Since the 1980s, ankle arthroplasty has been considered a dubious treatment for painful and degenerated joints. Many of the concerns that existed previously have been solved today. Prosthetic replacement of other joints also took a long time before reaching their present successful status. This is the first book to deal with ankle arthroplasty. International experts have given their opinion on (i) the kinematics of the ankle; (ii) the basics of replacing the ankle joint; and (iii) the current status of several modern ankle-prosthesis designs. A better understanding of the ankle kinematics, alignment techniques and biological stabilisation and the results of newer designs have changed the former attitude worldwide. Professor Gerhard Kuntscher trat 1940 vor die Deutsche Gesellschaft fUr Chir- urgie und stellte erstmals seine Ergebnisse der Marknagelung zur Behandlung von Frakturen der breiten Offentlichkeit vor. Die uberwiegende Ablehnung der Marknagelung vor dem wissenschaftlichen Forum wurde durch ihren Erfolg in den Kriegsjahren wett gemacht, und die eloquente Kritik der epochalen Opera- tionstechnik wurde in eine unerwartete weltweite Anerkennung der Methode verwandelt. In den Nachkriegsjahren wurde die Osteosynthesetechnik von Frakturen durch das Dogma der rigiden stabilen Osteosynthese gepragt. Die Kuntscher- Technik wurde erneut in den Hintergrund gedrangt. Das alte Argument gegen die Marknagelung, die Zerstorung der intramedullaren Blutversorgung, wurde von Kuntschers Gegnern ebenso heftig diskutiert wie die Gefahr der Rontgen- strahlen wahrend der Durchleuchtung. Das Pendel der Osteosynthesetechnik kehrte jedoch zur biologischen Operation von Kuntscher zuruck. "Metastatic Bone Disease: Fundamental and Clinical Aspects" is an expanded version of a workshop on bone metastases which was held in Heidelberg, Germany, in April 1993. Bone metastases very often develop from malignant tumors and lead to complications which considerably diminish the quality of life of the patient. Bone pain, pathological fractures, and hypercalcemic syndrome should be recognized and treated ear- ly, so as to prevent any further morbidity and immobilizati- on. Antiosteolytic substances such as biophosphonate can supplement the established palliative and supportive thera- pies. Highly qualified basic researchers and clinicians in pathophysiology, diagnosis, and treatment of metastatic bone disease have contributed to this book, providing a good overview ofpresent knowledge and research. The book will be of interest not only to specialists, but also to any physi- cian interested in oncology.
In the field of orthopedic surgery, additional areas of application and new indications have been discovered for fibrin glue treatment, for example, hemostasis in pseudotumors in hemophilic patients or in torn ligaments, and in spongiosa transplantation, primarily when nonautologous bone material is being used, or to fill large defects. In maxillofacial surgery fibrin glue is mainly used for osseous contouring of the facial and frontal bones, for alveolar ridge augmentations in preprosthetic surgery, and for soft tissue reconstructions.
In this volume a distinguished group of internationally renowned clinicians and basic researchers discuss the present state of knowledge of the etiologyand pathogenesis of Dupuytren's contracture, a disease responsible for a considerable portion of disabilities within the working population allover the world. Although the riddle of how the contracture of the palmar fascia develops is still unsolved, the data on the pathobiochemistry and clinic of Dupuytren's Disease achieved in recent years has led to a better understanding of the biochemical and morphological processes underlying the deformation and malfunction of the afflicted tissues. Research in Dupuytren's Disease now enters the era of molecular medicine, which opens up new experimental means of studying the pathological changes which occur during the formation of the contracture on a molecular level. In particular, data are presented as to the role of specific biological macromolecules influencing the phenotype expression of the palmar fascia cells which are involved in autoimmune reactions and present both at the cell surface as well as in the extracellular matrix to regulate cell-to-cell and cell-to-matrix interactions. In vitro cell culture models to investigate fibro-cytic modulations are available and havebeen applied to study the effects of specific biological molecules on isolated cells from healthy palmar fascia and Dupuytren's contracture. Complementary evaluations of morphological, epidemiological, and clinical data contribute essentially to the present understanding of the etiology and pathogenesis of Dupuytren's Disease. This comprehensive and informative summary of the state of the art is completed by numerous references affixed to the individual contributions.
References, to the reader, are like insulin to the diabetic: when needed they are indispensable, but in excess they induce coma. Moreover, when references are simply shovelled into a text in great gobbets, it is hard to resist the suspicion that the author has not read them all, but has copied some from a previous author's list. The story is told of one author who mischievously included in his list a bogus reference to an obscure foreign journal, and gleefully noted its frequent appearance in future articles. One of the joys of this present book is that the number of references to each topic is very small. But these few have been selected with dis cretion and studied with care. Each group of references is followed by a critical assessment, written with balanced judgment and commendable brevity, and how refreshing it is to find authors who read much but write little. In fact, these authors have followed the pattern of the sister work, Selected References in Orthopaedic Trauma, published in 1989."

Rheumatoid arthritis demands intensive interdisciplinary cooperation between the orthopaedic surgeon and internist, and this monograph provides all the information necessary for the specialist dealing with inflammatory changes of the hip joint in rheumatoid arthritis. The reaction of the joint to rheumatoid arthritis is generally uniform, but the diagnoses may vary. For this reason, particular attention is paid to the differential diagnosis of the chronic inflammatory changes at the hip joint. The course of the disease is outlined, so that varying treatment approaches can be understood. The book describes the medical and the physical treatment of the hip joint, including discussion of the mechanisms of action and side effects that may occur. The options for conservative orthopedic treatment and for operative intervention are carefully considered. The broad spectrum of treatment principles, courses of action and results that are discussed will aid every physician interested in the rheumatoid hip.
Demands on a Neurosurgeon Under Routine Clinical Conditions' "Hardly any other field of surgery requires such meticulous asepsis. No other field requires such a protection of the tissue and such reliable hemostasis. No one will question that neurosurgery makes exceedingly high demands. However, the degree of the demands on the personal ity of the surgeon who has to carry out very serious operations to the exclusion of all others and has little opportuinity to recover psycholog ically by working on simpler cases is concealed from the outer world. The mental strain which the coworkers and staff have to tolerate may not be denied." So wrote Wilhelm Tonnis in 1939. Although fifty years have now elapsed, his appraisal is still relevant today - hence my intention to discuss the everyday demands placed on neurosurgeons. My aim is not self-glorification to engender sympathy; rather it is to highlight the effects and repercussions of such demands for the well-being of the patients entrusted to us. The major onerous demands include: - those in the operating theater - those resulting from staff problems in nursing - those resulting from legal developments - those resulting from the increasing administrative tasks that cost valuable time and energy which are lost to our actual work in look ing after patients: science and research also suffer from this. I shall not go into the latter point within this preface. Let me first turn to the strains occurring in the operating theater."

Why and Wherefore? Welcome to our little introductory book It appears in response to be ginning students (i.e., especially general radiology residents) who have sought guidance with the methodology of evaluating positional rela tionships of the feet from radiographs. Since my stepwise "how to" technique has been well received both in the Show-Me state (at the University of Missouri.Columbia) and here in Ohio, it is offered to you as well. To simulate my conventional teaching method, the informal text has an interactive flavor, which I hope makes it useful for you. Please don't be offended by a touch of simplified walking.through of material here and there. After all, this is a "primer," not a dignified postgraduate treatise. Comments from readers would of course be appreciated by the author. The majority of illustrations are the result of the collaboration with my dear colleague Tamar, in which she artistically interpreted repre sentative radiographs for you to my specifications. The guidelines I gave her included line drawing outlines of the most pertinent bones, to which straight lines were added as appropriate. As a result certain bones, for example the fibula and many phalanges, are omitted if they are unlikely to enhance the impact of the illustrations. Often, too, the cuneiforms and cuboid remain undifferentiated as one bony element, or only a few of them are drawn, so that attention remains with the key bones involved."
