Das Krankheitsbild der Darmischamie mit konsekutivem Mesen- terialinfarkt und Darmgangran ist schon seit dem 19.1ahrhundert bekannt und immer wieder experimentell und klinisch untersucht worden. Trotz der permanenten Weiterentwicklung diagnostischer Me- tho den und therapeutischer Verfahren hat diese Erkrankung heute noch eine Letalitat von 70-93%. Dies liegt - wie unsere Ergebnisse im Tierversuch und am groBten radiologisch untersuchten Kran- kengut der Weltliteratur zeigen - an der kurzen ischamischen Tole- ranzzeit des Darms nach arteriellem oder venosem GefaBverschluB und an der zu spaten Diagnosestellung. Das vorliegende Buch erarbeitet ein klinisch-radiologisches Konzept, das es ermoglicht, mit Hilfe bildgebender Verfahren -ins- besondere der radiologischen Nativdiagnostik, der Realtime-Sono- graphie und der Angiographie -die Diagnose in vielen Fallen friih- zeitiger zu stell en und somit die Letalitat dieses Krankheitsbildes zu senken. Es zeigt aber auch die Wichtigkeit des interdisziplinaren Ge- sprachs zwischen behandelndem Arzt, Radiologen und Chirurgen, urn die Moglichkeit eines ischamischen Geschehens -insbesondere bei alteren Patienten mit bekannten praexistenten Herz-und Ge- fa13leiden oder hochdosierter Glykosidtherapie - in Erwagung zu ziehen und eine noch erfolgversprechende, kausale Therapie einzu- leiten.
