The modern rehabilitation medicine is an important part of the comprehensive me- cal attendance of hemophiliacs. The necessary multidisciplinary team of specialists is available in a qualitative high-class rehabilitation hospital. Indispensable is the point that the coordinating medical specialist in Rehabilitation Medicine on site is skilled in the treatment of hemophiliacs. But he doesn't want and he can't replace the patient's hematologist. He is just the coordinator of the comprehensive team in the rehabilitation clinic. I hope I have shown that rehabilitation is an important topic for most patients with hemophilia. References 1. Some 23. 5% of all members and patients of treatment centers completed the questionnaire. One striking fact is that participation was highest amongst elderly patients with severe hemophilia, whereas only 12. 2% of family members responded. Evaluation of the results revealed that the majority of participants prefer treat ment in a hemophilia treatment center with a high reputation, whereas only 2. 7% consider treatment in a specialized practice to be sufficient. The reasons for that are the necessity of the 24-h-availability of a physician, regular qualified examination of joints and muscles, documentation of product batches, laboratory tests and good cooperation with other faculties. As many as 68. 9% of the patients sometimes travel more than 200 km. What certainly plays a role here is trust in the treatment center and its physicians, for it is noticeable that irrespective of the fact that 74. 3% re quested standardized treatment regimens for all treatment centers, only about 20% would change to a nearer center. Treatment with factor concentrates is generally considered to be very safe. That is to say, 58. 1 % regard recombinant products to be very safe, whereas only 24. 3% assume this for plasma products. When compared, there were usually no major differences observed regarding outcome (factor consumption, duration of treat ment). Increase in maximum storage temperature (47. 2%) and half-life (73%) were the most frequent answers to the question of what features the products should have."

About 31% of the patients with a factor VIII replacement therapy develop a factor VIII inhibitor.From these are 23% low-responder (< 5BE) and 77% high-responder (> 5BE) [8].In the case of severe hemophilia B,about 10.5% of the patients develop inhibitory antibodies [9]. Anti-factor VIII-antibodies are also seen in 15-78% healthy people without hemophilia [7, 17, 19]. Lacroix-Desmazes et al. [10, 11] showed anti-idiotypic antibodies neutralizing the inhibitory activity of the an- factor VIII antibodies in healthy people. Well-known predisposing factors for inhibitor formation are genetic features of factor VIII,which include large deletions,nonsense mutations or intrachromosomal recombinations [5, 23].Also, ethnic groups other than Caucasians (e.g.Africans) have a higher risk of developing inhibitors.Other risk factors are presumably de- ved from the immune system. For instance, a reduction of the inhibitor was seen with lower CD4+ T helper cell counts in HIV positive hemophilic patients [3,4].The development of inhibitors is very likely to be a Th-2 mediated event where cyto- nes and their receptors,T-cell receptors and the Major Histocompatibility Complex may also play an important role. Hamophilie-Symposion in Hamburg und freue mich, auch in diesem Jahr wieder viele Teilnehmer aus unseren Nachbarlandern begruBen zu konnen. Ganz besonders hervorheben aber mochte ich, daB es in diesem Jahr Kolleginnen und Kollegen aus Dresden, Magdeburg und Rostock erstmals gelungen ist, zu unserer Tagung zu kommen. Wir nehmen das als hoffnungsvolles Zeichen und begriiBen Sie hocherfreut und herzlich in unserem Kreise. Ais betriiblich muB ich Ihnen hingegen mitteilen, daB sich unser altvertrauter und engagierter Mitveranstalter, Herr Professor Rudolf Marx, die Teilnahme in letzter Minute aus gesundheitlichen Grunden und gebotener Vorsicht zu seinem groBten Bedauern versagen muBte. Hervorragende Fortschritte in der Verhii tung bedrohlicher Blutungsfolgen, ein erster Durchbruch zu langer Lebens zeiterwartung und zur vollen sozialen Eingliederung waren die Meilensteine auf der ersten Halfte dieses Weges. Die Kraft dieses uberzeugenden Erfolges war so groB, daB selbst erste ernsthafte Hinweise auf eine womoglich hohe Rate chronisch verlaufender Transfusionshepatitiden gegen Ende der 70er Jahre den Nutzen der Therapie nicht in Frage stellen konnten."

Die operative Knochenbruchbehandlung hat in den letzten Jahren eine starke Zunahme ihrer Indikationen erfahren. Unter Zuhilfenahme metallischer Fremdkoerper lasst sich zumeist eine sog. ubungsstabile Osteosynthese erreichen. Trotzdem hat die Knochentransplantation bei diesen Verfahren und anderen Anwendungsgebieten ihre Berechtigung. Defekte und Inkongruenzen bei Trummerbruchen oder Falschgelenken werden mit Knochentransplantaten aufgefullt. Ebenso werden Knochen- hoehlen nach Entfernung gutartiger Tumoren in gleicher Weise behandelt. Neben dieser Eigenschaft als Fullsubstanz ist vor allem aber die osteo- genetische Potenz des Pflanzlings von grosser Bedeutung fur die Heilung der Knochenwunde. Allo-, homo- und heterologe Spongiosa- und Corticalisspane stehen zur Verfugung. In der vorliegenden Schrift hat mein fruherer Mit- arbeiter, Herr Schramm, die ausgezeichneten Eigenschaften des auto- logen Spongiosaspanes im Tierexperiment mit neuen Methoden der Auf- arbeitung bewiesen. Das weitverzweigte Maschenwerk dieses Gewebes gewinnt in einem gut durchbluteten Lager schnellen und guten Anschluss an das Wirtsgewebe. Bei der durch die Osteosynthese gewahrleisteten Stabilitat ist keine Schienung mit dem "Knochenprugel" Lexers aus Corticalis mehr erforderlich. Der ungestoerte Ein- und Umbau der Spongiosa ist durch die absolute Ruhigstellung gesichert. Die klinischen Anwendungsbereiche und die operative Methodik, wie sie am "Berg- mannsheil" Bochum gehandhabt werden, kommen zur Darstellung. Die experimentell gesicherten Ergebnisse erganzen in vorzuglicher Weise die in der Praxis erarbeiteten klinischen Erfahrungen. Die Monographie sollte die gute operative Methode einem weiten Kreis von Kollegen erschliessen. Herr Professor Dr. Dr. med. h. c. Heinz Burkle de la Camp vollendet am 3. 6. 1970 sein 75. Lebensjahr.
