At last, G. Ian Taylor has written his master work! This amazing book provides the background and the thought processes that led to the angiosome concept, its clinical impact, and the legacy it has left for the benefit of patients and surgeons. The concept is an interpretation of the vascular anatomy of the body as the basis for tissue transfer and the explanation of various pathological processes. Part history, part anatomical atlas, and part clinical application, this major reconstructive work is a virtual treasure trove of important historical, anatomical, and clinical pearls from one of the true masters of reconstructive surgery. In this landmark publication, Dr. Ian Taylor provides readers with a unique perspective on the evolution of reconstructive surgery and the developments that have shaped its path. The book begins with a review of the various surgeons and anatomists whose works heralded the anatomical renaissance of the 1970s and helped crystallize the Angiosome Concept that Taylor and Palmer published in 1987. This initial chapter includes "the journey" of clinical and research events that led to this concept and its legacy. In each case, a unique clinical problem is solved by entering the anatomical dissecting room to design an operation that is customized to the patient's individual needs. In many cases, this original work introduced new surgical procedures to the surgeon's armamentarium, including the "free flap," "the free vascularized bone flap," "the free vascularized nerve flap," and various combinations of tissues for transfer including skin, muscle, tendon, and bone. It has also helped to explain the basis for various pathological processes such as flap tip necrosis, tendon rupture and the ulceration that may be associated with varicose veins and meningococcal septicaemia. The second chapter provides an in-depth explanation of the angiosome concept with its arterial and venous counterparts, embryological, neurovascular, lymphatic, and comparative anatomy studies. The third chapter outlines the various anatomical concepts that have evolved from this study of the angiosomes with their legacy-the basis for the design of various tissues for transfer. It includes: analysis of the "delay phenomenon"; the anatomical and clinical territories of skin perforator flaps; and personal experience with various tissue transfers, including breast reduction and reconstruction, vascularized bone repair of head, neck, and limb defects, and the free vascularized transfer of nerve, tendon, and muscle. The chapter also discusses the impact of the recently developed technique of CT angiography on preoperative planning. The last four chapters focus on the regional angiosomes of the head and neck, torso, upper limb, and lower extremity with their clinical applications. Two DVDs supplement the book and contain key papers and films produced by the author and colleagues.

Learn Gluteal Sculpting from the Master! Gluteal sculpting procedures have become increasingly popular today as patients requesting body contouring improvements recognize that a nicely shaped buttock provides balance and proportion to the total body aesthetic. The Art of Gluteal Sculpting, by Dr. Constantino G. Mendieta, an acknowledged expert in this area, is the definitive work on this topic. The dramatic increase in requests for gluteal contouring procedures requires that plastic surgeons develop not only a mastery of a range of body contouring procedures, but also the ability to recognize each patient's body frame type so that appropriate interventions can be planned. This book provides surgeons with the comprehensive, clinical information they need to meet their patients' demands. Comprehensive Coverage This magnificently illustrated semi-atlas work consists of seven chapters in two parts. Part I begins with a seminal chapter, Evaluation Based on Anatomic Landmarks, that sets the tone for the entire book and demonstrates the importance of anatomy in influencing outcomes. In this invaluable chapter, Dr. Mendieta provides his comprehensive evaluation system developed and refined over an eight-year period. This system represents "another way of seeing"; it provides the foundation for all of the techniques that follow with critical guidelines for assessing specific anatomic components and developing a roadmap for treating individual patients. The second chapter, Patient Selection and Clinical Decision Making, takes the principles established in the evaluation system and applies them to a range of patients with different body types, quantities of fat, and anatomic configurations. From this clinical application a treatment plan is derived and recommended for each patient type discussed. Part II focuses on different techniques for gluteal reshaping. A detailed chapter on fat grafting introduces this section; it provides step-by-step details on Dr. Mendieta's technique for large volume liposuction and fat grafting in the gluteal area. This is his preferred method used in the majority of cases, and he details its advantages, limitations, and potential outcomes. The next two chapters focus on techniques for implant augmentation: one on intramuscular implant placement, and the second, contributed by the noted surgeon, Dr. Jose de la Pena, on subfascial implant augmentation. The final two chapters focus on excisional techniques. Chapter 6 describes Dr. Mendieta's exciting new Miami Thong Lift for Inner Gluteal Fold Excision, and Chapter 7 provides a detailed description of Circumferential Body and Buttock Lifting, by renowned surgeon Dr. Al Aly. Beautifully Illustrated The four-color artwork depicts the surface anatomy as well as the underlying structures to enhance understanding. Numerous case examples demonstrate the excellent outcomes from these procedures. Step-by-step technique is demonstrated for each of the procedures discussed. One DVD, with two operative videos, accompanies the book.

An illustrated guide to surgical procedures that can facilitate rehabilitation of selected patients with deformities due to poliomyelitis. Addressed to doctors providing surgical services in district hospitals the book concentrates on simple procedures that can significantly facilitate rehabilitation and improve the physical independence of polio victims. The importance of including physical therapy in the management of patients is repeatedly emphasized. The manual can be used for training purposes or as an aide mTmoire. The book covers both simple essential procedures that can be carried out by a general duty doctor with some experience in an average district hospital and those more difficult procedures that require more experience and better than average facilities. Most of the procedures covered are corrective and stabilizing procedures on the lower limbs which aim to enable patients to stand and move. The book features nine chapters presented in two parts. Chapters in the first part describe the disease and its sequelae provide detailed advice on the tests to be used when evaluating patients and selecting the appropriate procedures and explain a number of basic surgical techniques. Chapters in the second and most extensive part describe specific surgical procedures for the hip knee foot and ankle and elbow. Although some procedures involving bone are described the handbook stresses that these techniques require more training and experience.
Liposuction is the most commonly performed aesthetic surgery procedure in the world. Despite its popularity, the effectiveness of this procedure has been limited in fibrous areas of the body such as the back, flank, and breasts (in cases of gynecomastia). Ultrasound-assisted lipoplasty (UAL) permits the selective removal of adipose tissue using ultrasonic energy as an airbrush to sculpt fat with less trauma and enhanced artistry. It is the ideal complement to standard liposuction and is a means of optimizing results for a broad spectrum of patients. This technique has captured the imagination of aesthetic surgeons and their patients around the world. Some of the benefits of UAL include: Higher effectiveness in areas where fat is fibrous and difficult to remove by traditional liposuction and for large-volume liposuction Less surgeon fatigue, allowing the surgeon to focus on true sculpting in body contouring Less tissue trauma because less force is needed to push the cannula through tissue The authors have taken an anatomic approach to UAL, covering the special needs of each target area. This comprehensive book/DVD combination takes you through UAL step-by-step, including the basics of getting started and the setup of instrumentation and equipment. The large number of illustrations in the book and the high-quality surgical video demonstrations make it easy to follow the procedures closely. Special focus is given to safety and unusual situations. UAL is a great way to gain incremental income from your practice and to stay competitive. No aesthetic surgeon should be without this valuable book and DVD. |
