This book is the proceedings of the Falk Symposium No. 135 held in
Prague, Czech Republic, on September 12-13, 2003, and is dedicated
to the important issue of immunological aspects of diseases of the
liver and gut. Without any doubt, immunological pathways are among
the most important and universal factors in the pathogenesis of all
diseases. Their importance is also constantly increasing, because
these principles have been adopted in clinical practice for both
diagnostic and therapeutic procedures. Chapters by prominent
experts will stimulate new ideas and set the scene for productive
discussion on this topic.
In spite of significant scientific progress in recent years, the
aetiopathogenesis of chronic inflammatory bowel diseases (IBD)
remains elusive. With a prevalence of 1: 700 to 1: 500 in central
Europe, both diseases are not rare. However, most physicians will
generally treat only a few patients with IBD in their clinical
practice. Because of this limited experience and the variety of
clinical IBD manifestations, IBD patients are frequently treated in
specialized tertiary care centres. Nevertheless, IBD patients will
also consult less specialized general practitioners, who will not
be able to focus extensively on the management of IBD patients due
to time constraints.
Physicians who take care of IBD patients have to select among an
ever-increasing amount of new scientific information in order to
provide optimal and up-to-date care to their patients. In the era
of evidence-based medicine it is often under debate whether
evidence-based medicine may really improve the management of IBD
patients or whether the diversity and complexity of IBD needs an
individual and patient-oriented approach.
This book, the proceedings of the Falk Symposium 134 on Trends and
Controversies in IBD - Evidence-Based Approach or Individual
Management', held in Berlin, Germany, on 12-13 June 2003,
critically discusses established and emerging new concepts in the
field of IBD. It critically analyzes evidence-based and individual
approaches in the management of IBD and discusses these approaches
with the help of selected case reports.
The aetiology of the chronic inflammatory bowel diseases - Crohn's
disease and ulcerative colitis - is still enigmatic. The
therapeutic approach has therefore traditionally focused on
anti-inflammatory principles, including corticosteroids and
aminosalicylates. Since a significant proportion of patients is
steroid-dependent or refractory and because of the problematic
side-effects of long-term systemic steroids, active
immunosuppression has gained acceptance in the field. The classical
immunosuppressants azathioprine and 6-mercaptopurine have long been
evidence-based in IBD, but underused. Recently, methotrexate and
cyclosporine have also been proven to be effective in certain
situations. Newer drugs like tacrolimus, mycophenolate and others
may be similarly useful but their potential is still unclear.
Finally, the immune modifiers including IL-10 and TNF-antibodies
have been successfully subjected to controlled trials. Other
experimental drugs discussed in this book are on the horizon. This
volume is the Proceedings of Falk Symposium 119 held in
Freiburg-im-Breisgau, Germany, October 3-4, 2000, and covers
systematically the field of immunosuppression in inflammatory bowel
diseases. It is hoped that it will further the prudent use of these
drugs in the proper clinical situations by increasing our
understanding of both the mechanisms of action and the clinical
benefit to the patient.
Chronic inflammatory bowel disease still represents a major
gastroenterological challenge, in view of the unclear aetiology and
unsatisfactory therapy. This book, the proceedings of a Falk

on April 30 - May 2, 1998, brought together experts from different
fields of science to allow a unified view on clinical and basic
research. The main themes of the meeting were genetics, animal
models, immunology, epithelial cells, endotoxin, diagnostic
procedures, malignancy, medical therapies, and surgery. In each
section of the book experts present state-of-the-art reviews with
the very latest developments in the respective research areas
followed by chapters on different aspects of inflammatory bowel
diseases. They provide suggestions concerning future research and
clinical management of patients with inflammatory bowel diseases.
Significant scientific progress has been made in recent years in
the aetiopathogenesis of chronic inflammatory bowel diseases (IBD),
that has changed or will change in the future the diagnostic and
therapeutic approach in these diseases. The relevance of classical
conventional IBD therapies has been critically re-evaluated under
criteria of evidence-based medicine approaches. On the other hand,
new therapeutic options have emerged. These include modern
immunomodulators, biologics, probiotics, worm eggs and
extracorporeal therapies. The clinical relevance of these new
modalities is often difficult to assess for physicians who do not
work frequently with IBD patients.
This book, the proceedings of the Falk Syposium No. 139 on
"Gallstones: Pathogenesis and Treatment", held in Freiburg,
Germany, on January 15-16, 2004, reflects the tremendous advances
in the field of biliary diseases both at the genetic and cellular
as well as the clinical levels. In industrialized countries,
gallstones are a frequent clinical entity with a prevalence of
10-15%. In clinical practice one can differentiate between
cholesterol stones, black pigment stones and brown pigment stones.
80-90% of gallbladder stones are cholesterol stones, 10--20% are
black pigment stones. The prevalence of cholesterol stones
increases with age and in Europe in the 70+ age group reaches about
20% in males and about 30% in females. Gallstones and gallstone
diseases, therefore, are a frequent and important clinical problem.
The book addresses the basic aspects of gallstone pathogenesis as
well as the non-surgical and surgical treatment of gallstone
diseases. Of particular interest are the epidemiology and genetics
of gallstone formation, with definition of risk factors, in
particular of genetic predispositions. Other major aspects are the
hepatocellular and bilary transport mechanisms that predispose to
the formation of gallstones. Apart from cholesterol and lipid
transport and absorption, gallbladder function plays a central role
in gallstone formation. Another clinically very relevant topic is
the treatment of gallstone diseases by drugs, endoscopic and other
non-surgical interventions, and surgery, as well as the long-term
outcome of therapeutic interventions. The volume concludes with a
state-of-the-art chapter on "Gallstone Research in the Post-Genomic
Era". Gallstones: Pathogenesis and Treatment is an excellent update
on the pathogenesis of gallstones as well as on the treatment of
gallstone diseases, written by an international faculty of
outstanding clinicans and scientists. The book will, therefore, be
of great interest to hepatologists and gastroenterologists in
academic institutions as well as in clinical practice.
Der Schwerpunkt der Vortrage und Diskussionen der zweiten
Expertenrunde "Aktuelle Gastroenterologie" lag diesmal in der
differentialdiagnostischen Abgrenzung chronisch entzundlicher
Darmerkrankungen gegenuber intestinalen Infektionen. Neben der
Diskussion der wichtigsten differentialdiagnostisch relevanten
intestinalen Infektionen war es ein Hauptanliegen der
Organisatoren, differentialdiagnostische Strategien zur
Unterscheidung zwischen infektioser und chronisch entzundlicher
Darmerkrankung kritisch zu erortern.
