The goal of tissue engineering is to repair or replace tissues and organs by delivering implanted cells, scaffolds, DNA, proteins, and/or protein fragments at surgery. Tissue engineering merges aspects of engineering and biology, and many rapid achievements in this field have arisen in part from significant advances in cell and molecular biology. Functional Tissue Engineering addresses the key issues in repairing and replacing load-bearing structures effectively. What are the thresholds of force, stress, and strain that normal tissues transmit or encounter? What are the mechanical properties of these tissues when subjected to expected in vivo stresses and strains, as well as under failure conditions? Do tissue engineered repairs and replacements need to exactly duplicate the structure and function of the normal tissue or organ? When developing these implants in culture, how do physical factors such as mechanical stress regulate cell behavior in bioreactors as compared to signals experienced in vivo? And finally, can tissue engineers mechanically stimulate these implants before surgery to produce a better repair outcome? Chapters written by well-known researchers discuss these matters and provide guidelines and a summary of the current state of technology. The reconstruction of articular cartilage defects is still one of the major challenges for the orthopaedic surgeon. A variety of surgical techniques such as microfracturing, transplantation of osteochondral grafts and the autologous chondrocyte transplantation have been developed over the last decades. Recent progress in material science and cell and tissue engineering has led to a worldwide increasing number of scientists working on cell-based articular cartilage repair strategies. In a workshop symposium held in Würzburg, Germany, in November 2002, leading scientists from biomedical engineering, basic science and specialized surgeons discussed the state of the art in "Cartilage Surgery and Future Perspectives".

Recent developments in surgical techniques have led to an increased demand for bone stock. Fusion enhancement, bone-void filling in tumor surgery and trauma are only a few examples. Autologous bone can be considered as the most successful bone graft material, and constitutes the accepted "gold standard", as it combines all the properties required from the bone graft. Low back pain is a common disorder in the clinical treatment of the Department of Orthopedics. Lumbar intervertebral disc degeneration is a main reason for the chronic pain and the process is difficult to reverse. Traditional treatment methods include conservative treatment and surgical treatment. Although the clinical symptoms caused by intervertebral disc degeneration can be alleviated to a certain extent, these treatment methods do not solve the fundamental issues and they also produce corresponding complications. The rise of tissue engineering technology and its applications in different fields have brought new ideas for the treatment of intervertebral disc degeneration. This book discusses the fundamentals as well as more recent developments in stem cell therapy and tissue engineering technology and offers an alternative for treating degeneration of intervertebral discs.
The objective of the present work is to review the existing literature on joint incongruity, cellular mechano-transduction, and computer simulations of mechano-adaptive bone remodelling, and to quantitatively assess the effect of incongruity on load transmission and subchondral mineralisation. Idealised computer models of incongruous joints and a specific anatomically based model of the humero-ulnar joint articulation were analysed with the finite element method, and the results directly compared with experimental and morphological data.
The main themes presented in this volume are exercise intolerance and muscle contracture. These two topics could seem different at first sight but share common clinical features. For instance, symptoms of exercise intolerance are myalgias and cramps but metabolic contractures as well. Likewise exercise intolerance is sometimes due to dystrophinopathy, it self causing contractures. Therefore, it is justified to gather these two syn dromes in one study. Exercise intolerance and muscle contracture could seem well known, even obsolete. It is not right. A reappraisal of these has be come necessary for two reasons. First of all there are many new causes of exercise intolerance; secondly, the pathophysiology remains ob scure in particular for muscle contracture and a clear classification is probably possible. There is no book devoted to exercise intolerance and muscle contrac ture. This volume is divided in two parts related to the above topics. The two sections are introduced by a didactic and general overview, describ ing the main symptoms, classification and classical etiology. The other chapters, written by the best experts, present the recent advances in these syndromes and show many new aspects: new glycogenosis or mitochondriopathies, dystrophinopathies, malignant hyperthermia, my otonic disorders. Pathophysiology and classification of persistent con tractures are described as well as therapy. Each chapter is thoroughly referenced, representing important progress in research in the field."
