The estimated prevalence of erectile dysfunction (ED) ranges from 15 - 50 million men in the United States alone, with all age groups of men affected. Despite still-existing taboos surrounding male impotence, more men are seeking help and treatment with the advent of newer treatment options in recent years, most notably oral PDE-5 inhibitors that shed light on the primary vascular cause and treatment target in most cases. In the majority of men, ED has a physical cause and can be effectively treated. Increased knowledge of cause and treatment options, however, requires clinicians to become acquainted with side effects of drugs, alternative treatments, new research in the field, and potential disease conditions that might co-exist with ED. Part of the Oxford American Urology Library, this practical manual illuminates the pathophysiology of erectile dysfunction and approaches to diagnostic evaluation and therapeutic options, including specifics of ED in men with chronic diseases such as diabetes, hypertension, coronary artery disease, heart failure, and chronic kidney disease. All currently available treatment options from prostaglandins to vacuum devices, PDE-5 inhibitors, and interventional and surgical therapies are described for their efficacy and safety based on the published literature and ongoing research trials.

In Imagining the Holocaust, Daniel R. Schwarz examines widely read Holocaust narratives which have shaped the way we understand and respond to the events of that time. He begins with first person narratives-- Wiesel's Night and Levi's Survival at Auschwitz --and then turns to searingly realistic fictions such as Borowski's This Way to the Gas Chamber, Ladies and Gentlemen, before turning to the Kafkaesque parables of Appelfeld and the fantastic cartoons of Spiegleman's Maus books. Schwarz argues that as we move further away from the original events, the narratives authors use to render the Holocaust horror evolve to include fantasy and parable, and he shows how diverse audiences respond differently to these highly charged and emotional texts.

Doch fallt auf, wie wenig oft gerade die im Krankenhaus Tatigen sich dartiber im klaren sind, welche Konsequen- zen es hatte, wenn dieser Grundsatz ernst genommen wtirde. Gegentiber Fragen der Krankenhausorganisation, des Zusammenspiels zwischen arzt- lichem und administrativem Bereich, der wechselseitigen Beeinflussung zwischen der Gesamtgemeinschaft und ihren einzelnen Mitgliedern usw. herrscht oft eine Art blinder Selbstgefalligkeit, die nicht selten an Ignoranz grenzt. Dabei liegen etwa 50 Jahre intensiver Forschung tiber die psychi- sche und soziale Situation des Patienten im Krankenhaus hinter uns. Deren Ergebnisse konnten aber bisher nur in begrenzter Weise praktisch umge- setzt werden. Das trifft leider auch fUr Krankenhauser zu, deren erklartes Ziel die seelische Gesundung ihrer Patienten ist. Gerade beim therapeutischen Urn gang mit Menschen, die psychische StOrungen haben und im Krankenhaus behandelt werden, stehen uns kaum Mittel pharmakologischer oder physikalischer Art zur VerfUgung, urn ihre Schwierigkeiten zu beheben.
