Medicaid is the largest grant-in-aid program in the United States. Reform in this area, therefore, provides a unique opportunity to study the intersection between federal and state policy making in an area recently characterized by substantial uncertainty deriving from the lingering effects of the Great Recession, ongoing debate over the federal budget, and implementation of the Patient Protection and Affordable Care Act. Invariably states reform the way health care is delivered, regulated, and financed within broader parameters established by federal statutes and regulations. It is critical therefore that effective strategies be put into place if both current and future health and long-term care reform efforts are to have their greatest chances at success. Rhode Island is the first state to receive permission to operate its entire Medicaid program under a global cap. As a consequence, it has entered the national consciousness as a key data point potentially supporting the block grant approach to Medicaid reform. In this book, Edward Alan Miller identifies factors that either facilitated or impeded the design and implementation of Rhode Island's Global Consumer Choice Compact Medicaid Waiver in order to draw broader lessons for the Medicaid block grant debate and health and long-term care reform more generally. Evidence gathered from archival sources and in-depth interviews with key stakeholders exposes the role that provider capacity has played in the implementation process, including adult day care, assisted living, home maker, and other home- and community-based services. The impact of the Global Waiver on the nursing home sector is examined as well, in addition to new authority to obtain federal matching dollars for previously state-only funded programs. By providing a sophisticated understanding of factors enhancing or impeding state health reform, this book will contribute to improvements in the development and administration of policy development at both the state- and federal-levels.
American cities experienced an extraordinary surge in downtown development during the 1970s and 1980s. Pro-growth advocates in urban government and the business community believed that the construction of office buildings, hotels, convention centers, and sports complexes would generate jobs and tax revenue while revitalizing stagnant local economies. Power and Party in an English City provides an account of how decisions are taken by the state at the level of locality. More specifically, it is an account of the private policy-making activities of a ruling Labour group of councillors in the major English city of Newcastle-Upon-Tyne. Despite the fact that local government in most of the towns and cities of England is one-party government, very little is known abotu the private behaviour of ruling party groups. In this book David Green provides a penetrating empirical study of the realities of local government. The author seeks to examine and analyse the importance of party discipline, the relationship between the Labour group of councillors and the party outside the council, the power of the committee chairmen, the role of local patronage and the openness of the local policy-making process. The government of Newcastle is perhaps the most closely association in the public mind with T. Dan Smith, the corrupt local politician. In fact, Smith had left local politics in Newcastle in teh mid-1960s. How was the city being run a decade or so later? This study is however much more than an inside view of the affairs of a single authority. The last part of the book is devoted to a discussion of aspects of some traditional and modern theories of democracy and specifically to what author sees as the inadequate advocacy of participatory democracy in recent years. Green makes a major contribution to our thinking about the kind of democracy that is possible in modern large-scale societies, explores weaknesses of moder theories and puts forward some original modifications to modern democratic theory, in the light of a theory of knowledge which is seen as more appropriate for modern natural and social scientific activity. Local government passed unscathed through the political and economic upheavals which followed the Great Depression. Contemporary commentators had every reason to look forward to continued growth and expansion in the role of local government, which was seen as the main vehicle for the social programmes of the comeing Welfare State. That optimism was misplaced. Many of the trends of the early twentieth century have been reveresed. From the vantage point of 1985, local government was in crisis so severe that its continued existence was called into question. In this unique book eleven authors explain what happened and how the local government system weakened. Political, financial, economic and legal issues are explored, as are factors such as housing, planning, and social welfare. This book was first published in 1985.
Using the historic Minnesota state government shutdown of 2011 as a backdrop, Interfaith Advocacy describes the work of the Joint Religious Legislative Coalition, an interfaith advocacy group that brings together leaders from Catholic, Protestant, Jewish, and Muslim traditions to advocate on behalf of a range of policies. To see like a city, rather than seeing like a state, is the key to understanding modern politics. In this book, Magnusson draws from theorists such as Weber, Wirth, Hayek, Jacobs, Sennett, and Foucault to articulate some of the ideas that we need to make sense of the city as a form of political order. Locally and globally, the city exists by virtue of complicated patterns of government and self-government, prompted by proximate diversity. A multiplicity of authorities in different registers is typical. Sovereignty, although often claimed, is infinitely deferred. What emerges by virtue of self-organization is not susceptible to control by any central authority, and so we are impelled to engage politically in a world that does not match our expectations of sovereignty. This book examines one of the most high-profile municipal privatizations-the privatization of New York City's Central Park. The fiscal crisis of the 1970s established the political and cultural opening for privatizations, which were justified on the basis of increasing efficiency. However, as Cooke demonstrates, these justifications were deliberately blind to the social and economic implication of privatization. This fascinating account moves beyond the hackneyed pro- versus anti-privatization debate by reconceptualizing the park's privatization as an ensemble of contradictory class effects. The book addresses in detail local governance in Spain. In recent decades, local governments in Europe have increasingly found themselves under pressure from a multitude of new challenges, such as demographic change, climate change, fiscal austerity policies, digitization, the demand for more citizen participation in local affairs, and the migration crisis in some of them, to name just a few. Consequently, a wave of political and administrative reforms to address these challenges, pressures and problems, has changed local governance in many countries. In part, these changes were the result of reform policies introduced by national and state governments, often triggered by austerity policies, which has become an overwhelming reality for Spanish local governments that have been forced to introduce innovations in local governance. This book aims to give an account of these innovations in local governance in Spain. This book considers the local political-administrative structure in its dimensions, focusing on the analysis of its party system, electoral competition and political behavior in the local arena, as well as on local finances, all of which are determining elements in urban and rural governance processes. One important tradition in political science conceives of the Civil War in the United States serving as the functional equivalent of the English and French Revolutions, bringing with it the victory of liberal democratic industrialism over aristocratic agriculturalism. From this perspective, the Civil War is notable for its impact on the American state. Surprisingly however, little attention has been paid to the distinguishing features of this historic rupture in American politics. This book, first published in 1992, examines the attitudes of local elites - the hinge between Indian state and rural society - towards protest and participation in development, illuminating arguments about the nature of the state as well as the development process. It looks at the role of local elites in India both as the representatives of the state and of the rest of rural society, and explains their importance in the country's development. The book deals with the elites' contribution to the credibility of the state and examines the strategies through which they manipulate the allocation of resources and influence the pace and direction of social change. It contrasts the rural elites in two areas, one more economically advanced than the other. The elites in the first area were shown to be capable of combining institutional participation with radical protest, whilst in the other they tended to rely on state channels to achieve reform. The author concludes that despite the different settings, both groups were informed, active and responsive to political conditions. This contrasts with the conventional view that local elites of the dominant castes oppress the lower ones by obstructing reforms, for reasons of self-interest.

There is no one-size-fits-all decentralized fix to deeply divided and conflict-ridden states. One of the hotly debated policy prescriptions for states facing self-determination demands is some form of decentralized governance - including regional autonomy arrangements and federalism - which grants minority groups a degree of self-rule. Yet the track record of existing decentralized states suggests that these have widely divergent capacity to contain conflicts within their borders. Through in-depth case studies of Chechnya, Punjab and Quebec, as well as a statistical cross-country analysis, this book argues that while policy, fiscal approach, and political decentralization can, indeed, be peace-preserving at times, the effects of these institutions are conditioned by traits of the societies they (are meant to) govern. Decentralization may help preserve peace in one country or in one region, but it may have just the opposite effect in a country or region with different ethnic and economic characteristics. |
