or by undertaking a straightforward apicectomy. Regrettably, in many other cases, the problem would not have existed had the previously undertaken, and potentially simple, apicectomy been carried out competently. There is no good reason why apical surgery should not be undertaken by the general dental practitioner as part of the patient's overall treatment. After all, the problem will usually have arisen either during, or as a result of, routine dental treatment. In addition, the tooth may require to be restored, either at the time of surgery or fairly soon thereafter. Regrettably, inexperience in the handling of soft tissues, and concern about the risk of damage to associated structures, often deters the dentist from attempting what is, if sensibly approached, an uncomplicated minor dento-alveolar operation. Chapters 1-8 introduce the reader to the basic techniques of apicectomy. Chapters 9, 12, 16 and 17 will, it is hoped, help the interested dentist to develop a competence to treat more complex cases, once he or she has become proficient in the basic skills.

Despite the fact that the incidence of gastric cancer is declining in the Western world, it remains a significant problem with respect to accurate diagnosis and treatment since it has a high mortality rate. In June 1989 an International Conference was held at the University of Rome "La Sapienza" entitled "New Trends in Gastric Cancer: Background and videosurgery." During this meeting background information on the aetiopathogenesis of gastric cancer was presented together with talks and video presentations on the latest advances in the treatment of gastric carcinoma, both from the European and Japanese experience. Because of the poor prognosis of gastric carcinoma there is increasing pressure for early detection. Some of the problems in the early detection of gastric carcinoma are discussed together with methods of surveillance of high-risk subjects. It is generally accepted that the surgical approach to gastric carcinoma should take into account the site and extent of the lesion and there are chapters on new methods for pre and intraoperative staging of the disease which allow a more logical approach to surgery. A comparison between Japanese and Western rule and results was attempted and reasons for the differences were investigated. Since the field is still evolving not all aspects could be covered, and those angles not approached in this book will be addressed in a second International Conference to be held in Rome in June 1990.

This thesis focuses on a novel radio-guided surgery technique for complete tumor resections. It describes all aspects of the intraoperative probe, as well as testing and simulation of the novel technique. This thesis lays the groundwork for the automatic supervision of the laser incision process, which aims to complement surgeons' perception of the state of tissues and enhance their control over laser incisions. The research problem is formulated as the estimation of variables that are representative of the state of tissues during laser cutting. Prior research in this area leveraged numerical computation methods that bear a high computational cost and are not straightforward to use in a surgical setting. This book proposes a novel solution to this problem, using models inspired by the ability of experienced surgeons to perform precise and clean laser cutting. It shows that these new models, which were extracted from experimental data using statistical learning techniques, are straightforward to use in a surgical setup, allowing greater precision in laser-based surgical procedures.
