Main focus of the new book will be the description and discussion of rat and mouse models for organ transplantation. Various microsurgical techniques will be presented which allow transplantation of functional organs in syngeneic systems. In particular, the extremely difficult methods necessary for organ transplantation in mice will be presented and evaluated. Besides these practical aspects the book will also cover the theoretical sides of organ transplantation like the immunobiology of allotransplantation. Special emphasis will be given to the resurgent field of xenotransplantation. The results from xenograft models developed in the recent years using rats or mice will be reviewed and their impact on future human xenotransplantation will be discussed.

In contrast to the continuously increasing success in kidney, liver, heart, and pancreas transplantation, small-bowel transplantation has not shown simi larly impressive progress until recently. The few clinical attempts at small-intestinal transplantation in the late 1960s and early 1970s were unsuccessful. In spite of these initial failures, a few groups of surgeons continued to investigate the problems of small-bowel transplantation from the technical, functional, and immunologic point of view. Now, about 15 years later, conditions have changed. Total parenteral nutrition has made tremendous progress, thus maintaining patients with short-bowel syndrome who are potential recipients of small-bowel grafts in an acceptable physical condition. Immunologists and clinicians have furt hermore been able to expand our knowledge of basic immunologic reactions induced by transplanted organs. In addition, within the last 8 years, new immunosuppressive drugs and regimens have been introduced which have proved to be extremely effective. Taking all these aspects into account, we regarded it as an extremely worthwhile and effective undertaking to invite scientists from all over the world who are working in the field of small-bowel transplantation to participate in a symposium on this topic. This meeting, the first to deal exclusively with small-bowel transplantation, was held in Kiel, West Ger many, in October 1985. Its aims were twofold: 1. ) To'stimulate direct communication between basic scientists and clini cians 2."

Preclinical experimental transplantation research that is based on microsurgical models in rats fulfills two indispensable conditions for modern organ transplanta tion research: Almost all organ grafts can be performed on the rat with an amount of technical effort that is still justifiable. Thus transplantation models that are analogous to human organ transplantation can be developed, tested, and evaluated. This fulfills a necessary condition from the standpoint of surgery. With the species rat, we have a great variety of genetically different inbred strains. From the immunological point of view this is an indispensable prerequisite for the investigation of preclinical transplantation models that can be expected to produce controllable, reproducible results. In vivo experimental results can be supplemented by and correlated to in vitro tests. Lately these experimental results are being greatly expanded and more precisely defined by the application of immunohistological methods that have been established recently in Kiel. In this book we hope to present a cross section of the microsurgical models in use today and of current immunological and immunohistological models. Furthermore, we wish to record the present state of microsurgical organ transplantation research and to show its relationship to the current state and development of clinical organ transplantation. A special aspect of our Kiel research group is the long-term, well-functioning, interdisciplinary cooperation between surgery, immunology, and pathology. Through this cooperation we attempt to provide an atmosphere in which theoretical and practical viewpoints can mutually influence each other."

The anastomotic technique plays a predominant role in gastrointestinal surgery. A feared complication is leakage due to the sutures. Such leakage cannot be detected early; consequently, infection spreads locally and systemically. An anastomotic method is generally evaluated according to its rate of leakage, related to the localization, bur real scientific comparisons, i.e., controlled studies, are almost totally lack ing. The criteria of evaluation include the type of suture, the localization, the auxiliary technical tools, practicability, the different forms of wound healing, angiogenesis, and vascularization, among others. The postoperative criteria are complications shortly after surgery, such as bleeding, ruptures, and stenoses of the anastomosis. A standard comparison is made difficult by the variety of cytophysiological and biochemical factors that influence wound healing. In the comparison of larger series one must always take into account differences of auxiliary tools, strategies, and inhomogeneity of patients. A change of one auxiliary tool or of one strategy implies the modification of various target criteria. Often enough, however, one does not sufficiently consider the surgeon's most. important role."

Grundlage des vorliegenden Buches ist das am 11. und 12. 9. 1981 in Kiel abge- laufene internationale Symposium: "Standortbestimmung moderner Naht- materialien und Nahttechniken in der Chirurgie". Eine Standortbestimmung moderner Nahtmaterialien und Nahttechniken in der Chirurgie dient der Dar- stellung der neuentwickelten und der in der Praxis bewiihrten Nahtmaterialien. Da es sich primiir urn rein empirisch entstandene chirurgische Hilfsmittel handelt, die Folgeprodukte der Textilentwicklung sind, bedarf es zunehmend der wissen- schaftlichen Oberpriifung der Nahtmaterialien selbst, der durch die Nahtma- terialien induzierten Gewebereaktionen und der durch die Nahtmaterialien er- m6glichten Operationstechniken. Waren friiher Seide und Catgut die verfiig- baren Nahtmaterialien, so haben inzwischen eine Reihe von vollsynthetischen, nichtabsorbierbaren, u. a. Polyesterfiiden, und absorbierbaren, u. a. Polyglykol- siiurefiiden, die Rolle des chirurgischen Nahtmaterials weitgehend iibernommen. Durch die Fadenverarbeitung, monofil oder polyfil, lassen sich die physikalischen Eigenschaften manipulieren, wiihrend die Oberfliicheneigenschaft eines Fadens durch Oberfliichenbearbeitung, z. B. Beschichtung, variiert werden kann. Wer- den ausschlieBlich physikalische Parameter, wie lineare Zugfestigkeit, Knoten- sitz-, Knotenbruchfestigkeit, Elastizitiit, Kapillaritiit und Reibungskoeffizient der Fadenoberfliiche u. a., bestimmt, so k6nnen MeB- und Bewertungsverfahren der Textilindustrie iibernommen werden. Durch biochemische Veriinderungen der Fiiden nach Implantation ins Gewebe, insbesondere bei vollsynthetischen ab- sorbierbaren Faden, sind jedoch kompliziertere, weit weniger standardisierbare Dberpriifungsverfahren in vivo erforderlich, urn die gewebeabhiingige Fadenver- iinderung und die nahtbedingte Gewebereaktion zu erfassen.

In ubersichtlicher Weise werden Viszerosyntheseverfahren fur den oberen und unteren Gastrointestinaltrakt dargestellt. Fundierte experimentelle Grundlagen und die klinischen Anwendungen sind zum Teil als Studien dokumentiert. Die sich standig fortentwickelnde chirurgische Anastomosentechnik mit den typischen Verfahren der Hand-, Stapler- und Kompressionsanastomose wird erganzt durch die laparoskopischen Nahtverfahren im Gastrointestinaltrakt.

Der Einzug der minimalinvasiven Chirurgie hat in der Chirurgie in den letzten 3 Jahren zu einem Umbruch gef}hrt. Aber erst die Einf}hrung von speziell entwickelten Klammer- nahtinstrumenten f}r den laparoskopischen undthorakosko- pischen Einsatz hat zur Verbreitung dieser Techniken f}hren k-nnen. Dies ist eine aktuelle Zusammenstellung der M-glichkeiten und Indikationsgebiete f}r die Anwendung von Klammernaht- instrumenten in der konventionellen und minimalinvasiven Chirurgie. Das Buch behandelt schwerpunktm{ ig die klinische und praktische Anwendung der Klammernahtinstrumente, zeigt neue Perspektiven f}r die minimalinvasive Chirurgie durch die Anwendung von Klammernahtinstrumenten auf und gibt eine ]bersicht }ber Indikationen, Ergebnisse und Komplikationen.

Rektumamputation und Rektumresektion konkurrieren miteinander seit knapp 100 Jahren als Operationsverfahren beim Rektumkarzinom. In die Indikationsstellung fUr das eine oder andere Verfahren gehen vor allem Erkrankungslokalisationen, Typisierung, Grad- und Stadieneinteilung, die erforderlichen und erreichbaren Sicherheitszonen, anatomische Verhalt- nisse und der operationstechnische Standard und funktionelle Faktoren ein. J e gezielter die Abhangigkeit dieser Faktoren voneinander prazisiert werden kann, desto besser kann der Patient hinsichtlich Tumorradikalitat und postoperativer Folgen beraten und behandelt werden. In diesem lang anhaltenden Spannungs- und Diskussionsfeld hat die EinfUhrung zirkula- rer Nahinstrumente neue Entwicklungsrichtungen ermoglicht. Die Ent- wicklung der Nahinstrumente wurde Anfang dieses Jahrhunderts von Hultl begonnen und hat neben geraden auch zirkulare Stapler hervorge- bracht, die besonders fUr die Rektumchirurgie geeignet sind. Diese sind heute mit Einmalklammermagazinen oder als Einmalgerate verfUgbar. Als nachster Entwicklungsschritt folgt wahrscheinlich der Ersatz der Metall- klammern durch absorbierbare Klammermaterialien. Grundsatzlich wer- den im Dickdarmbereich folgende Ziele angestrebt: 1) hohe reproduzierbare Nahtsicherheit, 2) zeitliche VerkUrzung des Nahvorgangs und damit Herabsetzung der Gesamtoperationszeit, 3) Erweiterung der operativen Indikationen, 4) Herabsetzung der Offnungszeiten keimbesiedelter Hohlorgane durch maschinelle Anastomosen, 5) geringere Traumatisierung des anastomosierten Gewebes. Urn die Gefahren- und Fehlermoglichkeiten beim Einsatz der Nahappa- rate gering zu halten, bedarf es eines speziellen Trainingsprogramms vor dem klinischen Einsatz der Stapler. Urn die Wertigkeit solcher Nahtma- schinen zu Uberprlifen, wird in Kiel schrittweise vorgegangen. Der Ein- fUhrung der Nahinstrumente folgt eine prospektive Studie, die die Vor- aussetzungen fUr eine kontrollierte Studie schafft.
