In recent years, the Republic of Guinea has shed its reputation as one of the most tightly controlled state economies in Africa, leaving behind a cloistered era marked by an extraordinarily closed economic and political system. In breaking with its dismal past, Guinea has launched an ambitious program of reform which has affected the entire range of the country's institutions, regulations, and markets.

Culling data from the World Bank, the International Monetary Fund, the United Nations, and numerous interviews and previously unpublished government data, Jehan Arulpragasam and David E. Sahn here present an overview of the Guinean economy, and its evolution--from independence, through crisis, to reform--and model implications of these changes for economic performance and living standards of the poor. Highlighting the chasm between theory and practice, between well-intentioned program and problematic implementation, the authors reveal how Guinea both parallels and contradicts past experiences of economic reform in Africa. Most notably, reform in Guinea has been hindered by the weighty administrative, managerial, and logistical demands of undertaking a vast battery of economic adjustments, all in one fell swoop.

The most detailed and informative study of the Guinean economy to date, "Economic Transition in Guinea" illustrates not only the successes of the nation's reform agenda, but also the fundamental constraints to development that often lie beyond the reach of such reform.

Has economic reform hurt the poor in Africa? There is little disagreement that most African countries faced an economic crisis in the 1980s, characterized by worsening budget and balance-of-payment deficits, stagnant growth, and slow improvement in general indicators. Far less consensus exists, however, on the appropriateness and effectiveness of the macroeconomic and sectoral reforms these countries undertook in response to these conditions. More contentious still is the subject of this book: whether the poor are hurt, in absolute and relative terms, by the economic policies designed to restore macroeconomic stability, reinvigorate markets, and rationalize resource allocation in Africa. Critics claim that, while orthodox adjustment policies may make sense at the macro level, they have high social costs. Proponents deny that living standards have declined as a result of adjustment policies, arguing that any declines are due to other factors. The contributors to this volume employ empirical methods to separate the effects of the economic crises that induced countries to begin to adjust from the impact of the economic reforms themselves. This approach is more sophisticated than the standard comparison of economic performance and household welfare before and after reform, which attributes all changes to the reform process. With these models, the authors examine the impact of specific policy reforms - under the broad headings of trade and exchange rate, fiscal, and food and agricultural sector policy - in specific countries. The countries covered are Cameroon, The Gambia, Ghana, Guinea, Madagascar, Malawi, Mozambique, Niger, Tanzania, and Zaire.

Advances in science and policy during the past 50 years have prevented the predicted widespread food shortages as the world's population soared. Malnutrition, however, remains prevalent. This book details strategies and practical approaches designed to alleviate hunger and malnutrition in a new era where technological change, markets, patterns of governance, and social programs have an increasingly global dimension. More specifically, this book addresses a range of considerations including the role of small farmers in a world where the global reach of multinational corporations have enormous control from the farm to local markets and the grocery store; misgivings and misperceptions about genetically modified foods; the increasing competition of food and energy sectors for agricultural output; the importance of micronutrient deficiencies and chronic disease related to obesity, which often coexists in the same communities as hunger; and issues of sustainability of the food and agricultural system in an period when there is increasing concerns over global warming and environmental degradation. Currently there is also more emphasis on evidence-based policymaking, which has raised the standard of proof for evaluating the impact of micro-level interventions that have traditionally been so widely embraced and are now under increased scrutiny. It is in this context that this book provides practical advice on programs that can effectively target those at greatest risk of malnutrition and guidance on policies to promote a healthy and sustainable food and agricultural system. Overlaying all of these challenges is the book's emphasis on both identifying data and information needs for decision-making, and practical considerations for better understanding the domestic and international political and social constraints that need to be addressed when trying to translate scientific knowledge and information into practice.

Since the 1980s HIV/AIDS has occupied a singular position because of the rapidly emergent threat and devastation the disease has caused, particularly in sub-Saharan Africa. New infections continue to create a formidable challenge to households, communities, and health systems: last year alone, 2.7 million new infections occurred globally. Sub-Saharan Africa remains the epicenter of the suffering, with around two-thirds of infected individuals worldwide found there, and a disproportionate number of deaths and new infections.

For years there have been widespread and concerted efforts to prevent the spread of HIV/AIDS, identify a cure, and understand and mitigate the deleterious social and economic ramifications of the disease. Despite these efforts, and some apparent successes, there is still a long way to go in terms of altering behaviors in order to realize the objective of dramatic reductions in the spread of HIV/AIDS in Africa. The authors in this volume examine the HIV/AIDS crisis in Africa, which persists despite major strides in averting deaths due to antiretroviral therapy. They tell an important story of the distinct nature of the disease and its socioeconomic implications.

The often emotional debate over the impact of structural adjustment on the poor in Africa has been confused by the complexity of economic reforms and their inconsistent implementation, the diversity of prior conditions, and confounding effects of external shocks. Professors Sahn, Dorosh, and Younger isolate from other factors the effect of specific policy measures associated with adjustment programs. The authors suggest that contrary to common belief, adjustment policies do not harm the poor in Africa. Reforms in fact usually benefit the poor slightly, but alone are insufficient to reduce poverty significantly.
