It is at least a decade since scientists turned their imaginations to creating new compact, portable test instruments and self-contained test kits that could be used to analyze urine and saliva for alcohol, drugs, and their metabolites. Although the potential applications for such tests at the site of specimen collection, now called "on-site" or "point-of-care" testing, range far beyond hospital emergency rooms and law enforcement needs, it was catalyzed by the requirements of workplace drug testing and other drugs-of-abuse testing programs. These programs are now a minor national industry in the United States and in some western European countries, and cover populations as diverse as the military, incarcerated criminals, people suspected of driving under the influence of alcohol and other drugs, all athletes from college to professional ranks, and of course the general employed population, which is monitored for illegal drug use and numbers in the millions. It is not surprising, then, that the need for rapid and precise tests, conducted economically by trained professionals, has become a major goal. Current government approved and peer reviewed laboratory methods for urine analysis serve present needs very well and have become remarkably robust over the past twenty years, but the logistics of testing some moving populations, such as the military, the Coast Guard, workers on off-shore oil platforms, and athletes-perhaps the most mobile of these groups-are unacceptably cumbersome.

Bile acids are increasingly being seen as extremely important carcinogenic agents in cancers of the bile duct, liver, colon, rectum, and oesophagus. They are essential agents involved in lipid digestion and absorption in mammals, however, they also play wide-ranging roles in a variety of disease states ranging from diabetes to cancer. They have evolved exquisite mechanisms for controlling their own synthesis and to ensure that they are produced at correct concentrations and also kept in the correct anatomical environment. It is only when these fine levels of controls are breached that Bile acids become associated with disease. This breaching of control mechanisms can occur through dietary means (e.g. in obesity) whereby excessive levels of Bile acids are produced and converted (via the bacterial flora) to damaging secondary Bile acids. Furthermore, lack of re-absorption of Bile acids can lead to liver pathologies. The atypical movement of Bile acids into the oesophagus, stimulated by episodes of reflux, is linked to oesophageal cancer. In recent years there have been tremendous advances in the understanding of the mechanisms behind the toxicity and bioactivity of different Bile acids and these are covered at length in this book. Prior to the publication of this book there was no single source of information on the toxicology and bioactivity properties of Bile acids. The book also uniquely collects all the relevant information together regarding the role of Bile acids in human disease and the mechanisms underlying Bile acid induced pathology. Additionally, as Bile acids are synthesised from cholesterol, there is wide recognition of the role for Bile acids in obesity linked diseases and this is also covered in this new publication. There is arguably no more famous book about the arts of interpretation and analysis than Sigmund Freud's 1899 Interpretation of Dreams. Stanley Milgram is one of the most influential and widely-cited social psychologists of the twentieth century. Recognized as perhaps the most creative figure in his field, he is famous for crafting social-psychological experiments with an almost artistic sense of creative imagination – casting new light on social phenomena in the process. His 1974 study Obedience to Authority exemplifies creative thinking at its most potent, and controversial. Interested in the degree to which an “authority figure” could encourage people to commit acts against their sense of right and wrong, Milgram tricked volunteers for a “learning experiment” into believing that they were inflicting painful electric shocks on a person in another room. Able to hear convincing sounds of pain and pleas to stop, the volunteers were told by an authority figure – the “scientist” – that they should continue regardless. Contrary to his own predictions, Milgram discovered that, depending on the exact set up, as many as 65% of people would continue right up to the point of “killing” the victim.

The experiment showed, he believed, that ordinary people can, and will, do terrible things under the right circumstances, simply through obedience. As infamous and controversial as it was creatively inspired, the “Milgram experiment” shows just how radically creative thinking can shake our most fundamental assumptions.

Dr W J Jenkins In 1977 when the Sheffield Transfusion Centre took delivery of the first GROUPAMATIC blood grouping machine in the UK it was equipped with a sample identification system involving complicated and expensive disposable punched cards. In fact, the cards were so expensive that Dr Wagstaff was unable to find the revenue to support the system. A year later, when Brentwood took delivery of a GROUPAMATIC, we were faced with the same problem, but by chance we heard that KONTRON was developing a laser scanning system for bar code labels and we were able to have our machine modified. Subsequently the Sheffield machine was altered to take the bar code scanner. At about the same time the Bristol Centre was helping TECHNICON with the development of the AUTO GROUPER C-16, and fortunately they decided on a laser reader of the same type for bar code identification. Thus there were three centres with the capability for reading bar codes on blood grouping machines and it became necessary to find someone to produce the bar code labels. There was only on~ printer in the UK who could produce labels to the required specification. To cut the costs of printing, and in the hope of avoiding a wide variation in codes, I invited representatives of centres interested in the problem to a meeting, where we set up what we called the Group of Six. This later became an official Working Party of the Regional Transfusion Directors.
