The therapeutic principle of pharmacological vasodilation in
congestive heart failure is based upon established physiological
and pathophysiological mechanisms and has been proved to be
effective and useful. In acute heart failure this method of
treatment has assumed importance as the first measure to be taken.
In chronic heart failure a sizeable increase in cardiac output can
be achieved with a number of vasodilating drugs. This effect can be
maintained over several weeks or months. The adjustments of the
heart and the circulation to the arteriolar and/or venous unloading
effect of vasodilatation in the situation of acute and chronic
heart failure are well understood. However, long-term adaptations
of the organism to the vasodilat- ing intervention have not yet
fully been described. In many instances of chronic ad- ministration
of vasodilators, attenuation of the response has been observed. The
mech- anisms responsible have not been fully elucidated, nor has
there been a satisfactory demonstration of improved exercise
performance or of life expectancy under this mode of treatment in
the patient with chronic heart failure. In an attempt better to
understand the pathophysiological mechanisms of chronic congestive
heart failure as they apply to long-term treatment with
vasodilators, the working group on vasodilator therapy of the
Deutsche Gesellschaft fUr Herz- und Kreis- laufforschung has
assembled a group of physiologists, pharmacologists, and
clinicians. The Proceedings of their Symposium held in Hinterzarten
in June 1981 are presented here.
The aim of this book is to give an up to date account of recent
developments in the field of cardiomyopathy. Many authors, all
experienced workers in their fields, - most from Germany - have
contributed to this volume. It would appear that the diagnosis of
cardiomyopathy is now made more often than it was 10 years ago. The
reason for this is probably not that the incidence of the disease
has increased, but that it is better recognized. Where conventional
clinical investigation may have failed to define the underlying
disease process in a given patient, improved or more widely used
diagnostic tools, such as selective coronary arteriography and
endomyocardial biopsies, have contributed greatly to proper
diagnosis. Furthermore, uniformity of terminology has also taken
place during the last few years, permitting more accurate
categorization of patients suffering from this entity. The
difficult subject of terminology is discussed in a special section.
This volume contains the proceedings of the Fourth International
Symposium on Coronary Heart Disease, held in Frankfurt/Federal
Republic of Germany. In 1970, when the fIrst symposium was held,
the whole range of diagnostic and therapeutic measures was covered,
but since then interest in coronary heart disease has increased so
considerably that the fourth symposium was restricted to only two
therapeutic aspects, angioplasty and intracoronary thrombolysis. It
can be noted with pleasure that European cardiologists, namely
Andreas Griintzig and Peter Rentrop, have made essential
contributions to the applicability of these modes of treatment.
When angioplasty was fIrst discussed at the Third International
Symposium (Coronary Heart Disease, Stuttgart, 1978) a great deal of
astonishment was expressed. Today the procedure has gained an
important place in the treatment of angina pecto ris. The
proportion of candidates for coronary surgery in whom angioplasty
can be attempted instead of operation is around 10%, in certain
subgroups even higher. The most remarkable fmding of our 5-year
experience with angioplasty are the extre mely good long-term
results. Recurrences happen in about 15%-20% of cases within the
fIrst 3 months; after this period of time, recurrences are very
unusual and mostly due to progression of the disease at other
sites. Angioplasty has thus also contributed to our understanding
of the dynamics of the atherosclerotic process.
Die im Jahre 1981 publizierte Entdeckung von de Boldt und
Mitarbeitern, daB ein Extrakt aus Herzvorhofgewebe eine ausgepragte
Steigerung der renalen Wasser-und Elektrolytausscheidung bei
Versuchstieren bewirken kann, geh6rt sicherlich zu den gr6Bten
Uberraschungen der biomedizinischen Forschung innerhalb des letzten
Jahr- zehnts. DaB das Herz neb en seiner zentralen Rolle im
Kreislaufgeschehen auch noch die Rolle einer Hormondriise innehat,
muBte die Phantasie vieler Peptid-und Kreis- laufforscher
befliigeln. Schon innerhalb weniger Jahre gelang die Entlarvung und
Strukturaufklarung der atrialen natriuretischen Peptide, und die
Arbeiten zu ihrer phy- siologischen und pathophysiologischen
Bedeutung sind zur Zeit in vollem Gange. In dieser Situation
veranstaltete die Arbeitsgruppe "Vasodilatantien" der Deutschen
Gesellschaft fUr Herz-und Kreislaufforschung in Heidelberg ein
Symposium mit dem Thema "Atriales natriuretisches Peptid und das
kardiovaskulare System", zu dem theo- retisch und klinisch
orientierte Wissenschaftler aus der Bundesrepublik Deutschland,
Osterreich und der Schweiz eingeladen wurden. Das vorliegende Buch
enthalt die auf diesem Treffen prasentierten Beitrage in iiberarbei
eter und erweiterter Form. Es glie- dert sich in vier Teile, wobei
sich Uberschneidungen der Themenkreise nicht immer vermeiden
lieBen. Der erste Teil enthalt Beitrage, die der
Grundlagenforschung ent- stammen, der zweite Teil referiert
Untersuchungen zur Physiologie und Pathophysiolo- gie der atrialen
natriuretischen Peptide, der dritte Teil ist vor allem
klinisch-medizini- schen Aspekten der atrialen Hormone vorbehalten,
und der vierte Teil gibt einen kur- zen Uberblick iiber den
derzeitigen Kenntnisstand.
Die koronare Herzkrankheit fordert in allen zivilisierten Landern
einen hohen Zoll an Mortalitat und Morbiditat. Es ist deshalb
notwendig, nach Wegen zu suchen, dieser - zumindest in Deutsch-
land ungebrochenen -Entwicklung Einhalt zu gebieten. In manchen
Fallen genugt allein schon eine Anamnese, um relativ fruh und mit
hoher Treffsicherheit eine Myokardischamie auf dem Boden einer
stenosierenden Koronarsklerose zu diagnostizieren. In den meisten
Fallen sind jedoch wei- tergehende diagnostische Schritte zum
Ausschluss oder zur Bestatigung einer koronaren Herz- krankheit
erforderlich. Das vorliegende Buch will praktische Wege aufzeigen,
die mit moeglichst geringem Aufwand zu einer adaquaten Diagnose
fuhren. Deshalb ist bei der Darstellung der verschiedenen
Krankheits- stadien bzw. Symptomkonstellationen stets auf ein
ausgewogenes Verhaltnis zwischen diagnosti- schem Aufwand und
moeglichen therapeutischen Konsequenzen abgehoben. Entsprechend
ihrer diagnostischen Wertigkeit werden zunachst die nichtinvasiven
Methoden dargestellt. Dabei gilt dem Belastungs-EKG als dem wohl
wichtigsten Parameter unser besonderes Augenmerk. Neben zahlreichen
subtilen technischen Details findet der Leser Angaben uber
Treffsicherheit und Gren- zen dieser Methode. Besondere
Aufmerksamkeit wird auch dem Stellenwert eventuell notwendi- ger
erganzender Untersuchungsmethoden gewidmet, wie u. a. der
konventionellen Roentgendia- gnostik, der Echokardiographie und der
Nuklearmedizin. Die Verfasser sind der Meinung, dass die
Koronarangiographie in jedem Falle einer strengen Indikation
bedarf. Andererseits darf den Patienten, fur die lebenswichtige
therapeutische Mass- nahmen von dieser Untersuchung abhangen, die
Koronarangiographie nicht vorenthalten werden.
