Covering both the scientific basis of rheumatology and practical, clinical information for rheumatologists and trainees, Rheumatology, 8th Edition, remains a leading text in this fast-changing field. Dr. Marc Hochberg and his team of worldwide editors and authors keep you abreast of recent advances in the field- all in a user-friendly, accessible manner. Fully updated from cover to cover, this two-volume text is designed to meet the needs of all practicing and academic rheumatologists as well as arthritis-related health care professionals and scientists interested in rheumatic and musculoskeletal diseases. Covers the epidemiology, pathogenesis, clinical manifestations, therapeutic approach, and management of all major as well as rarely encountered rheumatic and musculoskeletal diseases. Discusses clinical examination, imaging principles, differential diagnosis, established and novel therapies, perioperative evaluation, pain management, basic science, and genetics of rheumatic and musculoskeletal diseases. Uses a consistent, logical, reader-friendly format with templated chapters, concise text, and large-scale, state-of-the-art illustrations for efficient visual reference. Contains new chapters covering pre-clinical disease and how to address these patients, common comorbidities in rheumatoid arthritis; emerging therapies for systemic sclerosis; immune mediated complications of checkpoint inhibitors; the epidemiology of COVID-19 and rheumatic and musculoskeletal diseases, emerging treatments for osteoarthritis, and big data analytics. Provides updates to key topics such as systems biology and its impact on our understanding of the pathogenesis of rheumatic and musculoskeletal diseases, the microbiome in rheumatic musculoskeletal diseases, how to manage chronic pain in the patient with a rheumatic disease, drugs and reproductive health, and emerging therapies for patients with RA, SLE, spondyloarthritis, inflammatory muscle disease, and vasculitis. Shares the knowledge and expertise of numerous new contributing authors, as well as new co-editor Dr. Desiree van der Heijde, who is an expert in psoriatic arthritis, spondyloarthritis, imaging, and clinical epidemiology. Provides access to concise videos depicting the use of ultrasound for diagnosis and treatment. Enhanced eBook version included with purchase. Many advances have occurred in the last 5 years especially in understanding genetic susceptibility factors, the role of immune response in chronic arthritis and the relationship between cellular processes that underlie inflammation and tissue damage. All these are dealt with in depth in the individual contributions. Genetics studies included illustrate how studies of HLA genes and genes controlling the autoantibody repertoire may be involved. Environmental factors are believed to be important and studies both in humans and in experimental models on the role of viral and bacterial agents are covered. The fascinating relationship between microbes, dysregulated immune responses and oncogenes are also examined. Since autoimmunity is the key aspect of the immunological response in rheumatoid arthritis, the role of antigens is re-examined as is the part played by immune T and B cells. New areas of clinical application of basic science is well presented in this book, forexample, in examining how markers of cartilage destruction may become important in monitoring disease activity. A chapter reviewing possible future novel therapies is included.

More than 140 years ago, lupus erythematosus (LE) was recognized as a disease entity by clinicians working in the field of dermatology, which had only recently become an independent medical discipline. Soon after cutaneous lupus was first reported, it was realized that, apart from the skin, the disease could involve other organs and thus be systemic in nature. The latter observations were first made by MORITZ KApOSI [1], whose work has attracted renewed attention re cently and who succeeded FERDINAND VON HEBRA to the chair of dermatology at the Medical Faculty in Vienna. The early description of lupus erythematosus in both its cutaneous and systemic manifes tations was thus intimately associated with Vienna and its Medical School. The next phase in the study of lupus was characterized by an in crease in knowledge of the type and extent of organ involvement. The work by OSLER [2], LIBMANN and SACKS [3], and KLEMPERER [4] best represents these advances. The increase in clinical knowledge of LE finally led to DUBOIS' famous monograph [5], which was pub lished at a time of renewed interest in SLE, elicited by the descrip tion by HARGRAVES et al. [6] of the LE-cell phenomenon. A more detailed analysis of this finding revealed that the disease was charac terized by an abnormal immune response, although its pathogenetic implications were still unclear.
