as a creator of seminal works of Land art, Walter De Maria also
maintained an extensive and highly original practice as a sculptor,
painter, draftsman, writer, filmmaker, and composer. This expansive
publication presents more than 200 works created or envisioned by
the artist from 1960 until his death in 2013. Each work is fully
illustrated and accompanied by detailed text entries written by
Elizabeth and Michael Childress, supplemented by archival images,
many of which have never been published before. Essays by Michael
Govan, Christine Mehring, and Lars Nittve provide detailed
overviews and analyses of the early, middle, and later periods of
De Maria's production. The monograph also includes an introduction

Blastomycosis remains the most enigmatic of human mycotic
infections. The enigmas encompass the natural habitat of the
etiologic agent, extent of exposure and subclinical infections in
endemic areas, distribution of en demic foci throughout the world,
inconsistency of serologic evaluation of infected patients, and
varying response of such patients to standard treat ment regimens.
In spite of diligent investigations by many competent
investigators, we still do not know the ecological niche inhabited
by the etiologic agent. We have many tantalizing clues but no
definite answers. Nor do we know the extent of the endemic areas in
the world for this disease. Skin testing, so useful in defining the
distribution of histoplasmosis and coccidioidomycosis, has been of
no value in mapping endemic areas for blastomycosis. Even the
serologic evaluation of known cases of the disease has been too
erratic in results to be useful as a diagnostic or prognostic
procedure. The enigmas of blastomycosis go straight to the disease
itself. There is an extensive literature on the debate concerning
the presence and extent of subclinical infections. Case report
series demonstrate that such transient infections do occur but,
unlike other mycoses, the extent to which this phe nomenon is
common in the general population still cannot be assessed. Even the
diagnosis of established disease is a major clinical problem.
The anesthesia and intensive care in infants and children have assumed positions of primary importance in contemporary medicine. In these delicate sectors of medicine, clinical research activities must be continually supported by interdisciplinary collaborations. Neonatal and pediatric intensive care requires that all involved physicians, including the surgeon and anesthesiologist, be willing to work together as a team. However, coordination of the various pathophysiological and clinical aspects of neonatal and pediatric critical medicine is highly complex. A complete and current review of the anaethesia and intensive care of infants and children must include discussions of morbidity rates, so as to guide the clincians in choosing the diagnostic approach, as well as of monotoring techniques appropriate to support the therapeutic decision. Recent progress in intensive care medicine for infants and children has been significant.
