The Bioethics of Pain Management - Beyond Opioids (Paperback): Daniel S Goldberg The Bioethics of Pain Management - Beyond Opioids (Paperback)
Daniel S Goldberg; Series edited by Mark Cherry, Ana S. Iltis
R1,372 Discovery Miles 13 720 Ships in 12 - 17 working days

In this book, public health ethicist Daniel S. Goldberg sets out to characterize the subjective experience of pain and its undertreatment within the US medical establishment, and puts forward public policy recommendations for ameliorating the undertreatment of pain. The book begins from the position that the overwhelming focus on opioid analgesics as a means for improving the undertreatment of pain is flawed, and argues instead that dominant Western models of biomedicine and objectivity delegitimize subjective knowledge of the body and pain in the US. This general intolerance for the subjectivity of pain is part of a specific American culture of pain in which a variety of actors take part, including not only physicians and health care providers, but also pain sufferers, caregivers, and policymakers. Concentrating primarily on bioethics, history, and public policy, the book brings a truly interdisciplinary approach to an urgent practical ethical problem. Taking up the practical challenge, the book culminates in a series of policy recommendations that provide pathways for moral agents to move beyond contests over drug policy to policy arenas that, based on the evidence, hold more promise in their capacity to address the devastating and inequitable undertreatment of pain in the US.

Presurgical Psychological Screening in Chronic Pain Syndromes - A Guide for the Behavioral Health Practitioner (Paperback):... Presurgical Psychological Screening in Chronic Pain Syndromes - A Guide for the Behavioral Health Practitioner (Paperback)
Andrew R. Block
R1,602 Discovery Miles 16 020 Ships in 12 - 17 working days

Pain is an unfortunate daily experience for many individuals. Chronic pain -- lasting six or more months -- is suffered by approximately 30% of the population in the United States. These individuals wake up, function during the day and go to sleep, trying to keep pain at a minimum while, at the same time, maintaining some quality of life. They may make frequent visits to the doctor and the pharmacy. When they find relief, it is usually short-lived and comes at a cost such as dependence on narcotic medications or complete limitation of activity. Pain often becomes the central point of their existence.
This practice guide describes an approach to psychological evaluation of the chronic pain patient who is being considered for surgery. A large body of research is accumulating which demonstrates that the outcome of surgical procedures aimed at chronic pain relief can be strongly influenced by psychological and emotional factors. This approach, termed "presurgical psychological screening" (PPS) uses interview and testing techniques to identify emotional, behavioral, and psychosocial difficulties which have been demonstrated to negatively impact surgical outcome. Studies show that even patients with clearly identifiable pathophysiology may respond poorly to surgery, due to issues such as pain sensitivity, medication dependence, rewards for pain behavior and personality style. Thus, some insurance carriers, rehabilitation nurses and state worker's compensation systems are encouraging, or even requiring, presurgical psychological screening in cases of surgery designed to relieve chronic pain.
The first to present a comprehensive, unified approach to PPS in chronic pain syndromes, this text is designed to provide the behavioral health practitioner, as well as the trainee, with all the tools and information necessary to conduct PPS evaluations. It identifies a multitude of risk factors for poor surgical outcome and reviews research associated with each risk factor. Hands-on techniques for eliciting information from the patient about risk factors is also detailed. Toward this end, the practice guide also contains a number of forms and session outlines which can be directly utilized, or which can be altered to fit readers' needs. Models for weighing and combining surgical outcome risk factors are also provided. Thus, practitioners are able to reach valid and reliable predictions of surgical results. Finally, the text provides outlines of psychological interventions which can facilitate surgical outcome as well as surgical treatment alternatives. Upon completion of this practice guide, readers should be able to begin providing PPS evaluations which are scientifically valid, clinically sound, and which result in significant overall improvement in the treatment of chronic pain syndromes.

Armfuls of Time - The Psychological Experience of the Child with a Life-Threatening Illness (Hardcover): Barbara M. Sourkes Armfuls of Time - The Psychological Experience of the Child with a Life-Threatening Illness (Hardcover)
Barbara M. Sourkes
R3,326 Discovery Miles 33 260 Ships in 12 - 17 working days

"I just wish I had armfuls of time." These are the words of a four year old facing a life-threatening illness. This text portrays the psychological experience of such children, who are irreversibly changed from the moment of diagnosis. Barbara Sourkes is a psychologist who specializes in psychotherapy with children who have cancer and other serious diseases. In the account, she describes how she works with these children, using drawings, soft toys and dolls, stories and real medical instruments to allow them to communicate their experience of the illness, the treatment they undergo, their relationship with their families, and their feelings of grief and loss in coming to terms with the prospect of death. Making use of the words of children, offering interpretations and practical advice, this is a book that should be useful reading for those concerned with the care of terminally ill children.

M. Eric Gershwin
R4,385 Discovery Miles 43 850 Ships in 12 - 17 working days

Those who do not feel pain seldom think that it is felt. SAMUEL JoHNSON, The Rambler, no. 48 (September 1, 1750) Who among us has not experienced the suffering of a patient with chronic disease, who in addition to the vicissitudes of fatigue, anxiety, and frustration, must also deal with the suffering of pain? Who among us has not considered, and then reconsidered, whether a patient's complaints are worthy of a narcotic and thence worried about the social and legal implications of chronic use? Who among us has not refused pain medications to our patients for fear that use was turning into abuse? Finally, who among us would not have liked a clinical guide to a myriad of syndromes, all of which have pain as their common denominator, in the hopes of developing some strategy to prioritize treatment. Our purpose in preparing The Pain Management Handbook is to provide the informa tion needed by clinicians to develop strategies that optimize pain management. It is the goal of the editors and authors that the present handbook, above all else, will be clinically useful. Its aim is to provide practical information regarding the diagnosis and treatment of disorders causing pain, along with tables and graphics to provide the busy practitioner with rapid access to relevant data."

Hooshang Hooshmand
R8,228 Discovery Miles 82 280 Ships in 12 - 17 working days

The most misunderstood and complex subject in medicine is the hyperpathic pain of sympathetic dystrophy. More common than previously thought, it comprises between 10 and 20 percent of chronic pain patients. Understanding this self-perpetuating pain -- which "never stops" -- requires unbiased knowledge of physiology and pathology.
Chronic Pain: Reflex Sympathetic Dystrophy, Prevention, and Management is devoted to the subject of Reflex Sympathetic Dystrophy (RSD). The book classifies the different stages of RSD and describes the qualitative and quantitative differences between natural endorphins and synthetic narcotics. Included are long-term follow-ups on sympathectomy patients.
This important reference explains why sympathectomy fails, but nerve block and physiotherapy is successful in the treatment of RSD. In addition, the mechanism of development of RSD is clarified through an extensive collection of drawings and anatomical pictures as well. Other topics include thermographic methods for the diagnosis of RSD, the role of ACTH in the management of chronic pain, and comparisons between the effects of ACTH and those of corticosteroids.

Anesthesia for Trauma - New Evidence and New Challenges (Hardcover, 2014): Corey S. Scher Anesthesia for Trauma - New Evidence and New Challenges (Hardcover, 2014)
Corey S. Scher
R3,657 Discovery Miles 36 570 Ships in 12 - 17 working days

Trauma is the leading cause of death among people under the age of 40 and it ranks third for all age groups. Still, relatively few clinicians specialize in trauma and training is often obtained through experience. The number of trauma patients is expected to continue to grow as pre-hospital care continues to advance. As well, hospitals increasingly see trauma treatment, which requires no pre-approval, as a good source of revenue. Given these developments, the number of opportunities for specialists trained in trauma, including anesthesiologists and critical care physicians, will expand in the years ahead. This book addresses the need for an up-to-date, comprehensive and clinically focused volume for practitioners and trainees in trauma anesthesia and critical care. It is organized by organ system. The editor is an attending physician at a major urban hospital center recognized worldwide for its outstanding emergency medical services including trauma care and is recruiting leading trauma anesthesiologists to contribute. Anesthesiologists, pain medicine physicians, critical care physicians and trainees are the target audience.

"A fascinating, totally seductive read!" -Eula Biss, author of Notes from No Man's Land: American Essays and On Immunity: An Inoculation "A book built of brain and nerve and blood and heart. . . . Irreverent and astute. . . . Pain Studies will change how you think about living with a body." -Elizabeth McCracken, author of Thunderstruck and Bowlaway "A thrilling investigation into pain, language, and Olstein's own exile from what Woolf called 'the army of the upright.' On a search path through art, science, poetry, and prime-time television, Olstein aims her knife-bright compassion at the very thing we're all running from. Pain Studies is a masterpiece." -Leni Zumas, author of The Listeners and Red Clocks In this extended lyric essay, a poet mines her lifelong experience with migraine to deliver a marvelously idiosyncratic cultural history of pain-how we experience, express, treat, and mistreat it. Her sources range from the trial of Joan of Arc to the essays of Virginia Woolf and Elaine Scarry to Hugh Laurie's portrayal of Gregory House on House M.D. As she engages with science, philosophy, visual art, rock lyrics, and field notes from her own medical adventures (both mainstream and alternative), she finds a way to express the often-indescribable experience of living with pain. Eschewing simple epiphanies, Olstein instead gives us a new language to contemplate and empathize with a fundamental aspect of the human condition. Lisa Olstein teaches at the University of Texas at Austin and is the author of four poetry collections published by Copper Canyon Press. Pain Studies is her first book of creative nonfiction.
