Tumour therapy depends essentially on being able to destroy the clonogenic activity of tumour cells while keeping the damage to the normal tissue low. Clinical experience shows that tumour response varies greatly even if tumours with the same localisation, clinical, and histopathological staging are compared. Some tumours appear to be resistant to conventional radiotherapy (X-rays, y-rays or fast electrons) or chemotherapy. In these cases new therapy modalities are necessary. Combined therapy modalities seem to have advan- tages for some resistant tumours; one possibility of such a treatment is to combine radiotherapy or chemotherapy with hyperthermia. This means that the local tumour, the tumour region or even the whole body of the patient has to be heated to temperatures between 40 to 45 C (in case of whole body hyperthermia to 42 C maximal) for a certain time (usually 30-60 min are adequate). Hyperthermia has a long tradition in medicine as a treatment modality for various diseases. Inscriptions of the old Egyptians and texts of the Greeks have pointed out its importance. Usually whole body hyperthermia has been used by the induction of fever. Local hyperthermia began around 1900 when Westermark treated unre- sectable cervix carcinomas with hot water in a metallic coil. By the beginning of this century an increase of radiation effects was hy- pothesised with hypothermia and later observed. However, only in the 1960s and 1970s were systematic investigations started which showed radiosensitisation and chemosensitisation by hyperthermia in cells and tissues including tumours.

Applied genetic research, genetic toxicology and mutation research investigate the mutagenicity of chemicals and other agents. Permanent mutation in genes and chromosomes can be induced by a plethora of agents, including ionizing and nonionizing radiations, chemicals, and viruses. Among the aspects discussed in "Advances in Mutagenesis Research" are: 1. The understanding of the molecular mechanisms leading to mutations, and 2. the prevention of a thoughtless introduction of mutagenic agents into the environment.

The Europaische Akademie Bad Neuenahr-Ahrweiler GmbH is concerned with the scientific study ofthe consequences of scientific and technological advance for the individual, society and the natural environment and, therefore, not least with the study of consequences of recent developments in life-sciences and medical disci- plines. The Europaische Akademie intends to contribute to find a rational way for society to deal with the consequences of scientific progress. This aim is mainly realised by proposing recommendations for options of action with long-term social acceptance. The work of the Europaische Akademie mostly takes place in tempo- rary interdisciplinary project groups, whose members are recognised scientists from European universities and other independent institutes. In the light of recent discussions on the validity of scientific assumptions con- cerning the extrapolation ofharmful effects into the low-dose range the Europais- che Akademie set up a project on "Lew-Dose Exposures in the Environment their Risk Assessments and Regulatory Processes" in January 2001. Experts ofradiobi- ology, toxicology, medical epidemiology, modelling,jurisprudence, psychology and philosophy from different European countries wcre brought together to discuss cur- rent scientific developments in environmental standard setting and risk evaluation of substances and ionising radiation at very low doses in order to ensure rational, efficient, and fair decisions. I am glad that by means ofthis memorandum, that continues our work on envi- ronmental standards and after our recommendations with respect to the regulation ofcombined agents we can now focus on the effects and the regulation ofharmful agents in the very low-dose range.

The ever-increasing release of harmful agents due to human activities has led in some areas of the world to heavy pollution. In order to protect human health and the environment, environmental standards that shall limit the release and the concentration of those toxic agents in the environment and hence the exposure to it have to be established. The related assessment and decision-making procedures have to be based on solid scientific data about the effects and mechanisms of these agents as well as on ethical, social and economic aspects. For risk evaluation, the knowledge of the dose response curve is an essential prerequisite. Dose responses without a threshold dose are most critical in this connection. Such dose responses are assumed for mutagenic and carcinogenic effects, which, therefore, dominate also the discussion in this book. In the environmentally important low dose range, risk estimation can only be achieved by extrapolation from higher doses with measurable effects. The extrapolation is accompanied with uncertainties which makes risk evaluation as well as risk communication frequently problematic. In order to ensure rational efficient and fair decisions beyond a sound scientific assessment the dialogue between disciplines, with the affected people and with the general public is necessary. In this book, the whole range of relevant and essential aspects of risk evaluation and standard setting is addressed. Starting with the ethical foundations, the sound analysis of recent scientific findings sets the frame for further reflections by theory of cognition, psychosocial sciences, and jurisprudence. The authors end up with concluding recommendations for coping with the recentproblems of standard setting in the field of environmentally relevant low doses. The book is designed to a readership of scientists, legislators, administrators, and the interested public.

Im Rahmen dieser Studie werden aus naturwissenschaftlich-medizinischer, soziologischer, oekonomischer, juristischer und philosophischer Perspektive Notwendigkeit, Realisierbarkeit und Konsequenzen von Umweltstandards fur kombinierte Expositionen untersucht. Auf Expositionen des Menschen - dabei besonders deren karzinogene sowie genotoxische Wirkungen - sowie Expositionen einer Auswahl von Pflanzen, anhand derer die pragmatisch dringlichsten Fragen beantwortet werden, ist das Hauptaugenmerk der Studie gerichtet. Auf der Grundlage einer auf Wirkungsmechanismen basierenden Kategorisierung kombinierter Expositionen werden Kriterien erarbeitet, die trotz der Komplexitat der einzelnen Wirkungszusammenhange eine Grenzwertsetzung zur Erhaltung bzw. Erreichung konkreter Umweltqualtitatsziele ermoeglicht.

Der vorliegende Band gibt die Vortrage eines Symposi !Us wieder, das im Rahmen des gemeinsamen Kongresses der Deutschen und Osterreichischen Rontgen- gesellschaft imJahre 1973 in Wien durchgefuhrt wurde. Die Arb itsgemeinschaft fur Strahlenbiologie in der DRG setzte damit gemeinsam mit der Osterreichischen Rontgengesellschaft eine Serie von Veranstaltungen fort, die den Dialog zwischen dem klinisch tatigen Radiologen und dem theoretischen Radiologen, insbeson- dere dem Strahlenbiologen, verstarken sollen. Di beiden thematischen Schwer- punkte des Symposiums, lymphatisches System und kleine Dosen, konnen diesem Vorhaben in besonderem MaBe dienen. Nicht nur aufgrund der aktuellen Situation muB es ein Anliegen der Radiologen sein, die biologische Wirkung kleiner Strahlendosen zu erfassen und verstehen zu lernen. Einerseits kann man einer emotionalen Darstellung, wie sie in der Offentlichkeit hier und da versucht wird, nur mit harten Fakten wissenschaft- licher Erfahrungen und Dberlegungen gegenubertreten, andererseits ist die Ab- schlitzung des Risikos, das durch die Absorption ionisierender Strahlen einge- gangen wird, insbesondere in Hinsicht auf Spliteffekte, wie z. B. die cancerogene Wirkung, keineswegs abgeschlossen. Seit den Arbeiten von Heineke in den Jahren 1903-1905 nimmt die Strahlen- empfindlichkeit von Lymphocyten bei strahlenbiologischen Untersuchungen eine hervorragende Stellung ein, die auch fur die radiologische Klinik Bedeutung erlangt hat. So gehort die Bestimmung der Lymphocytenzahl im Blut auch heute noch zur routinemliBigen Dberwachung strahlentherapeutischer MaBnahmen.

Die strahlenbiologische Forschung ist beinahe ebenso alt wie die Erkenntnis, daB es radioaktive Isotope gibt, die ionisierende Strahlen aussenden. Derartige Untersuchungen entsprangen nicht, wie es manch mal scheinen mag, der Angst vor den Gefahren dieser Strahlung sondern vielmehr dem Trieb des Menschen, die Umwelt mit ihren Phiinomenen zu erfassen. Heute, in einer Zeit, die sich anschickt, mit der Kerntechnik zu leben, stellt sich die Frage nach den biologischen Wirkungen ionisierender Strahlen zwar mit einer besonderen Aktuali tat. Sie sollte jedoch vor allem aus der Grundhaltung wissenschaftlichen Suchens, das Wissen zu vermehren, gesehen werden. Zum Verstandnis des Mechanismus kommt den Untersuchungen dieser Fragestellung im molekularen Bereich besondere Bedeutung zu. In den letzten 10-15 J ahren ist eine groBe Zahl biochemischer Messungen an bestrahlten Organismen verofl"entlicht worden. Es er schien mir daher reizvoll, einen Versuch zu unternehmen, die experi mentellen Befunde zusammenfassend darzustellen und einzuordnen. Dabei wurde nicht verkannt, daB die Forschung auf dem Gebiet der Strahlen-Biochemie sehr stark im FluB ist und die Sicht der kausalen Zusammenhange bisher in vielen Fiillen nicht moglich ist, so daB viele Fragen unbeantwortet bleiben mtissen. Da die Parameter, wie Zeit faktoren, Bestrahlungsbedingungen usw., die die biologischen Strah lenefl"ekte beeinfiussen, sehr vielfiiltig sind, ergibt sich haufig eine sehr komplexe Situation. Es kann daher nicht der Anspruch erhoben wer den, daB die in der Literatur beschriebenen Daten in den folgenden Kapiteln erschopfend dargestellt sind."
