This book examines the concept of "community", focusing on how communication practices help manage the tensions of creating and sustaining everyday communal life amidst the crisis of human loss. While acknowledging how the contradictory and inconsistent nature of human relationships inevitably affects community, this intimate and compelling text shows how community is created and sustained in concrete communication practices.

The authors explore these ideas at Bonaventure House, an award-winning residential facility for people with AIDS, where the web of social relationships and the demands of a life-threatening illness intersect in complex ways. Facing a life-threatening illness can defy meaningful social connections, but it can also inspire such ties, sometimes in ways that elude us in the course of daily life. By understanding how collective communication practices help residents forge a sense of community out of the fragility and chaos f living together with AIDS, we are able to better understand how communication is inexorably intertwined with the formation of community in other environments.

Based on seven years of ethnographic research including participant-observation, in-depth interviews, and questionnaires, thisbook weaves together narratives and visual images with conceptual analysis to uncover the ongoing oppositional forces of community life, and to show how both mundane and profound communication processes ameliorate these tensions, and thereby sustain this fragile community. Because the average length of stay for a resident is seven months -- in which time he or she moves from being a newcomer to a community member to someone the community remembers -- the text reflects this short, but crystallized life, starting with the day a new resident opens the door to the day he or she passes away.

The writing is very rich -- intimate, engaging, personal, compelling, and vivid. Bauhofer, Geneva In disaster situations, the particular concern of WHO is not so much to offer immediate relief and assistance for affected communities, but to have ready prepared plans for the provision of primary medical care as well as for resuscitation and casualty services. Disast ers are characterized by a need for rapid assistance and by the inability of affected communities to cope with the large scale mortality, morbidity, and damage to essential installations and homes. In some highly elaborate and centralized societies even small scale events may assume the proportion of a disaster, if they result in the serious break down of vital services. The Executive Board of the World Health Organization has defmed disasters or "emergencies," as situations where there are unforeseen, serious, and immediate threats to public health. Particularly severe disasters may be classified as catastrophes; such -occurrences, whether natural or man-made, disturb or overthrow the existing order. For planning purposes, it is important to distinguish between different types of catastrophes since they require special relief measures. In the past, medical assistance was primarily needed in epidemics of, for example, plague, cholera, and smallpox. Today, health authorities face emergency problems brought about by major accidents and outbreaks of chemical pollution and poisoning, which may have long-term effects. The role of health services may differ quite extenSively in different types of cata strophes, and an attempt must be made to draw up specific plans to deal with them."

Es ist zu erwarten, daB diese WUnsche des Klinikers am besten von Substanzen erfUllt werden, die aus korpereigenen Stoffen abgeleitet sind oder diesen sogar entsprechen. Ein interessantes parenteral wirkendes Anaesthetikum ist die aus der y-Aminobuttersaure im Zentralnervensystem entstehende y-Hy- droxibuttersaure. Das physiologische Vorkommen dieses Substrates hat E. ROBERTS beschrieben (1956), der UmwandlungsprozeB wurde von R. ALBERS und R. SALVATOR aufgeklart. Das Natriumsalz der y-Hydroxibuttersaure wirkt bei intravenoser Injektion schlaf- erzeugend, es verursacht keine Ubelkeit, keine Atemdepression und keine organschadigenden Nebenwirkungen. Wenn beispielsweise die per infusionem applizierte Dosis die kinetischen Eigenschaf- ten des Wirkstoffes berUcksichtigt, dann laBt sich die Schlaf- dauer gut vorprogrammieren. Bei der einmaligen Bolusinjektion hingegen weisen die Schlafzeiten eine gewisse Variationsbreite auf. Die Patienten erwachen, ohne daB unangenehme Traume oder unerwUnschte vegetative Nebenwirkungen bestehen: sie konnen bald wieder aufstehen, bald wieder essen und trinken. Von die- sen Eigenschaften habe ich mich im Selbstversuch wiederholt liberzeugen konnen. Auch die Gefahr der Ausbildung einer Sucht besteht nicht. Es ist ein Verdienst der Kohler Chemie, das besonders in Frank- reich populare Mittel, das dort in erster Linie von Frau LABORIT klinisch erforscht und empfohlen worden ist, auch in Deutschland auf den Markt gebracht zu haben.

Emergency care is improving throughout the world and thousands of lives are being saved each year. However, there are still too many patients who die before help reaches them in the form of advanced rescue and definitive emergency care techniques. In an effort to improve emergency care throughout the world, the International Committee of Emergency and Disaster Medicine meets bi annually. This meeting takes place in Mainz, Germany in September in order to discuss issues, exchange information, and establish re commendations designed to improve emergency care. The group is in dependent of political, national, racial, religious, or commercial influences and, in this sense, similar to the Club of Rome which attempts advances in sociology, biology and natural sciences. There fore, the organization could be called the "Club of Mainz" for the field of emergency and disaster medicine. The following book contains the proceedings of the International Symposiom on "Mobile Intensive Care Units and Advanced Emergency Care Units" at Mainz from September 24-27, 1973, and the discussions evoked by the first activities of the "Club of Mainz" and the inter nationally acknowledged "Recommendations." At this time we also announce the next meeting and International Symposium on Disaster Medicine, open to the public, organized by the "Club of Mainz" (October 1 - 2, 1977)."

No Straight Path tells the stories of ten successful female historians who came of age in an era when it was unusual for women to pursue careers in academia, especially in the field of history. These first-person accounts illuminate the experiences women of the post- World War II generation encountered when they chose to enter this male-dominated professional world. None of the contributors took a straight path into the profession; most first opted instead for the more conventional pursuits of college, public-school teaching, marriage, and motherhood. Despite these commonalities, their stories are individually unique: one rose from poverty in Arkansas to attend graduate school at Rutgers before earning the chairmanship of the history department at the University of Memphis; another pursued an archaeology degree, studied social work, and served as a college administrator before becoming a history professor at Tulane University; a third was a lobbyist who attended seminary, then taught high school, entered the history graduate program at Indiana University, and helped develop two honors colleges before entering academia; and yet another grew up in segregated Memphis and then worked in public schools in New Jersey before earning a graduate degree in history at the University of Memphis, where she now teaches. The experiences of the other historians featured in this collection are equally varied and distinctive. Several themes emerge in their collective stories. Most assumed they would become teachers, nurses, secretaries, or society ladies- the only ""respectable"" choices available to women at the time. The obligations of marriage and family, they believed, would far outweigh their careers outside the home. Upon making the unusual decision, at the time, to move beyond high-school teaching and attend graduate school, few grasped the extent to which men dominated the field of history or that they would be perceived by many as little more than objects of sexual desire. The work/home balance proved problematic for them throughout their careers, as they struggled to combine the needs and demands of their families with the expectations of the profession. These women had no road maps to follow. The giants who preceded them- Gerda Lerner, Anne Firor Scott, Linda K. Kerber, Joan Wallach Scott, A. Elizabeth Taylor, and others- had breached the gates but only with great drive and determination. Few of the contributors to No Straight Path expected to undertake such heroics or to rise to that level of accomplishment. They may have had modest expectations when entering the field, but with the help of female scholars past and present, they kept climbing and reached a level of success within the profession that holds great promise for the women who follow.

Die Erfolge der modern en operativen Medizin sind nicht nur der Verbesserung der anaesthesiologischen und chirurgischen Technik sondern auch der gezielten pra-intra";'und postoperativen Infu- sionstherapie zu verdanken. In den letzten Jahren hat sich klar erwiesen, daB Volurnenersatz durch kolloidale Plasmae-rsatzmittel in weiten Grenzen moglich ist und aufgrund der Verbesserung der FlieBeigenschaften des Blutes gegenUber Vollbluttransfusionen vorteilhaft sein kann. Extreme Blutverluste ausgenommen, sollte daher der primare Vo- lurnenersatz nicht mehr durch die_ risikobelastete Transfusion von Fremdblut erfolgen. Wahrend in den Vereinigten Staaten von Amerika heute den natUr- lichen Kolloidlosungen (Plasmaproteinlosung/PPL, Hurnanalburnin) weitgehend der Vorzug gegeben wird, werden in Europa fUr die Volurnenersatztherapie hauptsachlich kUnstliche Kolloide (Dextran, Gelatine und Starke) angewandt. PPL und Hurnanalbumin werden aus menschlichem Plasma- und Placen- tagewebe gewonnen. Urn den stetigen Bedarf an Plasma deck en zu konnen, wurde die Plasmapherese entwickelt, wobei Erythrozyten und Plasma durch Zentrifugation getrennt und erstere dem Spender retransfundiert werden. Da die wiederholte Plasmapherese erheb- liche Gesundheitsschaden zur Folge haben kann, wandte sich die World Health Organisation 1975 in einer Resolution gegen die kommerzielle Plasmapherese zur Gewinnung und zurn Export von Plasma und Plasmaderivaten. Im Februar 1977 sah sich auch die Bundesarztekammer veranlaBt, Richtlinien zur DurchfUhrung der Plasmapherese zu erlassen. PPL und Hurnanalburnin werden daher in Zukunft nur dann in groBerem Umfang als bisher zur VerfUgung stehen, wenn die Indikationsstellung zur Transfusion von Voll- blut starker zur Anwendung von Blutbestandteilen, d. h. im Sinne der gezielten Hamotherapie (Stich) geandert werden wird. PPLund Hurnanalburnin mUssen aufgrund ihrer l.

Noch vor 30 Jahren war die Infusionstherapie mit einer hohen Ne- benwirkungsquote belastet. Die Haufigkeit z.B. der pyrogenen Reaktionen in Form von Schuttelfroesten betrug manchenorts ca. 20 %, selbst bei Gabe einfacher physiologischer Kochsalzloesungen. Die Einfuhrung der Ringerloesung und der Traubenzuckerloesungen, sowie der Gebrauch von Einmalsystemen fur die Infusion brachten erhebliche Erleichterungen. Der Durchbruch erfolgte indes erst, als seit, etwa 10 Jahren alle Bestandteile der Nahrung nicht nur enteral, sondern auch paren- teral zugefuhrt werden konnten. Doch auch hierbei bestanden an- fangs "Kinderkrankheiten", besonders durch schlechte Vertraglich- keit und Nebenwirkungen. Erst heute kann man mit gutem Gewissen sagen, dass Probleme der parenteralen Ernahrung soweit geloest sind, dass diese Methode routinemassig in jedem Krankenhaus angewendet werden kann, so dass die vollwertige Ernahrung mit allen essentiel- len Aminosauren, mit verschiedenen Fettemulsionen kleinster Troepfchengroesse und mit verschiedenen Arten von KOhlenhydraten, die sich gegenseitig erganzen, selbst monatelang in ausreichen- der Calorienzahl zugefuhrt werden kann. Hierdurch sind zahlreiche Menschenleben heute zu retten, die fruher verloren gewesen waren. Einen dramatischen Erfolg dieser Art konnte ich z.B. bei der wochenlangen totalen parenteralen Ernahrung hungerstreikender Anarchisten erzielen, von denen kein einziger starb.

Viele Arzte hofften, daB mit der Reindarstellung des Morphins und der EinfUhrung des Athers und des Chloroforms im neunzehnten Jahrhundert, dem "Jahrhundert der Chirurgen," das Schmerzpro blem gel6st sei. Heute wissen wir, daB wir erst am Anfang der L6sung stehen und trotz wunderbarer Fortschritte in der Herstellung syn thetischer Analgetica, Lokalanaesthetica und Narkotica tausende von bisher unheilbaren Schmerzpatienten auch heute noch von Arzt zu Arzt und von Klinik zu Klinik wandern, ohne das ihnen wirkliche dauerhafte Hilfe zuteil wird. Nur wenn wir dieses qualvolle Leiden der Armsten der Armen in den Griff bekommen, wird das zwanzigste Jahrhundert als "Jahrhundert der Anaesthesisten" in die Geschichte der Medizin eingehen. Der kindliche Organismus ist nach der Geburt einem ReifungsprozeB unterworfen, der nur schrittweise vollzogen wird. Die Kompensationsfiihigkeit des kindlichen Organismus ist zuerst noch sehr beschrankt. Sowohl die Kreislauffunktion als die Atemfunktion sind nicht voll entwickelt. Metabolische Besonderheiten sind dafur ver- antwortlich, daB diese unreifen Funktionen oft noch groBeren Belastungen ausgesetzt werden, als die voll funktionstuchtigen Kompensationsmecha- nismen des Erwachsenen. So kommt es bei besonderen Belastungen in den ersten Lebensmonaten und -jahren sehr schnell zu lebensbedrohlichen St6rungen im Stoffwechsel, insbesondere im Wasser-, Elektrolyt-und Saure-Basen-Haushalt. Die diagnostische und therapeutische Bedeutung dieser besonderen Reaktionsart des kindlichen Organismus ist immer wieder Gegenstand der Diskussion zwischen Theoretikern und Klinikern. Der intravenoesen Ernahrung kommt bei der Behandlung schwerst- kranker Patienten im Rahmen der Intensivtherapie eine besondere Bedeu- tung zu. Die Stoffwechselfolgen von Nahrungskarenz, Traumen und Schock verursachen tiefgreifende Veranderungen im Zellstoffwechsel. Die Themen- stellung des letztjahrigen Mainzer Symposions erfasste in erster Linie die grundlegenden theoretischen Kenntnisse uber das Stoffwechselgeschehen bei Nahrungskarenz. Das Bild wurde durch Beitrage klinisch-experimenteller Untersuchungen uber die metabolischen Auswirkungen von Traumen und die Moeglichkeiten der Beeinflussung des posttraumatischen Katabolismus mit Hilfe der Infusionsloesungen abgerundet. Wir sind der Johannes Gutenberg-Universitat und der Jacques Pfrimmer- Gedachtnis-Stiftung zu Dank verpflichtet, dass sie die sehr fruchtbare Diskussion mit in-und auslandischen Forschern ermoeglichten. Die Veroeffentlichung der Vortrage und Diskussionen in diesem Band moege bei der Behandlung von schwerstkranken Patienten eine stete Hilfe sein. Daruber hinaus moegen sie zu weiteren wissenschaftlichen Anstrengun- gen zur Klarung noch ungeloester Probleme anregen. Die nasotracheale Intubation war friiher - vor der Ara der Muskelre- laxantien - weit verbreitet, weil die Einfiihrung des Tubus in flacherer Narkose gelang als auf oralem Wege, der eine gute Erschlaffung der Kiefer- muskulatur voraussetzt. Als die Muskelrelaxantien und insbesondere das Succinylcholin Ein- gang in die Klinik fanden, wurde die Intubation auf oralem Wege so ein- fach, daB manche Anaesthesisten glaubten, auf den nasalen Weg ganz ver- zichten zu konnen. Inzwischen hat es sich indes gezeigt, daB dieser Weg in vielen Fiillen V orteUe bringt, ja gelegentlich unumganglich sein kann. Die nasotrachealen Methoden haben heute wieder einen festen Platz in der Intubationstechnik. Die Literatur iiber die nasotracheale Intubation ist verhaltnismaBig sparlich. Es ist daher zu begriiBen, daB der Verfasser, der groBe, eigene Erfahrung mit dieser Technik besitzt, hier eine zusammenfassende Dar- stellung dieses Gebietes gibt. Diese erste Monographie iiber die nasotracheale Intubation will nicht nur die Anwendungsmoglichkeiten bringen, V or-und Nachteile schildern und die nasalen Methoden in den Rahmen der gesamten Intubationstechnik stellen, sondern es werden auch detaillierte Anleitungen zur praktischen Anwendung gegeben. Diese Anleitung wird besonders von denjenigen Kollegen begriiBt werden, welche diese Techniken zwar in ihrer Ausbil- dung gelernt, sie jedoch nur in besonderen Fallen einmal anzuwenden haben. Dariiber hinaus ist zu wiinschen, daB jeder Fachkollege und jeder Assistent diesen Band lesen wird, der iiber ein Gebiet informiert, das der Anaesthesist beherrschen soUte.

Die Messung des zentralen Venendrucks bei Patienten im Schock hat in letzter Zeit immer groBere diagnostische Bedeutung gefunden. Wahrend die klinischen Schockzeichen (wie BlaBe, cyanotische Haut, kalter SchweiB, niedriger Blutdruck und erhohte Pulszahl, sowie die Ein- schrankung der stundlichen Urinausscheidung vorwiegend den Funktions- zustand des Hochdrucksystems widerspiegeln, zeigt der zentral-venose Druck den Funktionszustand der KapazitatsgefaBe des "Niederdruck- systems" an und erlaubt Ruckschlusse auf den Fullungszustand des Kreis- laufes. Die GroBe des zentralen Venendrucks ist jedoch mehrdeutig, insbeson- dere, wenn cardiale Komponenten darauf einen zusatzlichen EinfluB nehmen. Die Sektion "Venendruckmessung" des Mainzer Symposions yom 13. und 14. 0 kto ber 196 7 hatte die Aufgabe, die methodischen und diagnostischen sowie differential-diagnostischen Probleme auf diesem Gebiet zu klaren. Der vorliegende Band stellt einen zusammenfassenden Bericht uber diese wissenschaftliche Sitzung dar. Die Kohlenhydrate sind wichtige energieliefernde Stoffe bei der Infusionstherapie. Sie haben jedoch auBer dieser Aufgabe andere stoffliche Funktionen zu erfullen. Die therapeutischen Moglichkeiten mit Zuckern und Zuckeralkoholen sind mit der Einfuhrung des 5-wertigen Zucker Alkohols X ylit in die klinische Therapie neuzeitlich erweitert worden. Das Xylit weist neben den Vorteilen der Fruktose und des Sorbits weitere gunstige Eigenschaften auf, die dies en Stoff fur die dringliche Infusionstherapie sehr brauchbar machen. Diese Eigenschaften sind: starke antiketogene Wirkung, Anwendbarkeit bei Patienten mit Diabetes mellitus, gute postoperative Verwendbarkeit und die anabole Wirkung. Damit aile interessierten Theoretiker und Kliniker uber diesen neuesten Stand unterrichtet werden, wurden diese Probleme in das Programm des Mainzer Symposions vom 13. und 14. Der erfahrene Kliniker war sich schon immer bewuBt, daB die Entschei- dung tiber Erfolg oder Millerfolg seiner Therapie oft mehr in der pflegen- den Hand seiner guten Schwestem liegt, als in seiner eigenen. Diese Er- kenntnis wissenschaftlich zu fundieren und personell, apparativ und orga- nisatorisch zum Wohle der uns anvertrauten Kranken auf die Hohe unseres arztlichen Wissens tiber die Wiederbelebung zu bringen, ist das Ziel der vorliegenden Zusammenstellung der Vortrage liber "Intensivbehandlung" des Deutschen Chirurgen-Kongresses 1966. Besonderer Wert wurde auf die Darstellung der heutigen Auffassung liber Dauerbeatmung, bilanzierte parenterale Emahrung und Ausbildung und Einsatz des neuen Standes der "Intensivpflegeschwester" gelegt, die nicht mehr Untergebene, sondem mitverantwortliche Partnerin des Arztes ist. Nachdem das Kurznarkoticum Bayer 1420 uber 11/ Jahre experi- 2 mentell und klinisch erprobt worden ist und bereits im Fruhjahr 1963 ein Kolloquium hieruber stattgefunden hat, halte ich die Zeit fur gekommen, uber dieses Anaesthetikum eine oeffentliche Diskus- sion zu veranstalten*. Das Praparat Bayer 1420 ist eine 5 bzw. 2,5%ige Loesung von Propanidid in wasserigem Cremophor EL. Propanidid ist die che- mische Kurzbezeichnung (generk name) fur 3-Methoxy-4-(N, N- diathylcarbamoylmethoxy)-phenylessigsaure-n-propylester. Mit der Bezeichnung Propanidid (Epontol (R)**) ist im Text der folgenden klinischen Arbeiten immer die injektionsfertige Loesung gemeint. Hamburg im Juli 1964 K.
